To get detailed information about your health care coverage, please see your Evidence of Coverage (EOC). You will have to choose one of our network providers listed in this directory to be your Primary Care Provider (PCP). The term PCP will be used throughout this directory. Generally, you must get your health care services from your PCP. Your PCP is a physician or nurse practitioner who meets the state requirements and is trained to give you basic medical care. You will receive your routine or basic care from your PCP. Your PCP will also coordinate the rest of the covered services you get as a member of our plan. For example, in order for you to see a specialist, you usually need to get your PCP’s approval first (this is called getting a “referral” to a specialist). The network providers listed in this directory have agreed to provide you with your health care, vision and dental services. You may go to any of our network providers listed in this directory; however, some services may require a referral. If you have been going to one network provider, you are not required to continue to go to that same provider. In some cases, you may get covered services from non-network providers. Other physicians / providers are available in our network. Some specialists might ask for a referral from your PCP before they will agree to see you. The specialist needs to have an understanding of your medical history and the conditions the specialist will need to treat. This information prepares the specialist to provide you with the most efficient and highest quality care. If you receive a bill from a non-contracted provider, do not pay the bill. Send the bill to Banner — University Care Advantage in order for us to process and determine if you have any liability for the bill. When you have a “medical emergency”, you believe that your health is in serious danger. A medical emergency can include severe pain, a bad injury, a sudden illness or a medical condition that is quickly getting much worse. If you have a medical emergency: Get help as quickly as possible. Call 911 for help or go to the nearest emergency room, hospital or urgent care center. Call for an ambulance if you need it. You do not need to get approval or a referral first from your PCP, even if you are out of the service area. As soon as possible, make sure that Banner — University Care Advantage has been notified about your emergency. We need to follow up on your emergency care. You or someone else should call to tell us about your emergency care, usually within 48 hours. You can find the number to call on the back of your plan membership ID card or on the back cover of this document. If you receive emergency care at an out-of-network hospital and need inpatient care after your emergency condition is stabilized, you must have your inpatient care at the out-of- network hospital authorized by the plan and your cost is the cost sharing you would pay at a network hospital. Emergency care can always be obtained in or out of the service area from the nearest available provider. When in the service area, you must use plan providers for urgent care. When out of the service area, urgent care may be obtained from the first available provider. In addition, when out of the service area, you can obtain dialysis treatment from any qualified dialysis provider. You must use network providers except in emergency or for out-of-area urgent care situations or for out-of-area renal dialysis services. If you obtain routine care from out-of-network providers, neither Medicare nor Banner — University Care Advantage will be responsible for the costs. To find providers in your area, begin by looking for the county in which you live, then for the name of your city or town and then the provider’s name. Its author was Sir George Hill, who had studied the collection in depth when it was still T in Dreyfus' hands in the Boulevard Malesherbes in Paris. In a prefatory note, Hill observed that 'keenly as Gustave Dreyfus appreciated all his beautiful things, he had a particularly soft place in his heart for the Italian medals, and ... he would have agreed with the German critic who declared that the medallic art was par excellence the art of the Renaissance, the expression of the quintessence of the spirit of that age.' The preface continues with the tribute: 'His was perhaps the finest collection that has ever been in the hands of a private collector - the "perhaps" might be omitted, but that it is difficult to range the great collections in a true perspective.' Thanks to the Kress Foundation, the Dreyfus collection of medals was not dispersed, like so many other medallic collections, but is preserved intact in the National Gallery of Art in Washington, where it bears out Hill's claim to be regarded as the finest private collection of medals ever to have been formed.

  • Calendar of Roman Events
    Introduction Steve Worboys and I began this calendar in 1980 or 1981 when we discovered that the exact dates of many events survive from Roman antiquity, the most famous being the ides of March murder of Caesar. Flipping through a few books on Roman history revealed a handful of dates, and we believed that to fill every day of the year would certainly be impossible. From 1981 until 1989 I kept the calendar, adding dates as I ran across them. In 1989 I typed the list into the computer and we began again to plunder books and journals for dates, this time recording sources. Since then I have worked and reworked the Calendar, revising old entries and adding many, many more. The Roman Calendar The calendar was reformed twice, once by Caesar in 46 BC and later by Augustus in 8 BC. Each of these reforms is described in A. K. Michels’ book The Calendar of the Roman Republic. In an ordinary pre-Julian year, the number of days in each month was as follows: 29 January 31 May 29 September 28 February 29 June 31 October 31 March 31 Quintilis (July) 29 November 29 April 29 Sextilis (August) 29 December. The Romans did not number the days of the months consecutively. They reckoned backwards from three fixed points: The kalends, the nones, and the ides. The kalends is the first day of the month. For months with 31 days the nones fall on the 7th and the ides the 15th. For other months the nones fall on the 5th and the ides on the 13th.

OF the first work contained in this volume, "A Report and Discourse of the Affairs and State of Germany, <kc" I have seen but one edition afac-simile separate ; of its title is prefixed to our reprint of the work. It is in small quarto, and has no date; but it is known to have been printed in 1552, and again in 1570. The copy which I have seen, is in the British Museum, and has furnished, besides many smaller cor rections, more than one whole sentence that had been omitted in the last edition of the English Works, 8vo, 1815. 2. The School-master also is here printed from the " English Works of Roger Ascham," collated through out with the earlier editions, which have furnished several important corrections of the text. This work was first published by Mrs Ascham, small 8vo, 1570, after her husband's death : a fac-simile of IV PREFACE. the title to that edition is prefixed to the work in the present reprint. The School-master was again printed in 1571, 1573, 1579, 1583, 1589, these editions vary very little the one from the other. It was again published, with notes by the Rev. James Upton, London, 8vo, 1711, and reprinted 1743.
