PsychoTropical Commentaries (2016) 1:1 – 90 © Fernwell Publications Review Paper Monoamine Oxidase Inhibitors: a Review Concerning Dietary Tyramine and Drug Interactions PK Gillman PsychoTropical Research, Bucasia, Queensland, Australia Abstract This comprehensive monograph surveys original data on the subject of both dietary tyramine and drug interactions relevant to Monoamine Oxidase Inhibitors (MAOIs), about which there is much outdated, incorrect and incomplete information in the medical literature and elsewhere. Fewer foods than previously supposed have problematically high tyramine levels because international food hygiene regulations have improved both production and handling. Cheese is the only food that has, in the past, been associated with documented fatalities from hypertension, and now almost all ‘supermarket’ cheeses are perfectly safe in healthy-sized portions. The variability of sensitivity to tyramine between individuals, and the sometimes unpredictable amount of tyramine content in foods, means a little knowledge and care are still advised. The interactions between MAOIs and other drugs are now well understood, are quite straightforward, and are briefly summarized here (by a recognised expert). MAOIs have no apparently clinically relevant pharmaco-kinetic interactions, and the only significant pharmaco-dynamic interaction, other than the ‘cheese reaction’ (caused by indirect sympatho-mimetic activity [ISA], is serotonin toxicity ST (aka serotonin syndrome) which is now well defined and straightforward to avoid by not co-administering any drug with serotonin re-uptake inhibitor (SRI) potency. There are no therapeutically used drugs, other than SRIs, that are capable of inducing serious ST with MAOIs. Anaesthesia is not contra- indicated if a patient is taking MAOIs. Most of the previously held concerns about MAOIs turn out to be mythical: they are either incorrect, or over-rated in importance, or stem from apprehensions born out of insufficient knowledge. -

World Day for Cultural Diversity Prahok ktiss (pork belly with fermented fish) With many thanks to the team at International Development Enterprises, Cambodia, for sharing this recipe and background. Prahok ktiss (image credit: International Development Enterprises). Prahok Ktiss is a very old, traditional recipe using prahok, a fermented salted fish paste which has been eaten by Cambodians almost every day. In fact, our diet is made up of over two thirds of fish and Prahok is a fermentation method to preserve fish even though it has a strong rich smell of rotten fish, but this smell is very well associated with Cambodia. Another main ingredient in this dish is Kroeung, a yellow curry-like paste which is made out of the combination of lemongrass, garlic, turmeric, shallot, galangal, makrut lime leaves, and red chilli. Kroeung has been used in many Cambodian dishes, we even use the ingredients in it for hot compress massage and believe it has benefits for our health as well. Recipe: Ingredients: • 150 g pork belly • 50 g prahok (fermented salted fish) • 2 garlic cloves, crushed www.mecs.org.uk For the sauce: • ½ red pepper, pounded to a paste • 2 garlic cloves, crushed • 20 g palm sugar • 125 ml (½ cup) coconut milk • 10 miniature eggplant or one small chopped eggplant (optional) • 2 makrut lime leaves, shredded • Splash vegetable oil For the Kroeung paste: • 1 stalk lemongrass, thinly sliced • 2 large garlic cloves, coarsely chopped • 1 small shallot, coarsely chopped • ½ tsp galangal, peeled, coarsely chopped • 3 makrut lime leaves, de-veined • ½ tsp turmeric, peeled, chopped • 1 chilli, chopped, seeded (optional) Method: • Using two knives or cleavers, finely chop the pork belly, prahok and two garlic cloves. -

La medicina moderna ha permitido alargar la vida humana en todo el planeta, especialmente en los países en desarrollo. Este hecho, aunque posi- tivo, coincide con una limitación en la producción de alimentos provocada por las sequías, heladas, inundaciones y cambios climáticos a largo plazo, con graves consecuencias actuales para una gran parte de la humanidad, e incluso con la posibilidad de una hambruna en masa en el futuro. Los gra- ves aspectos morales y sociales del hambre en el mundo requieren hoy de los recursos de todas las naciones para que este flagelo sea efectivamente reducido. Es un deber evitar el desperdicio de ali- mentos, para poder ayudar a disminuir la desnutrición e incluso la muerte por hambre.
