North Homeless Pathway referral form This form is to be used to refer single clients from the , Chesterfield & areas who appear to be Homeless or threatened with Homelessness in 56 days. If the client is homeless or due to be homeless on the same day Tel 01246 498204 immediately. Referring organisation statement & permission - You have told us that you are either homeless or threatened with homelessness within 56 days. In order to help you either remain in your current accommodation or find new accommodation we have to inform the relevant local authority homelessness & housing options service in accordance with the Homelessness Reduction Act 2017. We need your consent to make a referral to the relevant local authority. If you choose to give your consent then we will complete the information on this form and send this to the local authority for them to start making enquiries on your behalf prior to them contacting you to discuss your situation with you. The information will also allow them to form a basic assessment of how they will be able to help you. Client consent - I give permission for the referring organisation to make a referral on my behalf to the relevant local authority to allow them to help me with my housing situation.
