Project SEARCH is a year-long programme of work based training. It works through a partnership between: A host employer (Nottingham University Hospitals) A school (Foxwood Academy) A supported employment service (Nottinghamshire County Council i- Work) Dear Parents and Carers. In order to make this process as smooth as possible, Ash Lea has put together information that we hope you will find useful. Helpful Hints for Year 12: Parents to begin to make enquires and make initial visits to preferred post 18 provision. Helpful Hints for Year 13 Autumn Term: Student ‘Leavers’ meeting will take place to ensure 1. Any initiated transition plan is on track or initiate transition plan if not already started. 2. Begin post 18 provision application process if not already done so. 3. Transition Social Worker should attend this meeting to offer further input and guidance. 4. Transitions Nurse should also attend to offer health related advice, support and ensure Leaver’s medicals are initiated. Spring /Summer Term. School, Home and Transitions Team (Social Worker, Nurse Etc) to work together to. 1. Ensure any agreed transition plan is on track. 2. Liaise with relevant professionals to ensure any pre-entry assessment process has taken place. 3. Arrange taster days and or visits .
