WORKING WITH YOUR SCHOOL BOARD WELCOME to another Twin Falls School District Board of Trustees Meeting. You are encouraged to attend all regular board meetings.

We are proud of our school system and the board members are dedicated to the continued improvement of the Twin Falls Schools.

We understand that from time to time patrons of school districts have concerns and feel a need to bring those concerns to the attention of the school or teachers. In order to maintain continuity and best resolve these concerns, we recommend to our patrons the following procedures:

1. The first and most effective step is to take the concern to the staff member who is closest to the problem. (In many cases this will be the teacher, coach, etc.) We have found that most problems are resolved at this level.

2. If there is still a concern on the matter, we then recommend that the concern be brought to the attention of the Principal. We have found that unresolved concerns from item #1 are usually resolved satisfactorily by the building administrator.

3. If the concern is not satisfactorily resolved at that level, then the concern should be taken to the Director of Elementary Programs or Director of Secondary Programs.

4. If the concern is not satisfactorily resolved at that level, then the concern should be taken to the Superintendent of Schools. Difficult concerns can usually be resolved.

5. However, if the patron still feels the concern has not been properly resolved he/she may use the right of appeal to the Board of Trustees. This is done by calling or writing to the Superintendent and asking for the item to be placed on the agenda of the next regularly scheduled board meeting.

6. If you desire to address the board, we would like you to be as comfortable as possible. As a board we hold public meetings which are not public forums and therefore have rules which are necessary for the benefit of all. The following guidelines will assist you as you present your information to the board.

A. Prepare your thoughts ahead of time. Usually a brief written outline handed to each board member helps the board follow your presentation. This outline would also give them something upon which to make notes as you speak.

B. If a group is involved, select one individual to be your spokesperson. He/she can then guide the presentation with the board and help the board chairman in directing questions to the group.

C. Plan your presentation to be as brief as possible. This should include a question/answer period. If more time is needed it is best to give a complete written presentation to the board for their future study. If you give more than five to six minutes of testimony, time restraints will make it difficult for the board to respond that same evening.

D. Usually the board will direct the administration to help resolve the concerns and ask for a report back to the board at a later date. However, please remember a solution may take time to be resolved.

We are eager to have our patrons and school staff working together for the improvement of education in the Twin Falls Schools.

I. Call to Order - The Board of Trustees of School District No. 411, Twin Falls County, State of Idaho, met in its regular monthly meeting at Canyon Ridge High School Boardroom #301 located at 300 North College Road, Twin Falls, Idaho, at the hour of7:00 p.m., on Monday, May 11, 2015. The following trustees were present: Chairman Bernie Jansen, Vice-Chairman Bryan Matsuoka, and Trustees Mary Barron, Paul McClinton and Liyah Babayan. There were also present Superintendent Wiley Dobbs; Clerk Michelle Lucas; Director of Human Resources Shannon Swafford; Director of Federal Programs, Policy & Grants Bill Brulotte; Director of Elementary Programs Ted Popplewell; Director of Secondary Programs L. T. Erickson; and Director of Educational Technology/Operations Brady Dickinson; as well as interested patrons, employees and the media.

VI. Superintendent's Report

AIB. Superintendent Dobbs announced the certified employee of the month from Canyon Ridge High School for the month of May as Kim Draves counselor and certified employee of the month from Twin Falls High School as Sharee Hamilton, counselor.

CID. Superintendent Dobbs announced the classified employee of the month from Twin Falls High School as Carolyn Weister, registrar and classified employee of the month from Canyon Ridge High School as Robyn Weatherford, registrar. Superintendent Dobbs read a brief resume' and presented Ms. Weatherford, Ms. Weister, Ms. Draves, and Ms. Hamilton each with a gift, plaque, and dinner for two.

E. Michele Dameron with Western States Bus Company presented checks for three $500 scholarships (one for each TFHS, CRHS, and MVHS), a library grant for $100, and an elementary grant of $500. Mr. Dickinson introduced Barry Stock and Dave Lear, new shareholders of Western States Bus Service. Tony Barnhart will be retiring in August. Mr. Dave Lear will take over as President/CEO of Western States Bus Service.

F. Building Goal Reports

1. Principal of Harrison Elementary School Melissa Ardito began her report with building successes including closing academic and opportunity gaps. Ms. Ardito stated that they have received several grants; they have had a decrease in behavioral issues, she feels this is because the culture and environment are so positive; they have many technology opportunities; and they have a lot of family involvement. Ms. Ardito shared challenges which include social and emotional needs of students due to the lack of early child resources before they enter kindergarten. After a few questions from the board Ms. Ardito thanked the board for the honor to share her building goals.

2. Principal of Perrine Elementary School Tammy Rodabaugh began her report by sharing their objectives and what they use to obtain those objectives. A couple of Perrine's goals are to achieve 96.6% in all measures and to improve student's soft skills. Ms. Rodabaugh is proud of her staff in finding resources to help, 64.1 % of Perrine faculty applied for grants, 52% of those applications were awarded their grant. Challenges include retention of classified staff with 54% turnover this year. Her staff teamed up with Oregon Trail and Bickel Elementary School staff to participate in "Playworks" training for more organized play at recess. Ms. Rodabaugh thanked Sandy Beams, Charlie McGlochlin, Mickey Combs, Ted Popplewell, Michelle Lucas, Naomi Briggs, Sheila Rinehart, Board of Trustees, Shannon Swafford and her team for all their support through her first year.

G. The Twin Falls High School Finance Academy has been recognized nationally once again. In the National Academy Foundation annual assessment of programs across the nation, the 7477

TFHS Finance Academy achieved the highest rating possible. Our finance academy earned "Model Status" and will be highlighted at the National Academy Foundation Conference in Anaheim, California in June. TFHS Principal Dan Vogt added that the team just returned from competition and they earned the top spot in the nation. Congratulations Twin Falls High School Finance Academy!

VIII. Budget & Finance A. Information Items:

1. Director of Fiscal Affairs Bob Seaman shared the 2015-2016 preliminary budget report which included an 11.2 unit increase from 2014-2015. He shared that the district hasn't been able to address curriculum needs for some time now and needs some attention for math in both elementary and secondary levels. Mr. Seaman added, hopefully funding will increase to make up what was robbed during the great recession. Mr. Seaman commented on the salary base apportionment and the changes with new laws that will take place over the next five years. Currently the district spends 85% of their budget on salaries, as the state increases funding, the district will be able to decrease that percent. Mr. Seaman commented that the group with the biggest reimbursement trouble is classified employees, state funding is abysmal at best. Mr. Seaman ended his report on a positive note, Idaho Power announced a zero percent increase this year.

2. Director of Elementary Programs Ted Popplewell and Director of Secondary Programs L. T. Erickson updated the board on how leadership premiums have been allocated over the past year and highlighted the work people are currently doing.

3. This meeting has been posted as a hearing for a recommended school lunch fee increase. The fee increase request is due to an increase of food and non-food costs, salary and benefits increase, and Federal Government regulations for the 2015-16 school year. Federal Law Section 205 states: "Effective school year beginning July 1, 2011, schools are required to charge students for paid meals at a price that is on average equal to the difference between free meals reimbursement and paid meal reimbursement. Schools that currently charge less (such as our school district average - $2.70 for FY15 and the State average is $3.00) are required to gradually increase their prices over time until they meet the requirement." If the board accepts this increase our average fee will rise to $2.80, which is still below the State average by $0.20. There was no one to address the board at this hearing, administration recommends approval of this fee increase.

4. Director of Operations and Educational Technology, Brady Dickinson shared the results of the online land sale auction which closed May 6, 2015. Mr. Dickinson recommends accepting bids on two pieces that were over appraised value, Bohr Farm property and lot number two of the Sacket Farm property and reject all others for they did not at least meet appraised value.

1./2. Vice Chairman Matsuoka moved to set the 2015-2016 budget hearing for June 8, 2015 at 7:45 p.m. and the 2014-2015 amended budget hearing for 8:00 p.m. Trustee McClintock seconded the motion. Motion passed unanimously.

3. With no other questions and no patrons to testify, Vice Chairman Matsuoka moved to approve the increase to school lunch fees as recommended. Trustee Barron seconded the motion. Motion passed unanimously.

4. With no other questions or concerns, Vice Chairman Matsuoka moved to approve administrations recommendation to accept the bid for the 30.41 acre Sacket lot in the amount of $912,300 and bid for the 36.68 Bohr farm lot in the amount of$641,900 and reject the bids for all five Riverhawk lots and the 8.75 acre Sacket Farm lot and to direct administration to begin negotiating with interested buyers. Trustee McClintock seconded the motion. Motion passed unanimously.

IX. Personnel A: Information Items:

1. Director of Federal Programs, Policy and Grants, Bill Brulotte informed the board that all of the required policies up for annual review as listed in policy 3720 School Climate have been reviewed by the appropriate committees and there are no recommended changes at this time.

C. The ISBA Summer Leadership Institute is set from 1:00 p.m. to 6:00 p.m. on July 15, 2015 at Jerome School District Office.

Since there was no other business to come before the Board, Trustee McClintock moved that the Board retire into executive session in accordance with Idaho Code 67-2345 subsection (b) to consider the evaluation, dismissal or disciplining of, or to hear complaints or charges brought against, a public officer, employee, staff member or individual agent, or public school student; and (c) to conduct deliberations concerning labor negotiations or to acquire an interest in real property which is not owned by a public agency. Vice Chairman Matsuoka seconded the motion. Chairman Jansen asked the Clerk of the Board Michelle Lucas to poll the board. She reported all members voted "yes" and the motion carried. Trustee Barron moved to approve terminations for classified employees A, B, and C as recommended by administration. Vice Chairman Matsuoka seconded the motion. Motion passed unanimously.

Vice Chairman Matsuoka moved to uphold the hearing officer and administrations recommendation to expel three (3) students from school for "time served" and one (1) student from school for an indefinite period and that these students be expelled from school as defined in Idaho Code 33-205 and that these students be placed under the purview of the Juvenile Correction Act, Idaho Code 20- 510 and 20-511. Trustee Barron seconded the motion and was passed unanimously.

Trustee Barron moved to adjourn. Trustee Babayan seconded the motion. Motion carried. The time was 9:34 p.m.

Call to Order - The Board of Trustees of School District No. 411, Twin Falls County, State of Idaho, met in a special open session at the District Office Board room located at 201 Main Avenue West, Twin Falls, at the hour of2:00 p.m., on Tuesday, May 12, 2015. The following trustees were present: Chairman Bernie Jansen, Vice Chairman Bryan Matsuoka, and Trustee Mary Barron. Superintendent Wiley Dobbs; Clerk Michelle Lucas; Director of Operations/Educational Technology Brady Dickinson; Director of Human Resources Shannon Swafford; and Human Resources Manager Sonia DeLeon were also present.

1. Chairman Jansen asked for a motion to approve the agenda. Trustee Barron moved to approve the agenda adding a consent calendar due to time sensitivity. Vice Chairman Matsuoka seconded the motion. The motion passed unanimously.

Trustee Barron moved to approve the consent calendar which included a new teacher contract for Kevin Cato, and extra-curricular employee Kevin Cato as Girls Head Basketball coach. Chairman Jansen seconded the motion. Motion passed unanimously.

IV. Budget and Finance A. Information Item

1. Director of Operations/Educational Technology Brady Dickinson shared negotiated counter offers on all but one piece of property that the board rejected auction bid results on at their May 11, 2015 board meeting. There may be a reasonable offer on that piece shortly. The recommendation is to accept the counter offers as proposed: Riverhawk lot #1 to Gerald Martins for $6.30 per square foot for a total of $238,707; Riverhawk lot #2 to Gerald Martins for $6.30 per square foot for a total of $200, 182.50; Riverhawk lot #3 to China House for $6.30 per square foot for a total of $210,483; Riverhawk lot #5 to Capital Educators for $7.61 per square foot for a total of $345,188.81; and Sackett Farm property #2 to Gerald Martins for $33,500 per acre for a total of $293,124.98.

1. With no further questions Vice Chairman Matsuoka moved to approve all six counter offers as proposed. Trustee Barron seconded the motion. Motion passed unanimously. 7481

Seven Schools Achieve "Schools of Excellence" Status

The Idaho High School Activities Association has announced the 2014-2015 winners of the "Schools of Excellence" initiative, a year-long , voluntary program in which schools earn points based on their student-athletes' performance in the classroom , at the athletic venue, in activities' competitions and in the area of citizenship and sportsmanship. The participating schools in each classification with the best overall programs will be recognized at the IHSAA Annual Meeting, August 5th . These schools will be presented with a plaque, sponsored by the 4th District Coaches Association.

The three major areas of high school activities are weighted equally in determining recipients of the award. ~ The Academic Component is based on the varsity team's cumulative GPA as submitted on the State Academic Champions application for all IHSAA sponsored sports/activity programs. ~ The Performance Component is based on place of finish at IHSAA-sponsored state championships and activity program competitions. ~ The Citizenship Component is based on a school's self-evaluation of their citizenship/sportsmanship activities as submitted on an itemized form . Deductions are made by the IHSAA for player and/or coach unsportsmanlike ejections and also for unsportsmanlike school behavior at state tournaments.

In addition to the primary awards, schools that score 90% on the Citizenship Component and have no player/coach ejections throughout the year or incidents of unsportsmanlike school behavior at state tournaments will receive an "Award of Excellence" certificate. IHSAA Selects Five Interscholastic Stars

Boise, Idaho ... Five Idaho high school juniors have been named as the 2015 recipients of the Interscholastic Star Student Recognition Scholarships, which are awarded annually by the Idaho High School Activities Association and its state championships sponsor, The United Dairymen of Idaho.

Tianna Leitch, Nezperce High School; Johnny Lancaster, Wendell High School; Amree Bean, Sugar-Salem High School; Kennedy Jones, Twin Falls High School; and Nathan Marshall, Boise High School were selected from a field fifty-eight outstanding students who were nominated by their school principals. Other finalists were Brandon Higgins, Prairie High School; Darby Finnegan, Grangeville High School; Jase Roberts, Weiser High School; Samuel Seamons, Preston High School; and Aaron Smith, Eagle High School.

Nominees for the scholarships are exemplary students who participate in IHSAA-sponsored sports and activities. Each application includes an essay, recommendations from school administrators, description of athletic and activity participation, and service to school and community.

Interscholastic Stars will receive a $1000 scholarship to the college of his/her choice, and will be recognized at the IHSAA Hall of Fame banquet in Boise on August 5. The IHSAA, with the support of the United Dairymen ofldaho, will also contribute $1000 to the recipients' schools and $500 to the finalists' schools.

Tianna Leitch, daughter of Drew and Marietta Leitch, participates in volleyball, basketball and track Nezperce High School. She is also active in band and student government, currently serving as ASB Vice President. Tianna is involved in BPA, National Honor Society, FCCLA and HOSA. Johnny Lancaser, son of Kevin and Laurie Lancster, participates in football, basketball and track as well as band at Wendell High School. Other activities include National Honor Society, BPA, Natural Helpers and Boy Scouts of America.

Amree Bean, daughter of Brent and Gaylynn Bean, has participated in volleyball, and currently participates in soccer, basketball and track at Sugar-Salem High School. Amree is also a member of Sugar-Salem's choir and band along with student government. Kennedy ones, daughter of Kevin and im Jones, participates in golf, choir and drama at Twin alls High School. She has also participated in debate and currently participates in Key Club, BPA and IDFY at school and is heavily involved in community theater in the Magic Valley.

Nathan Marshall, John and Shannon Marshall, participates in cross country, track, orchestra, debate and choir at Boise High School. He is also active in a variety of leadership groups including Key Club, Fellowship of Christian Athletes and Quiz Bowl/Science Club. POLICY TITLE: Evaluation of Certificated Personnel POLICY NO: 5340 PAGE 1 of 1

The District has a firm commitment to performance evaluation of District personnel, whatever their category and level, through the medium of a formalized system. The primary purpose of such evaluation is to assist personnel in professional development, and in achieving District goals, and to assist with decisions regarding personnel actions. This policy applies to certificated personnel, but the District shall differentiate between non-instructional and pupil instructional personnel. The Superintendent is hereby directed to create procedures that differentiate between certificated non-instructional and certificated pupil instructional personnel in a way that aligns with the Charlotte Danielson Framework for Teaching Second Edition to the extent possible.

Each certificated staff member shall receive at least one (1) written evaluation to be completed by no later than May 1st for each annual contract year of employment and shall use multiple measures that are research based and aligned to the Charlotte Danielson Framework for Teaching Second Edition. The evaluation of instructional certificated personnel shall annually include a minimum of two (2) documented observations, one (1) of which shall be completed prior to January 1st. At least one of the documented observations will be a minimum of 30 continuous minutes. A residency teacher will have two (2) observations and both will be at least 30 continuous minutes. The formal written evaluation will be reviewed during a post evaluation conference to communicate results, discuss any concerns, commend the employee for a job well done, and to obtain signatures. In situations where a teacher is unavailable for two (2) documented classroom observations, due to situations such as long-term illness, late year hire, etc., one (1) documented classroom observation is acceptable.

Objectives

The formal performance evaluation system is designed to: 1. Maintain or improve each employee's job satisfaction and morale by letting him or her know that the supervisor is interested in his or her job progress and personal development; 2. Serve as a systematic guide for supervisors in planning each employee's further training; 3. Assure considered opinion of an employee's performance and focus maximum attention on achievement of assigned duties; 4. Assist in determining and recording special talents, skills, and capabilities that might otherwise not be noticed or recognized; 5. Assist in planning personnel moves and placements that will best utilize each employee's capabilities; 6. Provide an opportunity for each employee to discuss job problems and interests with his or her supervisor; and 7. Assemble substantiating data for use as a guide, although not necessarily the sole governing factor, for such purposes as wage adjustments, promotions, disciplinary action, and termination.

5340-1 Responsibility

The Superintendent or his or her designee shall have the overall responsibility for the administration and monitoring of the Performance Evaluation Program and will ensure the fairness and efficiency of its execution, including:

1. Distributing proper evaluation forms in a timely manner;

2. Ensuring completed evaluations forms are received at the district office on or before May 1.

3. Reviewing evaluations forms for completeness and identifying discrepancies;

4. Identifying discrepancies

4. Ensuring proper safeguards and filing of completed evaluations forms;

5. Creating and implementing a plan for ongoing training for evaluators and certificated personnel on the District’s evaluation standards, forms, and processes. and a plan for collecting and using data gathered from forms. The plan will include identification of the actions, if any, available to the District as a result of the evaluation as well as the procedure(s) for implementing each action

6. Creating and implementing a plan for collecting and using data gathered from evaluations.

7. Creating a plan for ongoing review of the District’s Performance Evaluation Program that includes stakeholder input from teachers, Board members, administrators, parents and guardians, and other interested parties;

8. Creating a procedure for remediation for employees who receive evaluations indicating that remediation would be an appropriate course of action; and

9. Creating an individualized evaluation rating system plan for how evaluations will be used to identify proficiency and record growth over time with a minimum of three (3) rankings used to differentiate performance of teachers and pupil personnel certificate holders including: unsatisfactory being equal to a rating of 1; basic being equal to a rating of 2; and proficient being equal to a rating of 3.

The immediate supervisor is the employee's evaluator and is responsible for: 1. Continuously observing and evaluating an employee's job performance including a minimum of two (2) documented observations annually for certificated instructional personnel, one (1) of which shall be completed prior to January 1st of each year; 2. Holding periodic counseling sessions with each employee to discuss job performance; 3. Completing Performance Evaluations Forms as required; and 4. Completing training on the District’s Performance Evaluation Program.

5340-2 Written Evaluation Form

An written evaluation form will be completed for each certificated employee. A copy will be given to the employee. The original will be retained by the Immediate Supervisor placed in the employee’s personnel file. This form The evaluation should be reviewed annually and revised as necessary to indicate any significant changes in duties or responsibilities. The form evaluation is designed to increase planning and relate performance to assigned responsibilities through joint understanding between the evaluator and the employee as to the job description and major performance objectives.

The written evaluation form will identify the sources of data used in conducting the evaluation. Aggregate data shall be considered as part of the District and individual school needs assessment in determining professional development offerings.

Evaluation Measures

Periodic classroom observations will be included in the evaluation process with a minimum of two (2) documented observations annually for certificated instructional personnel, one (1) of which shall be completed prior to January 1st.

Sixty-seven percent (67%) of the evaluation of certificated instructional personnel will be comprised of based upon the Professional Practice portion based on of the Charlotte Danielson Framework for Teaching Second Edition. The evaluation form will include at least one (1) of the following as a measure to inform the Professional Practice portion: input received from parents or guardians, or input received from students, and/or portfolios. The District has chosen parent or guardian input forms as its measure to inform the Professional Practice portion. Parent or guardian and/or student input forms will be made available. The Board shall determine the manner and weight of parent or guardian input. student input, and/or portfolios on the evaluation form.

Thirty-three percent (33%) of the evaluation of certificated instructional personnel will be based on multiple objective measures of growth in student achievement. One measure of growth in student achievement shall be as measured by Idaho’s statewide assessment for federal accountability purposes. The evaluation will also include at least one additional objective measure of growth in student achievement, based on research, as determined by the Board. The Board has chosen growth on common unit assessments at the secondary level done by pre- and post-tests, and growth on the IRI/MAZE from the fall to winter measures for the elementary level as its additional measure(s) of growth in student achievement. This portion of the evaluation may be calculated using current and/or past year’s data and may use one (1) or multiple years of data.

The evaluation form will be aligned with minimum State standards and based upon the Charlotte Danielson Framework for Teaching, Second Edition, and will include, at a minimum, the following general criteria upon which the performance evaluation system Professional Practice portion will be based:

A. Demonstrating Knowledge of Content and Pedagogy; B. Demonstrating Knowledge of Students; C. Setting Instructional Outcomes; D. Demonstrating Knowledge of Resources; E. Designing Coherent Instruction; and F. Designing Student Assessments.

A. Creating an Environment of Respect and Rapport; B. Establishing a Culture for Learning; C. Managing Classroom Procedures; D. Managing Student Behavior; and E. Organizing Physical Space.

3. Instruction and Use of Assessment

A. Communicating with Students; B. Using Questioning and Discussion Techniques; C. Engaging Students in Learning; D. Using Assessment in Instruction; and E. Demonstrating Flexibility and Responsiveness.

4. Professional Responsibilities

A. Reflecting on Teaching; B. Maintaining Accurate Records; C. Communicating with Families; D. Participating in a Professional Community; E. Growing and Developing Professionally; and F. Showing Professionalism.

Meeting with the Employee

Counseling Sessions for Improvement: Counseling sessions between supervisors and employees may be scheduled periodically. During these sessions, an open dialogue should occur which allows the exchange of performance oriented information. The employee should be informed of how he or she has performed to date. If the employee is not meeting performance expectations, the employee should be informed of the steps necessary to improve performance to the desired level. Counseling sessions should include, but not be limited to, the following: job responsibilities, performance of duties, and attendance. A memorandum for record will be prepared following each counseling session and maintained by the supervisor.

Meeting with the Employee Communication of Results: Each evaluation shall include a meeting with the affected employee to communicate evaluation results. At the scheduled meeting with the employee, the supervisor will:

1. Discuss the evaluation with the employee, emphasizing strong and weak points in job performance. Commend the employee for a job well done if applicable and discuss specific corrective action if warranted. Set mutual goals for the employee to reach before the next performance evaluation. Recommendations should specifically state methods to correct weaknesses and/or prepare the employee for future promotions.

2. Allow the employee to make any written comments he or she desires. Inform the employee that he or she may turn in a written rebuttal/appeal of any portion of the evaluation within seven (7) days and outline the process for rebuttal/appeal. Have the employee sign the evaluation form indicating that he or she has been given a copy and initial after supervisor’s comments.

No earlier than seven (7) days following the meeting, if the supervisor has not received any written rebuttal/appeal, the supervisor will forward the original evaluation form in a sealed envelope, marked “Personnel-Evaluation Form” to the Superintendent, or the designee, for review. The supervisor will also retain a copy of the completed form.

Rebuttals/Appeals Within seven (7) days from the date of the evaluation meeting with his or her supervisor, the employee may file a written rebuttal/appeal of any portion of the evaluation form. The written rebuttal/appeal shall state the specific content of the evaluation form with which the employee disagrees, a statement of the reason(s) for disagreement, and the amendment to the evaluation form requested.

If a written rebuttal/appeal is received by the supervisor within seven (7) days, the supervisor may conduct additional meetings or investigative activities necessary to address the rebuttal/appeal. Subsequent to these activities, and within a period of ten (10) working days, the supervisor may provide the employee with a written response either amending the evaluation form as requested by the employee or stating the reason(s) why the supervisor will not be amending the evaluation form as requested.

If the supervisor chooses to amend the evaluation form as requested by the employee then the amended copy of the evaluation form will be provided to, and signed by, the employee. The original amended evaluation form will then be forwarded to the Superintendent, or the designee, for review in a sealed envelope, marked “Personnel-Evaluation.” Form. The supervisor will also retain a copy of the completed form. The evaluation will be placed in the employee’s personnel file.

If the supervisor chooses not to amend the evaluation form as requested by the employee then the evaluation form along with the written rebuttal/appeal, and the supervisor’s response, if any, will be forwarded to the Superintendent, or the designee, for review in a sealed envelope, marked

5340-5 “Personnel-Evaluation” Form. The supervisor will also retain a copy of the completed evaluation form including any rebuttal/appeal and responses. The evaluation will be placed in the employee’s personnel file including any rebuttal/appeal and responses.

Action

Each evaluation will include identification of the actions, if any, available to the District as a result of the evaluation as well as the procedure(s) for implementing each action. Available actions include, but are not limited to, recommendations for renewal of employment, non- renewal of employment, probation, and others as determined. Should any action be taken as a result of an evaluation to not renew an individual’s contract the District will comply with the requirements and procedures established by State law.

Records

Permanent records of each certificated employee’s evaluation and any properly submitted rebuttal/appeal documentation will be maintained in the employee’s personnel file. All evaluation records, including rebuttal/appeal documentation, will be kept confidential within the parameters identified in State and federal law regarding the right to privacy.

Reporting

By July 1, 2014, the District shall submit an evaluation plan to the State Department of Education for approval. Any subsequent changes to the District’s evaluation plan shall be resubmitted to the State Department of Education for approval. The District shall report the rankings of individual certificated personnel evaluations annually to the State Department of Education.
