BOARD OF CONTROL MEETING MINUTES MONDAY, NOVEMBER 23, 2015

Director Berner called the Board of Control meeting to order at 11:03 am.

The meeting was called to award a contract for CCTV inspection and storm sewer flooding services for West 227 Street, West 229 Street, and Clifford Drive. This field testing will assist in clarifying the necessary scope of work for sewer improvements on those streets.

A Request for Proposals was composed and distributed by Black & Veatch on behalf of the City of Fairview Park. One proposal was received. Other companies refrained from submitting proposals due to their workloads.

Dianne Sumego of Black & Veatch made the written recommendation to award the contract to C&K Industrial Services of Independence, Ohio, in the amount of $29,996.90 (see attachment). C&K Industrial Services has provided sewer flooding services to cities throughout Northeast Ohio.

Interim Director Cingle made a motion to accept the recommendation of Black & Veatch and award the contract for CCTV inspection and storm sewer flooding services to C&K Industrial Services in the amount of $29,996.90. The motion was seconded by Mayor Patton and unanimously approved by the Board of Control.

Director Fagnilli motioned to adjourn the meeting. Interim Director Cingle seconded the motion and it was approved. The meeting was adjourned at 11:10 am.

City of Fairview Park CCTV Inspections and Storm Sewer Flooding

To Whom It May Concern:

Thank you for the opportunity to submit a bid on the CCTV Inspections and Storm Sewer Flooding for the City of Fairview Park. Please see the attached requested documentation.

C&K Industrial Services, Inc. has provided sewer flooding services in the past for the cities of Warren, Livingston, and Youngstown between the years of 2006 and 2009. If C&K is successful on this bid, we would like to begin this project as soon as possible. We anticipate the project taking a couple of weeks (approximately 10 shifts). As per the instructions, the City of Fairview Park will provide maintenance of traffic, furnish appropriate hydrants for our use, and will pay any associated costs.

Mr. Harrison was educated at Cardinal Allen College in , and after college pursued various jobs in the engineering field. He began his career within the Infrastructure and Pipeline industry as a Field Technician for Seers inspections in , England back in 1984. In England, he was introduced to hands-on inspections of sewers, culverts, chimneys and various types of pipelines in locations in the throughout the United Kingdom.

HISTORY After a couple of years there, he was given his own inspection team and spearheaded major walk-through and specialty investigations from difficult and one-access locations primarily in the area. In 1988, he joined Neil Bunting and Associates in Sheffield, as site foreman and supervised multiple evaluation, rehabilitation and investigative projects in the U.K., Portugal, and the Channel Islands. The company eventually became known as AMTEC. In 1992, Paul travelled to Dubai to train local people in inspection and cleaning protocol and the benefits of regular maintenance using CCTV. He also held classes teaching sewer classification, coding, and equipment setup. In 1997, he headed a team to startup office in Atlanta, GA specializing in Sonar and TISCET inspection performing contracts in USA and Mexico. He was offered position in Kenny Manta and worked on various projects in Chicago and Cleveland until eventually joining C&K Industrial Services in 2002.
