LOGBOOK INSTRUCTIONS: 1) DATE - enter month and date of fishing trip. (PLEASE ONLY REPORT ON TRIPS OCCURRING PRIMARILY IN NH WATERS) 2) # HOURS FISHED - enter the number of hours actually spent fishing to the nearest half-hour. 3) # ANGLERS IN PARTY - enter the number of people in your party that fished during the trip, including yourself (if you fished alone, then enter 1). 4) # FISH KEPT - enter the total number of striped bass caught and kept by people in your party during the fishing trip. 5) # FISH RELEASED - enter the total number of striped bass released by people in your party during the fishing trip (even legal sized fish). 6) # LEGAL SIZE FISH RELEASED - enter the number of legal sized striped bass (greater than or equal to 28”) that people in your party released during the fishing trip. 7) BOAT or SHORE - enter 1 if you fished from a boat or enter 2 if you fished from shore. 8) BAIT, LURE, or FLY - enter the appropriate code number for the type of terminal tackle you used. Enter as many as apply for that trip. 9) LENGTHS - enter the total length to the nearest inch (measure from tip of nose to end of tail) of all stripers you catch (both the fish you plan to keep and those you are planning to release). Please limit length ranges to 4 inches or less (i.e., "10 fish between 11-14 inches,").

When you have filled up the survey card or are finished striper fishing for the season, please fill in your name and address (so that we send you the results of the survey and a survey card for next year), fold the card in half on dotted line, tape it closed, and mail it.
