Resolution of this issue is epidemic affecting not only North America, important to prevention and treatment. Can obesity be but also many other developed and avoided by the rigorous daily activity and limited availability of food found in many hunter-gatherer developing nations (GBD 2013 Obesity groups, or is the accumulation of body fat an inevitable Collaboration, 2014; World Health consequence of the human genome? Perhaps as a narrative review has gathered available information on eating habits, habitual daily physical activity and body fat consequence of this attention, we tend to accumulation over various historical eras, ranging from regard an excessive accumulation of body the earliest Paleolithic and Neolithic communities to fat as a modern problem, brought about by Victorian society. • Inflammation in Type 2 Diabetes and the Neuroendocrine Role of IL-18
Inflammation in type 2 diabetes and the neuroendocrine role of IL-18 Chris Booker A thesis submitted for the degree of PhD at the University of Otago, Dunedin, New Zealand Date: May 2012 Abstract: Cardiovascular disease is one of the leading causes of morbidity and mortality in type 2 diabetes, and yet, recent randomised controlled trials investigating the effect of different regimens of blood glucose control failed to show any benefit of more intensive control on rates of cardiovascular mortality, and minimal benefit on levels of morbidity. This thesis examines the theoretical understanding of type 2 diabetes, including the historical process which led to the above trials being conducted. A review of the historical and modern evidence surrounding the role of chronic inflammation suggests that this may be a common pathogenic alteration in obesity, diabetes and cardiovascular disease which may contribute to the development of cardiovascular disease in persons with type 2 diabetes, and even the pathogenesis of type 2 diabetes itself. This thesis therefore investigates the role of inflammation in type 2 diabetes, by a two-pronged approach: one investigates the effects of a dietary intervention on levels of chronic inflammation in participants with type 2 diabetes in the Lifestyle Over and Above Drugs in Diabetes (LOADD) randomised controlled trial. In this trial, chronic inflammation is assessed by a range of inflammatory markers and adipokines including leptin, neopterin, tumor necrosis factor α and its soluble receptors, the interleukins 1, 6, 10, and 18, and interleukin 1 receptor antagonist. The dietary intervention group exhibited a significant reduction in circulating levels of interleukin-18 (IL-18) and neopterin relative to a 'care as usual' control group; reductions which further analyses suggest were unrelated to concomitant changes in adiposity or glycaemia.

Abstract The first low carbohydrate diets for diabetes, which were introduced in the 18th century, were not high fat diets so much as high protein diets. By the late nineteenth century a consensus had begun to appear in European studies that a higher fat and lower protein intake was more tolerable. Frederick Allen confirmed this in animal studies, but the diet he introduced into clinical practice in 1914 restricted fat, protein, and total calories, becoming in extreme cases a starvation diet with the deadly side effect of “inanition”. Interpreting Allen’s research in light of the studies in chemical metabolism of Rollin Woodyatt, Louis “Harry” Newburgh, with Phil Marsh, dared to increase fat in the diets of diabetic patients at the University of Michigan Hospital in 1918, with gratifying results which were published in 1920.

Sede Amministrativa: Università degli Studi di Padova Dipartimento di Biologia _________________________________________ SCUOLA DI DOTTORATO DI RICERCA IN BIOSCIENZE E BIOTECNOLOGIE INDIRIZZO: NEUROBIOLOGIA CICLO: XXVIII TITOLO TESI EFFECTS OF A KETOGENIC MEDITERRANEAN DIET ON PHYSIOLOGICAL AND PSYCHOLOGICAL VARIABLES Direttore della Scuola: Ch.mo Prof. Paolo Bernardi Coordinatore d’indirizzo: Ch.ma Prof.ssa Ornella Rossetto Supervisore: Ch.mo Prof. Antonio Paoli Dottoranda: Alessandra Lodi ACKNOWLEDGEMNTS The writing of a doctoral dissertation is not an easy task, but it was the best thing that could happen to me to deepen a subject, that of ketosis and weight loss, that I love so much. Thanks to the confidence of my supervisor, I could prove myself in the field of scientific research, designing experiments and actively taking care of each phase of the research. I encountered the difficulties of doing research on humans, but the reward is an enormous satisfaction. I'm honoured to have worked with many people I did not know before and who - thanks to the participation in my research - could improve their eating habits. I am therefore deeply satisfied to have completed these four years, which have taught me what it means doing scientific research, analysing data and writing scientific articles. I am deeply grateful to all the people who have made this possible. In particular, special thanks go to my project supervisor Prof. Antonio Paoli, for his trust and intellectual help and to Gianluca Mech S.p.A. for the financial support. Sincere thanks go to Prof. Gerardo Bosco for his support and encouragement and to Prof. Antonino Bianco for his help.
