The Excess Readmission Ratio (also referred to as the Standardized Readmission Ratio [SRR]) is the measure that will be used to determine the payment adjustment for the Program. If a hospital performs better than an average hospital that admitted similar patients (that is, patients with similar risk factors for readmission such as age and comorbidities), the ratio will be less than 1.0000. If a hospital performs worse than average, the ratio will be greater than 1.0000. Excess Readmission Ratios greater than 1.0000 will be included in the payment adjustment formula. 22 [d] The number of eligible unplanned 30-day readmissions nationally divided by the number of eligible discharges nationally. 2. Prioritization and Roadmap: Heart Failure Example Based on the data, the team identified areas where heart failure care can be improved. We prioritized and mobilized four workgroups. Home Health & SNF Care 23 3. 90 Day Improvement Cycle: Heart Failure Example Once the three workgroups were identified and mobilized, they each went through a 90-day improvement cycle.     Rochester Regional Health Leave of Absence Returning to Work from a Continuous Leave of Absence How to return to work from a disability leave or a continuous leave under the Family and Medical Leave Act (“FMLA”) for a reason related to your own illness or injury: If you are returning from a disability leave or a continuous FMLA leave due to your own illness or injury, you must be cleared by Employee and Occupational Health (“EHS”) prior to returning to work. To submit appropriate medical documentation from your medical provider in order to be cleared to return to work, the documentation should be emailed to: [email protected]. • Please note that EHS will review the medical documentation provided and prepare a clearance note, which will then be sent to RRH Leave Administration and your direct supervisor. Upon receipt of the clearance note provided by EHS, Leave Administration will return you from leave in Workday, which will trigger a reinstatement of your access by IT. • Please note that if you have not been cleared by EHS, you will be unable to return to work in any capacity and your access may remain suspended until you are cleared.
