Introduction

Purpose and scope

This document is intended for use by Industry Partners and Researchers in the development and negotiation of study budgets for all commercially sponsored clinical trials research to be conducted within Queensland Health Institutions. All budget negotiations are to reflect the requirements outlined in the Queensland Health Research Management Policy and Implementation Standards. http://www.health.qld.gov.au/ohmr/documents/res_man_pol_fram08.pdf

Section 1 of the document provides information relating to fees and charges for services provided for research projects undertaken within Queensland Health. It also provides guidance on application processes, contract requirements, correct nomenclature and preferred forms. Information in this section applies to all sites within Queensland Health.

Section 2 provides information on individual Health Service Districts, Human Research Ethics Committees, Research Governance Offices and other research related contacts within major hospitals.

All prices quoted are not inclusive of GST (10%). GST may be levied in accordance with the GST Legislation.

SECTION 1: Queensland Wide Fees, Charges and Policies

1 Ethics Applications

1.1 Application Format

Research applications undergoing full HREC review must be made on the NEAF. If using the NHMRC version of NEAF (https://www.neaf.gov.au/), the application must be converted into the “Infonetica” version of NEAF (https://ethicsform.org/au/) to enable uploading into the AuRED database. All supporting study documents must be electronically attached to the application. Instructions for performing these actions may be found in the Queensland Health Researcher User Guide: http://www.health.qld.gov.au/ohmr/documents/researcher_userguide.pdf.

1.2 Ethics Review Fee

HREC Review fee: $3000 (excluding GST)

2 Central Coordinating Service and Multi-Centre Research

Queensland Health has established a Central Coordinating Service (CSS) for all multi-centre research projects.

The Coordinating Principal Investigator must contact the CSS to enable allocation of the study to the appropriate reviewing Human Research Ethics Committee (HREC). The decision regarding which HREC will review the study is made in consultation with the CPI.

This service will also provide advice on supporting documentation requirements and will also assist with nomination of possible sites to participate in multi-centre research projects. For further information, please contact the Central Coordinating Service (CSS) at QH REGU.

3 Governance Applications

3.1 Application Format

Governance applications are made on the SSA (Site Specific Application) Form, generated out of the Infonetica NEAF. Governance authorisation is not issued until after the ethics approval is received. All relevant study documents must be electronically attached to the SSA Form. Site staff will complete this form. 3.2 Governance Review Fee

Application fee: $3000 (excluding GST)

3.3 Clinical Trial Agreements

3.3.1 Qld Health requests the use of the approved Clinical Trial Agreements as defined on the QH REGU webpage, “Step 2: Research Governance Processes”: http://www.health.qld.gov.au/ohmr/html/regu/for_researcher.asp

Where these preferred contracts are not used, study approvals will be delayed as legal review will be sought for all non standard contracts. These costs will be borne by the sponsor. Please refer to the Research Management Policy - Implementation Standard 4 for details. http://www.health.qld.gov.au/ohmr/html/regu/for_researcher.asp

3.3.2 Schedule 7 clauses must be reviewed and approved prior to their implementation. Non-approved Schedule 7 clauses intended for general use for all research projects should be submitted to Queensland Health Research Ethics and Governance Unit ([email protected]) to enable legal review and pre-approval. Approved Schedule 7 clauses remain valid for 12 months. If amendments are requested within this time, a fee will be levied for this service according to the complexity of the legal review.

“Project Specific” amendments to approved schedule 7 clauses will be submitted by study sites to the District Solicitor for review. A fee will be charged for this service according to the complexity of the legal review. Project specific amendments to Schedule 7 clauses are approved only for that study and do not affect current approved schedule 7 clauses for that company.

In the case of “one off, study specific” schedules, the lead site in multi-centre research will conduct the legal review for all QH sites.

3.3.3 Study budgets should automatically be adjusted for CPI on the anniversary of the date of the contracts’ signing. Sponsors are requested to provide a break down of per patient costs as part of the study budget.

3.4 Legal Name of Institution / Party to be Indemnified

The 'State of Queensland' is the contracting party for all QH agreements. The various state government departments (including QH) are not separate legal entities and cannot enter into contracts. Any wording which follows "The State of Queensland" is descriptive only and intended to assist the parties in identifying the relevant part / area / department within the State involved in the contract.

QH should be described on all research contracts as:

"The State of Queensland acting through Queensland Health (ABN:66329169412), (name of hospital/district) of (Address of Institution)".

3.5 Insurance Certificate Requirements QH

The Insurance Company must have a representative office in Australia to enable rapid review and settling of any claims. For sponsored clinical trials, the sponsor or CRO should indemnify Queensland Health against claims by patients arising from the study in terms consistent with the Medicines Australia Standard Forms of Indemnity: http://www.medicinesaustralia.com.au/pages/images/Form of Indemnity.doc

In addition, the Sponsor/Local Sponsor must have and maintain for the term of this Agreement and for a period of 6 years thereafter the following insurances:

(a) Clinical Trial/Product Liability insurance for an amount not less than $10m AUD per claim;

(b) Public liability insurance for an amount not less than $10m AUD per claim;

(c) Professional indemnity insurance for an amount not less than $10m AUD per claim;

(d) Workers compensation insurance in accordance with applicable legislation.

All insurance certificates must be valid at the time of submission for Governance review.

3.6 Overnight Hospital Stay Fee (Commercially Sponsored Clinical Trials)

If the protocol requires an overnight hospital admission - which is not a standard clinical requirement (eg an unregistered anti-arrhythmic drug requiring overnight monitoring after administration of the first dose. etc) - the standard fee for Medicare eligible patients will be charged to the sponsor company.

If an overnight stay is a requirement of standard clinical practice for the disease that is the subject of the trial, overnight accommodation fees may not be charged.

The standard Medicare reimbursement fees for private patients in a public hospital standard ward bed or other bed stay services is available from the “Queensland Health Fees and Charges Register” which can be accessed on the Queensland Health intranet by Queensland Health staff. Please note: There is a range of accommodation available. Fees should be negotiated as required. If study participants are ineligible under Medicare benefits, separate fees apply, depending on the level of service and interventions required.

3.7 Archiving and Storage of Study Documents

All study documents and folders are to be archived off site. Sponsor companies are requested to pay for archiving fees. Queensland Government has a "standing offer" arrangement for the provision of records storage, retrieval and destruction services. This information is Commercial in Confidence, and cannot be used for the purposes of obtaining quotes for the storage of research materials from commercially sponsored research. http://qcd.govnet.qld.gov.au/Pages/Details.aspx?SOANumber=QGCPO747-08

3.8 Medical record pulling charge

Qld Health does not have a formal policy for charging for the handling of Medical Records requested for the purposes of Research. However, a fee may be levied by individual sites and this fee should be covered by the Sponsor and noted in the Contract.

3.9 Parking Fees

Where study participants incur parking fees as a result of attendance at a hospital or clinic for the purposes of a clinical trial visit, the sponsor is requested to cover the cost of parking. Parking Fee invoices will be forwarded by study coordinators. Parking fees are considered an additional expense and should be listed separately from the “per patient” fee.

3.10 Travel Expenses for Patients

When travel expenses are incurred for travel specifically related to Clinical Trial participation, reimbursement of these expenses for study participants is requested. This may be in the form of taxi vouchers, public transport ticket reimbursement, or a "per/km" fee if private vehicles are used.

Details for participant travel reimbursement should be stated in the Clinical Trial Agreement and listed separately from the “per patient” fees.

4 Cardiology Fees

4.1 Non cardiology Research Projects

For non-cardiology research projects, where cardiology services are required as part of the research procedures, the fee schedule will be based on the MBS Schedule Fee + 20%, with the exception of any bespoke requirements.

4.2 Cardiology Research Projects

For cardiology research projects the fee schedule will be based on the MBS Schedule Fee + 20% with the exception of any bespoke requirements.

Depending on the complexity of the project, an annual administration fee may also be levied.

5 Commercially Sponsored Clinical Trials

Pre screening fees:

Chart/patient review prior to signing consent and only on $25 for each pre-screened possible receipt of pre-screening log signed off by P.I. (if applicable): participant recorded on the pre- (to be negotiated with site and determined according to screening log. type and complexity of study). To be re-negotiated after 250 pre-screened patients.

Screening Fee:

If a screening visit fee is not included in the study budget, Negotiated according to complexity of all procedures undertaken for screening for the purpose of Screening visit. potential trial recruitment must be paid (screening visit relates to the signing of the consent form).

If a study is extended for a protracted period of time the study budget may require renegotiation.

If the project is terminate early, the sponsor is required to cover full costs associated with the study preparation and conduct including any HREC and RGO fees dues.

* A major amendment is defined as an amendment to the protocol or any other supporting documentation, that is likely to affect to a significant degree: • the safety or physical or mental integrity of the participants of the trial • the scientific value of the trial • the conduct or management of the trial • the quality or safety of any investigational medicinal product used in the trial.

** A Minor amendment is defined as changes to the details of research that have no significant implications for participants or for the conduct, management or scientific value of the study and can be regarded as minor amendments (sometimes referred to as ”administrative amendments”). These amendments do not require review by a full HREC and can receive approval outside of scheduled HREC meeting. Examples as follows: • Correction of typographical errors in the protocol or other study documentation • Amended contact details for the sponsor or project staff • Appointment of new support staff

6 Health Support Queensland

Health Support Queensland (HSQ) delivers a wide range of diagnostic and scientific services to enable the delivery of frontline care. Pathology Queensland, Forensic and Scientific Services, and Biomedical Technology Services are placed within HSQ, which conducts analytical, clinical and forensic research in collaboration with Hospital and Health Services, other government agencies, universities, industry groups and specialist research organisations.

6.1 Forensic and Scientific Services

The Forensic and Scientific Services Human Ethics Committee (FSS-HEC) is constituted and functions in accordance with the NHMRC National Statement on Ethical Conduct in Human Research 2007 (NS).

The FSS-HEC is responsible for review of:

• use of human samples submitted to FSS for testing • use of data about human subjects held at FSS • access to coronial autopsies, including retrieval of tissue, for purposes other than the coronial investigation • review of FSS practices not related to research that raise ethical issues.

Depending on the complexity of the project, fees may be levied for services provided.

DISTRICT PROFILES

Cairns and Hinterland Health Service District

The Cairns and Hinterland Health Service District covers an area from Cardwell in the south, Mossman and Daintree in the north and Croydon in the west, encompassing an area of about 143000 km². The population of this area totals approx. 221,000, but as the referral hospital for the whole of Far , Cairns Base Hospital serves a broader population of about 250,000.

There is one HREC in this HSD: Single site research studies should be submitted to Cairns and Hinterland Health Service District Human Research Ethics Committee. Multi-centre research studies must be submitted through the QH Central Coordinating Service for allocation to a reviewing HREC.

The main hospital in the District is Cairns Base Hospital.

Cairns Base Hospital provides community and specialist hospital services for Cairns and its immediate surroundings and is the major referral centre for Tropical North Queensland. Services include all major health specialties (medicine, surgery, women’s health, paediatrics and mental health) and more than 30 sub-specialties. The hospital also is a major provider of outreach specialist services to remote and rural areas, including obstetrics and gynaecology, anaesthetics, orthopaedics, general medicine, renal medicine diabetes and thoracic medicine. Central Queensland Health Service District

The district includes the Capricorn Coast, Rockhampton, Gladstone, Banana and Central Highlands out to the Central Queensland gem fields and services a population pool of about 200,000 people.

The District is divided into four hubs: Banana Hub, Central Highlands Hub, Gladstone Hub and Rockhampton Hub. Darling Downs and West Moreton Health Service District

The DD-WMHSD covers approximately 95,000km² to the west of Brisbane, extending south from the NSW border to Wondoan in the north and Glenmorgan in the West. The Health Service District services a population of approximately half a million people, and was created 10 2008 from the merging of five health service districts. The two main cities in the district are Toowoomba and Ipswich.

The West Moreton South Burnett Health Service District (WMSBHSD) incorporates Ipswich, Boonah, Cherbourg, Esk, Kingaroy, Laidley, Murgon, Nanango and Wondai hospitals; and a tertiary mental health facility: The Park - Centre for Mental Health.

The main hospitals in the District are Toowoomba Hospital, Ipswich Hospital, and The Park – Centre for Mental Health.

There are two HREC’s in this HSD:

The Prince Charles Hospital (TPCH)

With a workforce of approximately 3500 staff, The Prince Charles Hospital provides health services to residents living in the northern suburbs of Brisbane and specialist services to the broader Queensland and northern New South Wales population.

Metro South Health Service District

The Metro South Health Service District is situated within the southern suburbs of Brisbane and extends from Brisbane River in the north, to the coast line and Bay islands in the east and to the border of New South Wales in the south west. It encompasses the following major health services: Princess Alexandra Hospital, Queen Elizabeth II Jubilee Hospital , Logan Hospital, Beaudesert Hospital, Redland Hospital, Wynnum Hospital and Dunwich Health Service Centre.

The main hospital in the District is the Princess Alexandra Hospital.

There is one HREC in the District – Metro South Health Service District HREC, based at Princess Alexandra Hospital. Princess Alexandra Hospital

(PAH) is one of three tertiary level facilities in Queensland, providing care in all major adult specialties, with the exception of obstetrics. It is one of Australia’s leading teaching and research hospitals.

The PAH provides acute medical, surgical, mental health, cancer, rehabilitation and allied health services as well as statewide services - including the Acquired Brain Injury Outreach Service, Queensland Amputee Limb Service, Spinal Outreach Team and the Transitional Rehabilitation Program.

It is nationally recognised for its expertise in spinal injury management and is a major transplantation centre for livers, kidneys, bone, cartilage, and corneas. The hospital has responsibility for the management of the Queensland Liver Transplant Service, Queensland Eye Bank and the Queensland Bone Bank. It also houses one of the major Pathology Queensland services.

The PAH has an aeromedical retrieval service transporting patients to hospital via helicopter or fixed wing aircraft, a road accident response service and a major trauma retrieval service managed by the Emergency Department and Intensive Care Unit.

Children’s Health Services District

The Royal Children's Hospital (CHSD ) is a 168 bed, tertiary referral paediatric teaching hospital with a number of specialties including burns, liver transplant, oncology, paediatric surgery, rehabilitation, respiratory medicine, and trauma services.

It is the largest paediatric tertiary referral hospital in Queensland and provides services to patients throughout the State, Northern New South Wales, Northern Territory, Japan, and from neighbouring countries in the South West Pacific. Located at Herston and close to the central business district of Brisbane, the Royal Children's Hospital shares the campus with the Royal Brisbane and Women's Hospital.

The Children's Health Services District is structured into the following Divisions: Division of Allied Health , Division of Corporate Services (District-wide service) , Division of Medical Services , Division of Surgical Services.
