Job Purpose/Summary

 To manage person-centred rehabilitative community services for patients attending outpatients and day services at the Edinburgh Hospice.  To be responsible for the development and support the delivery of therapeutic programmes of care across Edinburgh and West Lothian, working in partnership with the clinical nurse managers.  To be accountable for the overall management of, recruitment, deployment and supervision of outpatients and day services staff and volunteers ensuring the co- ordination of all services provided to the patients.  Optimise the cost effective use of all resources including monitoring the relevant budget.  To promote an environment conducive to the training needs of staff and volunteers and participate in teaching.  To be accountable for defining and ensuring that agreed professional standards are reflected in practice across all AHP services covered by Edinburgh hospice, managing the AHP team.  To provide professional leadership within the Senior Management Team of the hospice.

Accountabilities (Duties & Responsibilities)

Clinical

 Work with the multidisciplinary team to provide a high standard of specialist palliative care to patients and their carers within community services, including the facilitation of a safe and effective discharge from the service, where appropriate.  To support a specialist AHP resource across hospice services.  To work closely with the clinical nurse specialists to ensure the development and maintenance of a problem solving approach to the assessment, planning, implementation and evaluation of care for all patients.  To develop and support the delivery of therapeutic programmes of care across Edinburgh and West Lothian.  To act as a specialist resource for patients, carers, staff, volunteers and non-specialist groups of staff.  To be responsible for the development, implementation and maintenance of agreed clinical policies and procedures. To ensure that review and regular update is undertaken in line with the changing and developing service within Marie Curie and nationally.  Foster a sensitive and caring environment for patients, carers, staff and volunteers leading the outpatients and day services teams by example and professionalism.  To maintain/develop clinical skills through regular direct clinical input.  To ensure that services support access for all and address health inequalities.

Managerial

 To manage the outpatients and day services ensuring the effective deployment of staff, allied health professionals, complementary therapists and volunteers to meet care needs. This will include overseeing duty rotas, anticipating problems and, where they exist, taking appropriate action.  Be responsible for assessment and management of risk, including infection control within outpatients and day services.

2 July 2015  Develop and project manage new service interventions/ activities to meet the needs of patients and carers  Participate in the SMT ‘on-call’ rota for the Hospice.  Have line management responsibility for designated staff including AHP team and Care Assistant including ensuring professional registration, insurance, continual development and supervision requirements are met.  Undertake management of sickness, absence and performance issues, reporting to the Lead Nurse, Community Services.  Participate in the Hospice’s Personal Performance Review and Development programme undertaking PPRDs with directly managed staff members and assisting them to formulate their Personal Development Plans, as well as ensuring appropriate system for reviews of all directly managed staff and volunteers.  Be responsible for the recruitment, selection and induction of staff and volunteers within community services  Ensure all activities are consistent with Marie Curie and the Hospice’s policies.  Play an active role as a member of the Senior Management Team including contributing to relevant hospice strategy and plans.

Communication

 Maintain effective support and communication with patients, families and carers.  Ensure effective communication is maintained: o With external health and social care partners o Within the community services multidisciplinary team and across the wider hospice clinical team o With wider hospice support and administration services  Ensure appropriate and timely use of outpatients and day services  Participate in or lead the weekly MDT meetings, ensuring effective patient management within community services, including timely discharge.  In co-operation with the Lead Nurse, Community Services and other senior colleagues, support systems for user feedback.  Ensure effective record keeping and documentation.  Promote an open and honest culture which reflects a ‘can do’ attitude.

Clinical Governance

 Play a key role in Marie Curie Clinical Governance Programme, participating in, and supporting research and audit both within the Hospice, regionally and nationally.  Actively encourage and participate in the Clinical Supervision process.  Develop research based practice and lead practice development activities.  Develop clinical protocols to facilitate practice developments, benchmarking where possible, to ensure that the Hospice adheres to best know practice and makes optimum use of resources to achieve the highest standards of care and service.  Maintain a safe environment for patients, visitors, staff and volunteers, ensuring that staff and volunteers are aware of their responsibility in relation to health and safety, and cleanliness.  Investigate all accidents, untoward incidents and complaints relating to the outpatients and day services, reporting to the Lead Nurse, Community Services and ensuring that appropriate action is taken in accordance with agreed policies and procedures.  Take an active role in quality assurance initiatives and implementing resulting action plans.

3 July 2015 Professional Development and Education

 Working closely with the senior clinical team, promote an environment conducive to the development needs of staff and volunteers, and participate in the teaching of staff including clinical staff, students and volunteers.  Ensure staff development is consistent with Marie Curie Nursing Strategy and national guidance.  Take responsibility for the educational needs of directly managed team and, where appropriate, work with the Lead Nurse, Community Services and Practice Educator to develop programmes to meet their needs.  Co-ordinate areas of expertise within the team in conjunction with the wider clinical teams.  Oversee and participate in induction programmes for new staff and volunteers.  Ensure all staff and volunteers participate in mandatory update training.  Adjust scope of practice where necessary to improve patient care.  Adhere to relevant Code of Professional Conduct in the performance of clinical practice and to be professionally accountable for all actions while on duty.  Keep up to date with current issues and changes in clinical practice, and ensure ongoing personal development in line with professional requirements.  Participate in Personal Performance Development Review with the Lead Nurse, Community Services to discuss own professional and managerial development and agree objectives in line with hospice priorities.
