JOB TITLE: Consultant Psychiatrist - Home Treatment Crisis Response Service

BASE/LOCATION: All posts are appointed to the Southern Health and Social Care Trust. The base for this post will be Armagh Community Hospital however the post holder may be required to work on any site within the Southern Health and Social Care Trust.

SUMMARY OF POST:  This replacement post will participate in a 1:20 on-call rota. Current pay supplement is 3%.  This is a permanent post.  This post will attract a salary of £82,500 - £111,230 per annum.  This is a full-time position, however anyone interested in working part-time / job share is also welcome to apply.  This post will work alongside a team of dedicated multi-disciplinary staff and a close knit team of Consultant Psychiatrists.  Annual leave will be 32 days per annum initially rising to 34 days after 7 years’ seniority, plus 10 statutory and public holidays.  The post also has an attractive study leave entitlement of up to 30 days paid leave with expenses in any period of three years.  A relocation package may also be available if appropriate.  The Southern Trust has established a dedicated revalidation support team which ensures all doctors have an annual appraisal with a trained appraiser and supports all doctors through the revalidation process. The Trust has also appointed corporate, Consultant and SAS Leads for appraisal and revalidation.  The Trust supports the requirements for continuing professional development (CPD) as laid down by the GMC and is committed to providing time and financial support for these activities.  The post will attract all the terms and conditions and employment benefits associated with an NHS post e.g. NHS indemnity; access to NHS pension scheme and many additional benefits such as child care vouchers etc.

THE MENTAL HEALTH AND DISABILITY DIRECTORATE The Directorate is managed by a Director and two Assistant Directors. The management team is supported by an Associate Medical Director and two Clinical Directors.

The Mental Health Division of the Directorate underwent major service reconfiguration over 10 years ago merging four legacy trusts. All acute in-patient services have been amalgamated onto the Bluestone site including PICU, functional old age admissions, general adult admissions and the Intellectual disability assessment unit. Teams are aligned with General Practitioner practices providing a sector-based service with Support and Recovery consultants having responsibility for all inpatients and outpatients in that sector, with the support of the inpatient and Community Mental Health Teams. Some of the other consultant post are more Primary Care facing. The sector-based approach also applies in Psychiatry of Old Age, but 1 of the four post sits outside that and provides all the consultant input to patients admitted to the functionally mentally ill elderly ward at the Psychiatric Unit, Bluestone.

The Consultant posts in the Directorate at present are as follows:

 Psychiatry of Old Age – 4.0 WTE  Forensic Psychiatry – 1.0 WTE (includes 0.5 WTE in Shannon Clinic, Belfast Trust)  Learning Disability – 5.0 WTE

MENTAL HEALTH FACILITIES Bluestone Unit – a 74 bedded unit for acute inpatient care on the Craigavon Hospital site. The new build on the site also houses PICU (Rosebrook) and the Learning Disability Treatment and Assessment Unit (Dorsy).

St Luke’s Hospital – still retains beds for the dementia services for the Trust and provides the base for the Forensic Team and the Addictions Service. The Primary Mental Health Care Team and the Home Treatment Team are based at Armagh Community Hospital, Trasna House and Daisy Hill Hospital.

Mental Health Resource Centres operate in Craigavon, Dungannon and Newry.

Community Team bases in Armagh, Dungannon, Lurgan and Newry.

ADULT MENTAL HEALTH SERVICES There are currently three arms to the service; Primary Mental Health Care, Acute Mental Health Care and Support and Recovery, they work across three geographical areas, Craigavon and Banbridge, Newry and Mourne and Armagh and Dungannon. Allied to this are teams for Eating Disorders, Addictions, Forensic Psychiatry and Liaison Psychiatry.

PRIMARY MENTAL HEALTH CARE This service is based on the principles of the stepped care model, providing specialist mental health input to tier two/ three.

THE ADDICTION SERVICE The Addiction Service is a Trust wide service which is based in St Luke’s Hospital site and provides community based clinics in 9 different locations across the Trust. It also provides enhanced community support and liaises with the inpatient facilities based in Downpatrick and Enniskillen, for this catchment area, to facilitate stepdown and step up of patients.

INPATIENT SERVICE The Bluestone Unit is a purpose-built unit on the Craigavon Area Hospital site. It is located adjacent to the PICU. There are 3 Acute General Adult wards each with 18 beds. We have 2/3 consultants feeding into each ward, with cross cover provided each day so that clinical decision making occurs daily. Willows ward has 20 beds and provides acute admission beds for functional psychiatry of old age patients. On site there is also a 10 bedded PICU, Rosebrook, and the Intellectual Disability acute admissions ward, Dorsy.

SUPPORT AND RECOVERY TEAM S&R consultants provide clinical leadership to the Support and Recovery Teams in the Trust. Patients are allocated by GP practice and relate to the CMHT in that area. The CMHT is multi-professional and led by a Team Leader. The teams have a tiered approach to patient care with the most complex patients seeing the most senior staff and less complex patients being managed through an agreed shared-care arrangement with the patients GP.

LIAISON TEAM The Mental Health Liaison team provides a daily assessment service to the Emergency departments in Craigavon Area and Daisy Hill hospitals and responds to referrals from the medical, surgical and maternity wards. The team also cover Lurgan and South Tyrone Sub-acute Hospitals. The team integrated three existing teams, general adult liaison, memory liaison and addiction liaison services in February 2019. The team assesses and/ or advise on all over 18 year olds presenting with mental health symptoms regardless of cause, working alongside CAMHS services and Intellectual disability services who also input into the acute hospitals for their client base.

HOME TREATMENT CRISIS RESPONSE SERVICE The service provides acute psychiatric treatment to all patients from 17 and 9 months residing in the Southern Trust catchment. The aim is to provide an alternative to hospital admission for those unwell enough to be at the point of admission to an acute mental health unit. The service also covers crisis presentations with advice from the consultant as needed. The teams work across 24 hours per day and from 9pm night staff also cover referrals from the emergency departments of Craigavon area hospital and Daisy Hill hospital. Telephone advice to this extended service is through the on call consultant/ 2nd tier rota..

There are 3 teams based on the old legacy Trust catchment areas, Armagh and Dungannon, Newry and Mourne and Craigavon and Banbridge. The 3 three teams are currently covered by input from 2 consultant posts, with 1 consultant covering the Armagh and Dungannon area and part of the Craigavon and Banbridge area and the other consultant post covering the rest of Craigavon and Bainbridge and the Newry and Mourne areas. It is anticipated that 3 WTE consultants will cover one of these catchments each. There are 3 team leaders and a service co-ordinator. There is usually a higher and core psychiatry trainee doctor who allocated to the service and they span the three area..

PRINCIPLE DUTIES/ RESPONSIBILITIES The post holder will be expected to:

 Develop a close working relationship with the general practitioners in the area.  Work with consultants in the mental health directorate and their teams to ensure appropriate management of patients in the least restrictive environment and best suited to their needs.  To work with inpatient services to ensure patient flow through acute services and discuss early discharge of patients to HTCR team with consultants at an early stage after admission.  Work closely with Home Treatment Crisis Response teams to ensure optimal care for patients, being mindful of appropriate risk management.  Assess and treat patients as a member of the multidisciplinary team in keeping with the principles of ‘New Ways of Working’.  Provide professional leadership including clinical supervision to the medical team and to provide clinical leadership to the HTCRT MDT  Develop a close working relationship with colleagues in the acute hospital and participate in regular interface meetings.  Ensure effective liaison occurs between mental health services both community and hospital based, acute hospital colleagues, and also with other agencies in the voluntary and statutory sectors as appropriate  To assist with the further development of training and education for trust staff across a range of directorates.

Professional Responsibilities  The post holder will be expected to be approved as a Responsible Medical Officer under Part II of the mental health order 1986 or be willing to undertake training to obtain this.  To maintain professional standards and obligations as set out by the General Medical Council and comply in particular with the GMC’s guidance on Good Medical Practice as amended or substituted from time to time.  Participate in clinical governance, clinical audit/research, medical advisory committees and other committees as may be necessary.  Engagement in medical audit and utilise service statistics in order to develop and further evolve his/her sessions.  Carry out any work related to and reasonably incidental to the duties set out in the job plan or rota including keeping of records and the provision of reports, the proper delegation of tasks and maintaining skills and knowledge.

Other duties will include  Participation in the academic/teaching programme within the Trust for undergraduate and postgraduate medical education and act as clinical supervisor to those junior medical staff allocated to him/her.  Participation in Continuing Professional Development as recommended by the Royal College of Psychiatrists.  Participation in the consultant on-call rota, which will be approximately 1 in 20 covering services throughout the trust, including St Luke’s Hospital, Armagh and Bluestone Unit, CAH attracting a Band A 3% supplement.  Provide cross cover for consultant colleagues on leave.

Management Responsibilities  Provide advice and direction, together with consultant and other colleagues, to the Director of Mental Health and Disability and to other senior managers within the Trust, on the range of services necessary to increase the effectiveness of the support given to primary care.  Relevant administrative duties associated with the care of his/her patients and the running of the service.  To undertake, as required, various management roles in relation to activities of the Trust.  To participate in the education, training and support of the multidisciplinary team.

Teaching and Research Responsibilities  The post holder will be expected to be fully involved in the teaching of undergraduate medical students and the training of junior medical staff within Mental Health and across other relevant specialties.  The post holder will be required to take part in relevant audit activities.  It is hoped that the post holder will carry out research according to their own areas of interest.

SUPPORTING RESOURCES This post is supported by access to a full-time Personal Secretary who is shared with other consultant colleagues.

The successful candidate will have access to IT facilities and there is a newly refurbished Medical Education Centre on the Craigavon Area Hospital site.

SUPERVISION /MANAGEMENT REQUIREMENTS The post holder will be expected to provide clinical supervision for Psychiatric Trainees as well as for the members of the multidisciplinary team.

CONTINUING PROFESSIONAL DEVELOPMENT The Trust supports the requirements for CPD, as laid down by the Royal College and is committed to providing time and financial support for these activities.

REVALIDATION The Trust has the required arrangements in place, as laid down by the Royal College to ensure that all doctors have an annual appraisal with a trained appraiser and supports doctors going through the revalidation process.
