Applications by mail Applications for certificates of Resident card (available for persons who have registered as Nisshin citizen) and of Family register (available for persons who have legal domicile in Nisshin City) can be made by mail. For detailed information, please refer to the Citizens Div. ●Issue of certificates of family register and of resident card Type Details and instructions Fee Copy of resident card The certificate of address, name,date of birth and sex. 300 yen per certificate Certificate of entries The certificate of contents registered in the resident card. 300 yen per certificate in resident card Certificate of registration Please bring a Seal registration notebook. Applications by mail are not 300 yen per certificate of personal seal available. In case of a representative’s applications, the seal registration notebook is required, but the power of attorney is not necessary. Resident card for The resident card issued for persons who live outside Nisshin City. 300 yen per certificate non-Nisshin City (Available for only persons who live in the municipalities connected to the residents Basic Resident Registration Network.) Notification card The card to notify the individual number. Available for free at the first 500 yen per certificate (re-issuance) issuance. Individual number card The card which can be used as document of personal identification. 800 yen per certificate (re-issuance) Available for free at the first issuance. Note: An electronic certificate costs 200 yen separately as the commission of re-issuance of the individual number card. Personal identification and confirmation of authority Personal identification is performed under a law in the applications for each certificate.Please present a document for confirming your identity at the service window. And in case a representative (a third party) makes applications for certificates, the power of attorney of the person who needs certificates is necessary. ※In case a representative (a third party) requests certificates of family register and ・exercises his or her right or fulfills his or her duty or ・needs to present organizations of national government or of local government, presentation of power of attorney is not required. In case of requests of other than ① and ②, a power of attorney is required. Please fill in the details such as place of submission or reasons for request when you apply. You’re requested to present prima facie documents (documents on which reasons for request are apparent objectively), when the contents are not clear. ● In case you need a resident card and a certificate of the registration of personal seal on the day you submit Moving-in notification, Notification of household change and Notification of moving-in from abroad, please come to the Nisshin City Hall as early as possible. ● Please understand the issue of certificates may not be possible on the same day in case of congestion. 15 National Health Insurance ( NHI ) People who are on medication by your employers or by the Late-Stage Medical Care System for the Elderly and who are on welfare, must enroll in National Health Insurance. Insurance Certificate Insurance Certificates are issued to each insured person. The insured must present this certificate to hospitals and medical institutions for treatment. The following persons listed below are also required to present other certificates. ・Persons aged between 70 and 74: The Insurance Certificate for the Elderly(You can use the Certificate from the next month after your birthday month. If your birthday is on the 1st day of the month, you can use it in that month.) ・For Physically or mentally challenged persons, Single-Parent households or children: Social Welfare Service Recipient Certificate for Physically or Mentally challenged persons, Single-Parent households or children National Health Insurance Tax( Insurance Premium for Health Care＋Insurance Premium for the Health Care System for the Elderly aged 75 and older＋Insurance Premium for Nursing Care) NHI Tax is calculated per household and is notified to each head of the household. Insurance Premium for Nursing Care should be paid by the insured persons aged between 40 and 64. Injuries caused by a third party’s conduct (including traffic accidents) ID No. : N21141 Date of update: Nov. 28. 2016 If you get injured due to conducts of a third party If you are injured in an accident or an assault case caused by a third party, a party at fault should pay for medical expense in principle. But, your treatment expense can be covered by NHI if you take necessary procedures except for the following cases; 1. You have already got medical expense from the party at fault. 2. You have got injured due to a disaster when commuting or while you were working. 3. Injuries caused by your own faults, such as drunken driving or driving without your license. Contact the following division urgently If you want your medical expense to be covered by NHI, please report to National Insurance Section within Insurance and Annuity Div. Nisshin City Hall. The division staff will tell you how to take necessary procedures following the simple hearing investigation. Reasons why you should notify Medical expense, in principle, should be paid by a party at fault if somebody gets injured due to conducts of a third party (a party at fault). If the injured gets medical treatment covered by NHI, NHI reimburse for the expense. Thus, please be sure to take necessary procedures to ask a party at fault to pay the expense which NHI reimbursed. Procedures should be taken within 14 days in the following cases ! ●When the applicant’s situation applies to any of the following cases, he/she should notify the Nisshin City Hall within 14 days. For procedures, the applicants are required to show My Number Card (Individual Number Card) or My Number Notification Card, and appropriate documents that can identify him/her. TEL: 0561-73-1450 FAX: 0561-72-4554 The public pension system (such as National Pension system) is designed to financially support our life. The public pension system provides the old-age pension when you become old. It also provides the disability pension or survivors’ pension when you have suffered unexpected incidents or you have lost the wage-earner in your household. Enrollment to National Pension Self-employed business owners, students, unemployed persons are included in the CategoryⅠ.If you have registered to reside in Japan, aged between 20 and 59, and you CategoryⅠinsured are not a CategoryⅡor CategoryⅢinsured person (see (2) and (3) below), you must persons enroll to National Pension system. Please be sure to make enrollment procedures at Insurance and Annuity Div. in Nisshin City Hall. (You need to pay the contributions. If your income is too low to pay contributions or have other reasons, you can apply for the exemption from payments.) If you are salaried workers at companies or civil servants, aged younger than 70, and CategoryⅡinsured are covered by employees’ pension or mutual-aid pension systems, you are a Category persons Ⅱinsured person. Enrollment procedures are taken at your workplace. (Your contribution is deducted from your salary and your employer will pay it for you.) If you are a dependent spouse of a CategoryⅡinsured person, and aged younger than CategoryⅢinsured 65, you are a CategoryⅢinsured person. (Enrollment procedures are taken at the workplace the CategoryⅡinsured person is working for. You don’t need to pay persons contributions. they are financially covered by the contributions paid by the CategoryⅡ insured persons.) Voluntary Enrollment to National Pension system The following persons who have not paid the National Pension contributions for at least 10 years (to be enforced from August.
