Pharmacies that engage in sterile compounding are required to perform viable surface sampling and viable air sampling by volumetric methods under dynamic conditions that simulate actual production at least every six months, pursuant to Title 16, California Code of Regulations, section 1751.4(j).

Pursuant to Business and Professions Code (BPC) section 4126.8, compounding of drug preparations for furnishing, distribution or use in California must meet standards established by the United States Pharmacopeia-National Formulary. According to United States Pharmacopeia chapter 797 (USP <797>), regardless of the number of colony forming units (CFU) identifed, further corrective action may be warranted by the identifcation (at least the genus level) of the microorganism(s) recovered. Objectionable organisms considered to be highly pathogenic – i.e., gram-negative rods, coagulase positive staphylococcus, molds and yeasts – shall be immediately remedied regardless of CFU count with the assistance of a competent microbiologist, infection control professional, or industrial hygienist.

During the annual sterile compounding license renewal, a Board inspector noticed all compounding was occurring in a laminar fow hood within the non-classifed segregated compounding area (SCA). This compounding included the preparation of a total parenteral nutrition (TPN) solution. TPN solution is a medium-risk sterile compounded preparation. The inspector queried the pharmacist-in-charge (PIC), who stated the CAI had been having problems a few months back and the pharmacy decided to replace it with a laminar fow hood.
