Vestibular neuritis, labyrinthitis, and a few comments regarding sudden sensorineural hearing loss Marcello Cherchi §1: What are these diseases, how are they related, and what is their cause? §1.1: What is vestibular neuritis? Vestibular neuritis, also called vestibular neuronitis, was originally described by Margaret Ruth Dix and Charles Skinner Hallpike in 1952 (Dix and Hallpike 1952). It is currently suspected to be an inflammatory-mediated insult (damage) to the balance-related nerve (vestibular nerve) between the ear and the brain that manifests with abrupt-onset, severe dizziness that lasts days to weeks, and occasionally recurs. Although vestibular neuritis is usually regarded as a process affecting the vestibular nerve itself, damage restricted to the vestibule (balance components of the inner ear) would manifest clinically in a similar way, and might be termed “vestibulitis,” although that term is seldom applied (Izraeli, Rachmel et al. 1989). Thus, distinguishing between “vestibular neuritis” (inflammation of the vestibular nerve) and “vestibulitis” (inflammation of the balance-related components of the inner ear) would be difficult. §1.2: What is labyrinthitis? Labyrinthitis is currently suspected to be due to an inflammatory-mediated insult (damage) to both the “hearing component” (the cochlea) and the “balance component” (the semicircular canals and otolith organs) of the inner ear (labyrinth) itself. Labyrinthitis is sometimes also termed “vertigo with sudden hearing loss” (Pogson, Taylor et al. 2016, Kim, Choi et al. 2018) – and we will discuss sudden hearing loss further in a moment. Labyrinthitis usually manifests with severe dizziness (similar to vestibular neuritis) accompanied by ear symptoms on one side (typically hearing loss and tinnitus). -

Hearing Loss Facts S The NIDCD estimates that approximately 15 percent (26 million) of Americans between the ages of 20 and 69 have high frequency hearing loss due to exposure to loud sounds or noise at work or in leisure activities. S Only 1 out of 5 people who could benefit from a hearing aid actually wears one. S Three out of 4 children experience ear infection (otitis media) by the time they are 3 years old. Hearing Loss Facts S There is a strong relationship between age and reported hearing loss: 18 percent of American adults 45-64 years old, 30 percent of adults 65-74 years old, and 47 percent of adults 75 years old or older have a hearing impairment. S Roughly 25 million Americans have experienced tinnitus. S Approximately 4,000 new cases of sudden deafness occur each year in the United States. Benign Paroxysmal Positional Vertigo (BPPV)
Patient Information Patient Information Benign Paroxysmal Positional Vertigo (BPPV) BPPV causes spinning dizziness (‘vertigo’) due to crystals in the inner ear breaking loose. The crystals are normally embedded in a jelly and when we move forwards and backwards, or up and down, in space they send our brain messages about where we are. If they become loose they can find their way into the wrong area and cause the sensation that we are moving when we are actually not – the result is a sudden feeling of spinning, usually set off by turning your head. What causes BPPV? Perhaps half of all cases of BPPV are "idiopathic" – they occur for no known reason. The most common identified cause of BPPV in people under fifty is a previous head injury. In older people the most common cause is ‘wear and tear’ of the vestibular (balance) system of the inner ear. Some people have a history of previous inner ear inflammation with a period of severe, prolonged dizziness. Diagnosing BPPV The diagnosis is made based on history, physical examination and sometimes with hearing or balance testing. Other diagnostic tests may be required: for example, a CT or MRI may be required for cases that don't fit the usual pattern. It is possible to have BPPV in both ears, which may make the diagnosis and treatment more challenging. How is BPPV Treated? BPPV is often described as ‘self-limiting’ because symptoms often subside or disappear without any medical treatment. Symptoms tend to wax and wane. Physical manoeuvres and exercises are very effective in stopping your symptoms, and are the main basis of treatment. -

Objectives: The aim of this study was to describe the prevalence and the clinical patterns of tinnitus episodes which occurred in association with BPPV and to suggest possible interpretative hypotheses. Methods: 171 normal hearing patients affected by BPPV (50 males and 122 females; age range: 25-77 years; mean age 60.3 years ± 14.9) underwent pure tone audiome- try, immittance test and a clinical vestibular evaluation before and after repositioning manoeuvers. Those suffering from tinnitus were also assessed using visual analogue scales and tinnitus handicap inventory. Results: 19.3% of the patients reported the appearance of tinnitus concurrently with the onset of the positional vertigo. It was mostly unilateral, localized on the same ear as the BPPV, slight in intensity and intermittent. Tinnitus disappeared or decreased in all patients except two, either spontaneously, before performing the therapeutic manoeuvers, or shortly after. Conclusions: A possible vestibular origin of tinnitus determined by the detachment of macular debris into the ductus reuniens and cochlear duct is discussed. Meniere’s disease is a progressive inner ear disorder that causes repeated spells of 1) vertigo (spinning), 2) fluctuating hearing loss, 3) ringing in the ears, and 4) fullness or pressure in the ear. These episodes, or attacks, start suddenly and last for minutes to hours. Most people do not have symptoms in between episodes. The exact cause of Meniere’s disease is not known. One thought is that the fluid in the inner ear builds up, causing pressure within the inner ear. Meniere’s disease usually starts at ages 20-50 years of age. The severity of symptoms may be different with each episode and between Produced by people. Some people have spells weekly, others may not have spells for months or years. During a Meniere’s spell, a person may have nausea and vomiting, and they may not be able to walk or perform their daily activities. Physical and Sports Therapy Dizziness and Vertigo 70% of people will experience some form of dizziness in their life. It is the second most common reason for doctor visits and the primary reason for nearly half of all falls. Vertigo is a medical condition that is the leading cause of dizziness. Vertigo has several causes but most commonly is due to a problem between the inner ears and eyes. It can be resolved by a physical therapist with experience in treating the condition. Some common issues that cause Vertigo: Benign Paroxysmal Positional Vertigo (BPPV): Caused by the dislodging of tiny crystals in the inner ear. This can be caused by trauma, such as a fall, or even from quick head movements. Physical therapy can typically cure this condition in just a few visits. Labyrinthitis: Inflammation of the inner ear. It can be caused by infection or trauma to the head. Dizziness can frequently be diminished by physical therapy through specific exercise interventions. Meniere’s Disease: A chronic inner-ear condition that creates periodic episodes of Vertigo. Physical therapy can be beneficial in assisting with balance issues encountered during episodes. What can physical therapy do for Dizziness/Vertigo? Physical Therapy works to identify the underlying cause of dizziness. Once the source is determined, a treatment plan is designed. For some conditions, such as BPPV, curative manual and exercise interventions are applied to eradicate the condition. For other conditions, such as Meniere’s Disease, efforts are directed at decreasing symptoms and improving uninvolved systems to enhance function. It was traditionally divided into four categories based on the patient’s history: vertigo, presyncope, disequilibrium, and light-headedness. However, the distinction between these symp- toms is of limited clinical usefulness. Patients have difficulty describing the quality of their symptoms but can more consistently identify the timing and triggers. Episodic vertigo triggered by head motion may be due to benign parox- ysmal positional vertigo. Vertigo with unilateral hearing loss suggests Meniere disease. Episodic vertigo not associated with any trigger may be a symptom of vestibular neuritis. Evaluation focuses on determining whether the etiology is peripheral or central. Peripheral etiologies are usually benign. Central etiologies often require urgent treatment. The HINTS (head-impulse, nystagmus, test of skew) examination can help distinguish peripheral from central etiologies. The physical examination includes orthostatic blood pressure measurement, a full cardiac and neurologic examina- tion, assessment for nystagmus, and the Dix-Hallpike maneuver. Benardete, [email protected] Abstract Anterior inferior cerebellar artery (AICA) aneurysms are rare, less than 1%-2% of all intracranial aneurysms. Aneurysms of the distal AICA are even less common and can present with hearing loss and facial paralysis because of their relationship with the internal auditory canal (IAC). A 65-year-old male was followed for fluctuating left facial weakness and left-sided hearing loss for over a year. Serial magnetic resonance imaging (MRI) scans showed a mass near the left IAC, thought to be a vestibular schwannoma. Just prior to his next clinic visit, the patient deteriorated suddenly from a subarachnoid hemorrhage. Cerebral angiography revealed a 5.5 mm saccular aneurysm at the distal left AICA, which was clip ligated via a translabyrinthine (TL) approach. The patient had a good functional outcome (modified Rankin Scale [mRS] 1) after 30 days despite persistent left facial weakness. Stable obliteration of the aneurysm was demonstrated by cerebral angiography postoperatively. Distal AICA aneurysms are rare and can have a similar presentation to tumors in the cerebellar pontine angle. Because of the unique anatomy of the distal AICA, open clip ligation via a TL approach is an effective method to secure these aneurysms. -
