To maintain the highest degree of integrity and continue the tradition of excellence and education, the American Royal Livestock Show Management reserves the right to disqualify any animal that:  Fails to meet the provisions of the 2021 Premium Book including but not limited to the Code of Ethics, Rules of Liability, Quality Assurance and Drug Testing (Premium Book, pg. 27-28)  Fails to meet the USDA Wholesome Meat Act (Premium Book, pg. 24 & 35)  Fails to meet the Wholesome Assurance statement (Premium Book, pg. 36)

All show animals are subject to inspection and evaluation for signs of unethical fitting, including but not limited to: injection sites, abrasions, artificial filing and surgical alterations.

In the chart below, please identify ALL medications, vaccinations, injections, medical treatments, and any foreign substances administered, in any form or manner, from the ownership deadline (Sunday, August 15th, 2021) through the Junior Premium Livestock Auction. Examples of medications, injections, and vaccinations that must be disclosed include: Penicillin, Flunixin, vitamins, etc. (These are only examples, and any medications, injections, substances or vaccinations administered to your animal must be recorded.)

If the information provided does not match carcass findings or drug test results, you may be asked to appear before the American Royal to explain any discrepancies. The American Royal reserves the right to impose sanctions for incomplete or inaccurate disclosures including, but not limited to, forfeiture of titles, winnings and premiums and restrictions on future participation in future American Royal events.

I understand this is a mandatory form that will be used by Livestock Show Management, and that all medications, vaccinations, injections, medical treatments, and all other foreign substances administered to my animal, by myself or others, must be disclosed.
