Introduction to the Post Post and specialty: Consultant Psychiatrist in General Adult Psychiatry Base: The Coast Resource Centre Number of programmed activities: 6 PAs per week Accountable professionally to: Medical Director Accountable operationally to: Medical Lead Context for the role The Trust is seeking a consultant psychiatrist to join the North Somerset Early intervention for psychosis team based at the Coast resource centre. This post is a new post. The previous medical cover was provided by the recovery services. Service Details The post holder will provide medical input in to the Early Intervention service. Summary of the Post The Early Intervention Team in North Somerset covers GP surgeries in the North Somerset area and operates five days per week from 9am to 5pm. Based at the Coast resource centre, the Early Intervention Team provides interventions to those with `at risk’ mental states or an established first episode of psychosis, referred internally from access services (Primary Care & Hospital Liaison services, Intensive Service) the Inpatient Service and externally directly from General Practitioners & self-referrals where appropriate. New Assessments are conducted in pairs and are assessed within two weeks of referral, the majority of referrals being seen within one week of referral. The Team works with patients aged from 14 to 35 years of age. Those under 18 are medically managed via the AWP Child & Adolescent Mental Health Service (CAMHS) and receive joint care coordination from a team member working across both services. The EI Team works with clients for up to three years, with the majority of clients having recovered to a sufficient degree to be discharged from secondary mental health services at that point. The team includes a broad skill base and delivers NICE-approved, evidence based interventions including CBT for psychosis, family work, occupational & vocational advice/interventions & support & pharmacological interventions. In addition, the team facilitates social & activity groups, carers’ courses. Staffing & Team Activity The Team includes Community Psychiatric Nursing, Social Work, Occupational Therapist and Vocational Worker skills as well as core skills across the Team in psychosocial and pharmacological interventions and physical health monitoring. The Early Intervention Team includes: 1.0 Team Manager 1.0 Senior practitioner 2.0 WTE 3.5 WTE Band 6 3.34 WTE Band 4 0.4 WTE Psychological therapist At the time of writing the Team carries a total caseload of approximately 44 with caseloads per staff member ranging from 5 – 10. In 2019, the Team received 64 referrals, and discharged 46. No patients are subject to community provisions of the Mental Health Act at the present time. Page 4 of 25 Avon and Wiltshire Mental Health Partnership Trust Consultant workload The Trust is committed to New Ways of working and in accordance with this the Consultant does not carry a personal caseload. The post holder will be aware of the caseload from regular supervision with the Team, with trainees, and from reviews with patients as appropriate and at the request of team members. The Consultant sees assesses new patients prioritised on the basis of complexity and risk and would expect to see 1-2 new patients per week and 2-3 reviews per week. The Post-holder will see patients at the Outpatient Department at the Coast Resource centre, The Longfox Unit at Weston general hospital, Windmill house and in the patient’s own homes & other locations as needed. The Consultant will expect to see 1-2 patients per Direct Clinical Care PA dedicated to either Clinics or home visits. Mental Health Act assessments are not taken as a routine part of the Consultant’s activity as this is outside of contractual work. However, by local agreement, patients who are under the care of the Team to be assessed under the Mental Health Act can be seen by the Consultant as a Section 12 approved Doctor who does not submit a claim to avoid any confusion over probity. The provision of Section 12 cover is the subject of Trust-wide review and so the description given represents the current situation which may be superseded as Policy is developed. There is no dedicated Place of Safety suite in BaNES, this provision being located at Southmead Hospital, Bristol (outside of the BaNES area). Care Clusters The service will use the care cluster model as a clinical framework, alongside CPA to asses, plan, deliver and review care. The Model uses 21 care clusters to describe a set of needs and each cluster attracts a number of different things: a range of interventions, drawn from NICE guidance a number of different professional roles to be involved for given amounts of time a clinical review interval, and an anticipated course for each condition ranging from six weeks to long term. Each service user will have their needs allocated to a care cluster, with the aid of the Cluster Allocation Support Tool (CAST) tool. Specific Care packages will be offered according to each cluster. Although each cluster has its own specific review period, currently review will be undertaken following assessment, at each CPA review, and at points when needs change significantly. There will be regular re-assessment/re-cluster and CPA review according to clinical guidelines and a clinical decision made as to whether the service user will step up, or step down within AWP or outside of the service. Clinical Duties Provide clinical leadership and consultation for Early Intervention Team members, supporting them in providing focussed home based care designed to enable recovery Assess and treat patients as requested by Care co-ordinators working closely with GPs in ensuring continuity of care between Primary and Secondary sectors Work with colleagues in the Intensive and Inpatient services to lead the acute care pathway Participate in Job Planning and Appraisal processes Support and participate in Clinical Governance and Quality Improvement. Comply with all Trust policies including Equal Opportunities and to promote equality of access to healthcare at all times. The post holder will be encouraged to develop an area of special clinical interest in line with service needs. This may be practice-based research. The Trust expects engagement in Continuing Professional Development (CPD) and, in view of this, appropriate study leave and expenses will be supported. CPD will include continued training to maintain the role of Educational supervisor for trainees. Responsible for the mental health care of service users in line with new ways of working, monitoring and evaluating service delivery and care through regular reviews using evidence based interventions. To actively participate in team caseload management and supervision. To participate in MHA assessments where appropriate. The Consultant has a duty to collaborate with key personnel to ensure the smooth transition through any changes in the care pathway The Consultant is expected to contribute to the development of EI services in line with best practice and nationally accredited standards. Responsible with the Service team members for the identification, monitoring and evaluating of the care package and care cluster delivery for individual service users, identifying service users who require access to specialist services and/or require stepping up or down to other services. Page 6 of 25 Avon and Wiltshire Mental Health Partnership Trust Understand and participate in the maintenance, monitoring and evaluation of service specific performance indicators and targets. The Consultant is expected to provide visible and clear leadership at team level. . Management Responsibilities The post holder will be expected to be a key player in the development of the service. The post holder will be expected to provide medical leadership and participate in strategic development within the service. To support and participate in Clinical Governance and Audit and in particular the design and completion of any service evaluation. Administrative Responsibilities: The post holder will undertake administrative duties associated with the running of his/her clinical work. Training and Supervision of Junior Medical Staff The post holder will be expected to undertake the supervision and training of medical students, junior and middle grade doctors, Specialist Registrars and multidisciplinary colleagues, where appropriate. As a junior doctor trainer the post holder will be responsible for their professional supervision and management. Where appropriate, the post holder will be named in the educational contract of junior staff as the person responsible for overseeing their training. The post holder will be expected to be involved and contribute to the regular teaching programme for trainee medical staff and medical students. Quality Improvement Leads and manages a team in a way that supports the development of a culture of continuous improvement and learning. Utilises a quality improvement approach to think systemically about complex problems, develop potential change ideas and test these in practice using a systematic QI methodology.
