U.S. Department of Transportation FRA FACTUAL RAILROAD ACCIDENT REPORT FRA File # HQ-2019-1365 Federal Railroad Administration SYNOPSIS On September 25, 2019, at approximately 2:59 p.m., CDT, a southbound CSX freight train M97225 (Train 1) derailed at CSX Milepost (MP) J22 on the CSX Nashville Division, Chattanooga Subdivision in Smyrna, Tennessee. Train 1 was making a shoving movement with 85 loaded autoracks at a recorded speed of 6 mph, when the train coupled with a boxcar at an excessive speed. The excessive speed coupling caused the 33rd autorack in the train to derail. The Engineer and Conductor were unaware of the derailed equipment and proceeded northbound causing 31 additional autoracks to derail. All autoracks remained upright and parallel to the track. At the time of the derailment, it was daylight, cloudy and the temperature was 85 ºF. No injuries were reported initially; however, the Conductor reported being injured two days after the derailment. Damages reported by CSX were estimated to be $102,883 for equipment and $251,200 for track, totaling $354,083. FRA determined the probable cause of the derailment was H601 – Coupling speed excessive.

    U.S. Department of Justice National Drug Intelligence Center Atlanta High Intensity Drug Trafficking Area Drug Market Analysis 2011 Source Summary Statement The National Drug Intelligence Center (NDIC) has high confidence in this drug market analysis as it is based on multiple sources of information that have proved highly reliable in prior NDIC, law enforcement, and intelligence community reporting. Quantitative data, including seizure, eradication, and arrest statistics, were drawn from data sets maintained by federal, state, or local government agencies. Discussions of the prevalence and consequences of drug abuse are based on published reports from U.S. Government agencies and interviews with public health officials deemed reliable because of their expertise in the diagnosis and treatment of drug abuse. Trends and patterns related to drug production, trafficking, and abuse were identified through detailed analysis of coordinated counterdrug agency reporting and information. NDIC intelligence analysts and field intelligence officers obtained this information through numerous interviews with law enforcement and public health officials (federal, state, and local) in whom NDIC has a high level of confidence based on previous contact and reporting, their recognized expertise, and their professional standing and reputation within the U.S. counterdrug community. U.S. Department of Transportation FRA File #HQ-2018-1265 Federal Railroad Administration FRA FACTUAL RAILROAD ACCIDENT REPORT SYNOPSIS On May 1, 2018, at 3:21 p.m., EST, CSX Transportation (CSX) southbound freight train Q24129 (Train 1) derailed 2 locomotives and 19 loaded auto carriers at CSX Milepost (MP) 00026.7, on the Louisville Division in Lebanon Junction, Kentucky. Train 1 was operating in throttle notch 6 as the train speed increased to 57 mph when the crew noticed a severe track misalignment ahead. As Train 1’s locomotives traversed the track misalignment, they derailed and rolled onto their left side, with the head 19 cars derailing in a general pileup. CSX reported $1,282,600 in damage to equipment and $193,090 in damage to track and signal. Weather at the time of the accident was clear, with a temperature of 82 °F. FRA determined the probable cause of the accident was T109 – Track alignment irregular (buckled/sunkink).
