    Inflammation in type 2 diabetes and the neuroendocrine role of IL-18 Chris Booker A thesis submitted for the degree of PhD at the University of Otago, Dunedin, New Zealand Date: May 2012 Abstract: Cardiovascular disease is one of the leading causes of morbidity and mortality in type 2 diabetes, and yet, recent randomised controlled trials investigating the effect of different regimens of blood glucose control failed to show any benefit of more intensive control on rates of cardiovascular mortality, and minimal benefit on levels of morbidity. This thesis examines the theoretical understanding of type 2 diabetes, including the historical process which led to the above trials being conducted. A review of the historical and modern evidence surrounding the role of chronic inflammation suggests that this may be a common pathogenic alteration in obesity, diabetes and cardiovascular disease which may contribute to the development of cardiovascular disease in persons with type 2 diabetes, and even the pathogenesis of type 2 diabetes itself. This thesis therefore investigates the role of inflammation in type 2 diabetes, by a two-pronged approach: one investigates the effects of a dietary intervention on levels of chronic inflammation in participants with type 2 diabetes in the Lifestyle Over and Above Drugs in Diabetes (LOADD) randomised controlled trial. In this trial, chronic inflammation is assessed by a range of inflammatory markers and adipokines including leptin, neopterin, tumor necrosis factor α and its soluble receptors, the interleukins 1, 6, 10, and 18, and interleukin 1 receptor antagonist. The dietary intervention group exhibited a significant reduction in circulating levels of interleukin-18 (IL-18) and neopterin relative to a 'care as usual' control group; reductions which further analyses suggest were unrelated to concomitant changes in adiposity or glycaemia.

Abstract The first low carbohydrate diets for diabetes, which were introduced in the 18th century, were not high fat diets so much as high protein diets. By the late nineteenth century a consensus had begun to appear in European studies that a higher fat and lower protein intake was more tolerable. Frederick Allen confirmed this in animal studies, but the diet he introduced into clinical practice in 1914 restricted fat, protein, and total calories, becoming in extreme cases a starvation diet with the deadly side effect of “inanition”. Interpreting Allen’s research in light of the studies in chemical metabolism of Rollin Woodyatt, Louis “Harry” Newburgh, with Phil Marsh, dared to increase fat in the diets of diabetic patients at the University of Michigan Hospital in 1918, with gratifying results which were published in 1920.

Sie stellt einen Beitrag zur Biochemiegeschichte des 19. Jahrhunderts dar, ein Arbeitsgebiet, das mich als Biochemikerin besonders interessierte. Die Zielsetzung der Arbeit bestand darin, den wissenschaftlichen Werdegang des physiologischen Chemikers Felix Hop- pe-Seyler darzustellen und in diesem Zusammenhang die inner- und außerwissen- schaftlichen Faktoren der Disziplinengenese der physiologischen Chemie im 19. Jahr- hundert zu untersuchen. Der vorgegebene inhaltliche Rahmen der Studie machte nach einer kritischen Sichtung des zugänglichen Quellenmaterials zunächst eine sinnvolle Beschränkung desselben notwendig. In den Mittelpunkt der vorliegenden Arbeit wurden daher die wissen- schaftshistorisch noch unerschlossenen Briefe Hoppe-Seylers an seinen Lehrer Rudolf Virchow, an Emil Du Bois-Reymond, Wilhelm Ostwald, Heinrich Caro, Emil Erlen- meyer und den Verleger Karl Trübner, Schreiben Eduard Pflügers an Emil Du Bois- Reymond und Willy Kühnes an Rudolf Virchow sowie weitere handschriftliche Quel- len zum institutionengeschichtlichen Hintergrund aus dem Archiv der Berlin- Brandenburgischen Akademie der Wissenschaften, den Straßburger Archives départe- mentales du Bas-Rhin, dem Geheimen Staatsarchiv Preußischer Kulturbesitz Berlin, der Staatsbibliothek zu Berlin, dem Deutschen Museum München und dem Universi- tätsarchiv der Humboldt-Universität zu Berlin gestellt.
