This directory provides a list of Banner – University Care Advantage’s current network providers. This directory is for the following counties in Arizona: Maricopa, Pinal and Gila. To access Banner – University Care Advantage’s online provider directory, you can visit www.BannerUCA.com. For any questions about the information contained in this directory, please call our Customer Care Center at (877) 874-3930, 8 a.m. to 8 p.m., 7 days a week. TTY users should call 711. I Section 1 – Introduction This directory provides a list of Banner – University Care Advantage network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage (EOC). You will have to choose one of our network providers listed in this directory to be your Primary Care Provider (PCP). The term PCP will be used throughout this directory. Generally, you must get your health care services from your PCP. Your PCP is a physician or nurse practitioner who meets the state requirements and is trained to give you basic medical care. You will receive your routine or basic care from your PCP. Your PCP will also coordinate the rest of the covered services you get as a member of our plan. For example, in order for you to see a specialist, you usually need to get your PCP’s approval first (this is called getting a “referral” to a specialist). The network providers listed in this directory have agreed to provide you with your health care, vision and dental services. You may go to any of our network providers listed in this directory; however, some services may require a referral. If you have been going to one network provider, you are not required to continue to go to that same provider. In some cases, you may get covered services from non-network providers. Other physicians / providers are available in our network. Some specialists might ask for a referral from your PCP before they will agree to see you. The specialist needs to have an understanding of your medical history and the conditions the specialist will need to treat. This information prepares the specialist to provide you with the most efficient and highest quality care. If you receive a bill from a non-contracted provider, do not pay the bill. Send the bill to Banner – University Care Advantage in order for us to process and determine if you have any liability for the bill. When you have a “medical emergency”, you believe that your health is in serious danger. A medical emergency can include severe pain, a bad injury, a sudden illness or a medical condition that is quickly getting much worse. If you have a medical emergency: Get help as quickly as possible. Call 911 for help or go to the nearest emergency room, hospital or urgent care center. Call for an ambulance if you need it. You do not need to get approval or a referral first from your PCP, even if you are out of the service area. As soon as possible, make sure that Banner – University Care Advantage has been notified about your emergency. We need to follow up on your emergency care. You or someone else should call to tell us about your emergency care, usually within 48 hours. You can find the number to call on the back of your plan membership ID card or on the back cover of this document. II If you receive emergency care at an out-of-network hospital and need inpatient care after your emergency condition is stabilized, you must have your inpatient care at the out-of- network hospital authorized by the plan and your cost is the cost sharing you would pay at a network hospital. Emergency care can always be obtained in or out of the service area from the nearest available provider. When in the service area, you must use plan providers for urgent care. When out of the service area, urgent care may be obtained from the first available provider. In addition, when out of the service area, you can obtain dialysis treatment from any qualified dialysis provider. You must use network providers except in emergency or for out-of-area urgent care situations or for out-of-area renal dialysis services. If you obtain routine care from out-of-network providers, neither Medicare nor Banner – University Care Advantage will be responsible for the costs. Its author was Sir George Hill, who had studied the collection in depth when it was still T in Dreyfus' hands in the Boulevard Malesherbes in Paris. In a prefatory note, Hill observed that 'keenly as Gustave Dreyfus appreciated all his beautiful things, he had a particularly soft place in his heart for the Italian medals, and ... he would have agreed with the German critic who declared that the medallic art was par excellence the art of the Renaissance, the expression of the quintessence of the spirit of that age.' The preface continues with the tribute: 'His was perhaps the finest collection that has ever been in the hands of a private collector - the "perhaps" might be omitted, but that it is difficult to range the great collections in a true perspective.' Thanks to the Kress Foundation, the Dreyfus collection of medals was not dispersed, like so many other medallic collections, but is preserved intact in the National Gallery of Art in Washington, where it bears out Hill's claim to be regarded as the finest private collection of medals ever to have been formed.

    Introduction Steve Worboys and I began this calendar in 1980 or 1981 when we discovered that the exact dates of many events survive from Roman antiquity, the most famous being the ides of March murder of Caesar. Flipping through a few books on Roman history revealed a handful of dates, and we believed that to fill every day of the year would certainly be impossible. From 1981 until 1989 I kept the calendar, adding dates as I ran across them. In 1989 I typed the list into the computer and we began again to plunder books and journals for dates, this time recording sources. Since then I have worked and reworked the Calendar, revising old entries and adding many, many more. The Roman Calendar The calendar was reformed twice, once by Caesar in 46 BC and later by Augustus in 8 BC. Each of these reforms is described in A. K. Michels’ book The Calendar of the Roman Republic. In an ordinary pre-Julian year, the number of days in each month was as follows: 29 January 31 May 29 September 28 February 29 June 31 October 31 March 31 Quintilis (July) 29 November 29 April 29 Sextilis (August) 29 December. The Romans did not number the days of the months consecutively. They reckoned backwards from three fixed points: The kalends, the nones, and the ides. The kalends is the first day of the month. For months with 31 days the nones fall on the 7th and the ides the 15th. For other months the nones fall on the 5th and the ides on the 13th.

The School-master was again printed in 1571, 1573, 1579, 1583, 1589, these editions vary very little the one from the other. It was again published, with notes by the Rev. Creighton University [email protected] It has seemed opportune to republish this index in a corrected and extended version; there has been added to the list of names a list of selected words, and the method of referring to the text has been somewhat altered. The exact scope of C.H. Turner’s masterpiece is not immediately apparent from its title or subtitles (Canonum et conciliorum graecorum interpretationes latinae; Canones et concilia graeca ab antiquis interpretibus latine reddita). What one in fact finds here is an edition of the earliest surviving collections of church canons in Latin which are undoubtedly from the fourth century or have been assigned to it by at least some consensus of scholarship; some of them are translations from Greek, while others were originally drafted in Latin itself. Most of them, with the exception of the Apostolic Constitutions and Canons, are attributed to church councils. Turner, however, also offers a selection of doctrinal and historical material of enormous value, some of it indeed connected to the councils whose canons he edits; a comparison, for instance, of the creed contained in the synodical letter of the Council of Sardica with the Tomus Damasi, will suggest how far the doctrinal education of the western church advanced during the course of the fourth century. The connection of other historical matter to the canons is less obvious, however; one wonders what the Athanasian Historia acephala is doing here (granted that it is part of the collection of Theodosius the Deacon), however grateful one is to have the edition.

    Flying Buttress and Pointed Arch in Byzantine Cyprus Charles Anthony Stewart Though the Byzantine Empire dissolved over five hundred years ago, its monuments still stand as a testimony to architectural ingenuity. Because of alteration through the centuries, historians have the task of disentangling their complex building phases. Chronological understanding is vital in order to assess technological “firsts” and subsequent developments. As described by Edson Armi: Creative ‘firsts’ often are used to explain important steps in the history of art. In the history of medieval architecture, the pointed arch along with the…flying buttress have receive[d] this kind of landmark status.1 Since the 19th century, scholars have documented both flying buttresses and pointed arches on Byzantine monuments. Such features were difficult to date without textual evidence. And so these features were assumed to have Gothic influence, and therefore, were considered derivative rather than innovative.2 Archaeological research in Cyprus beginning in 1950 had the potential to overturn this assumption. Flying buttresses and pointed arches were discovered at Constantia, the capital of Byzantine Cyprus. Epigraphical evidence clearly dated these to the 7th century. It was remarkable that both innovations appeared together as part of the reconstruction of the city’s waterworks. In this case, renovation was the mother of innovation. Unfortunately, the Cypriot civil war of 1974 ended these research projects. Constantia became inaccessible to the scholarly community for 30 years. As a result, the memory of these discoveries faded as the original researchers moved on to other projects, retired, or passed away. In 2003 the border policies were modified, so that architectural historians could visit the ruins of Constantia once again.3 Flying Buttresses in Byzantine Constantia Situated at the intersection of three continents, Cyprus has representative monuments of just about every civilization stretching three millennia.
