Il est l’inventeur de la méthode de Salomon qui consiste à couper le sandwich en deux parts égales pour les avaler simultanément. Malgré un record personnel de 63 hot dogs en 2007, il est battu cette année-là, Joey “Jaws” Chestnut en ayant mangé 66. Le leader actuel du classement mondial a gagné les 6 derniers concours et possède le record avec l'ingurgita- tion de 68 hot dogs. La championne féminine de ce concours est Sonya Thomas (48 kg) qui a réussi à avaler 40 sandwichs. Cette compétition est organisée sous l’égide de la Major League Eating (MLE) dans le cadre de l’International Federation of Competitive Eating (IFOCE), car il existe des compétitions dans d’autres pays que les États-Unis (Canada, Alle- magne, Grande-Bretagne, Japon, Thaïlande, Russie). Ces manifestations permettent d’associer deux traits spécifiques aux Américains : la nour- riture et la compétition. La fédération valide les records dans plusieurs dizaines de catégories : pizza, asperges (4 kg en 10 mn), beurre (800 g en 5 mn), poisson-chat (3,4 kg en 10 mn), œufs durs (65 en 6 mn 40 s), frites (3,6 kg en 10 mn), homard du Maine (5,1 kg de chair en 12 mn), huîtres (46 douzaines en 10 mn), pâtes (3 kg en 10 mn), etc. (voir la liste sur le site http://www.ifoce.com/). Le troisième du classement actuel de la MLE est Tim Janus, alias “Eater X.”, qui se produit avec un masque peint sur le visage. S’il n’est pas le premier du classement, c’est tout de même un concurrent sérieux capable d’avaler en 10 mn 3,8 kg de poutine (mets québécois composé de pommes de terre frites garnies de fromage en grains et arrosées d’une sauce brune chaude) ! Ce sujet de 1 m 78 pour 75 kg a fait l’objet d’une étude “scientifique” (1) qui l’a comparé à un sujet témoin choisi pour son appétit solide et ses mensurations de 1 m 88 pour 95 kg. Les deux sujets ont tout d’abord subi un test de surcharge en eau décrit afin d’étudier les sujets dyspeptiques. Ce test a montré des capacités supérieures à la normale dans les deux cas. Ils ont eu ensuite une étude scintigraphique de la vidange gastrique aux solides montrant que 75 % du repas avait été évacué de l’estomac 2 h après chez le témoin, mais seulement 25 % chez Eater X. Le test consistait ensuite à ingurgiter le maximum de hot dogs en 12 mn. Les sujets avaient tout d’abord une radiogra- phie de l’estomac en double contraste permettant de visualiser un estomac de taille normale dans les 2 cas. Ils avalaient ensuite les hot dogs avec une surveillance fluoroscopique intermittente et des clichés pendant et en fin de test. L’aspect visuel de leur abdomen était observé. Le sujet témoin ne pouvait avaler plus de 7 hot dogs et son abdomen n’était pas modifié. Radiologiquement, on notait une accumulation progressive des hot dogs avec une dilatation gastrique minime. Eater X. avalait 2 hot dogs à la fois de façon régulière. À la 6e minute, l’estomac apparaissait comme un sac dilaté, sans péris- taltisme. À 10 mn, il avait avalé 36 hot dogs (image) et l’expérience était arrêtée par les médecins inquiets de la Image. Estomac distendu par 36 hot dogs à 10 mn distension abdominale simulant une grossesse avancée (d’après 1). alors que le sujet voulait continuer, ne ressentant aucune satiété, nausée ou douleur. Cette étude et l’interroga- toire de compétiteurs montrent que ces mangeurs de compétition sont capables de dilater leur estomac et n’ont pas de vidange gastrique accé- lérée. Cette faculté s’obtient par l’entraînement en ingérant des volumes de plus en plus importants, souvent de l’eau, pour éviter les calories. Après une compétition, les compétiteurs professionnels ne mangent pas pendant plusieurs jours, le temps que leur estomac reprenne un volume normal. Dans la vie courante, ils n’ont jamais de sensation de satiété et doivent surveiller strictement leur apport calorique pour ne pas grossir. On ne connaît pas le devenir de ces compétiteurs. Il n’a pas été décrit de rupture gastrique, de syndrome de Mallory-Weiss ou de Boerhaave, et l’estomac de ces sujets semble conserver ses capacités de vidange après quelques années de compétition.
