Tell the class that hazards can be divided into four categories. Sensory irritation occurs from stimulation of receptors on trigem- inal nerves. In general, chemically reactive compounds are more potent than non-reactive congeners. Animal studies allow prediction of sensory irritation effects in humans; the concentration–effect relationships are often steep. In humans, thresholds and suprathreshold effects can be obtained from short-term (seconds) exposures and from longer exposures (hours). Sensory irritation may develop over time and odour cues may influence reported sensory irritation symptoms; generally, the slope of the irritant effect is steeper than the slope of odour cues. A best available no-observed-adverse-effect level (NOAEL) should be based on a combined estimate from the three types of study. The NOAEL/5 is considered sufficient to protect individuals not especially sensitive. The present knowledge suggests that espe- cially sensitive individuals may be protected by an additional uncertainty factor (UF) of 2, suggesting a combined UF of 10. In published studies, the combined UF is up to 300, highlighting the need of evidence-based UFs. -

Chronic cough and throat clearing: guide for patients Throat problems produce a number of symptoms such as cough, throat clearing, irritation in the throat and mucus. This leaflet helps to explain the normal function of the throat and how some of these symptoms may be produced and treated. Why is the throat so sensitive? Given that we eat drink and breath through the same hole – the mouth, it is remarkable that we manage to direct all the food and drink into the gullet (oesophagus) and air into the windpipe (trachea). If we were not designed in this way we would soon drown in our own saliva or food would block our airways. Our ability to do this is largely due to the fact that our airways are guarded by extremely sensitive tissues, which can detect solids and liquids and will divert them towards the gullet or expel them from the airway by coughing. This protective reflex is very important and sensitive but in some circumstances it can be the source of throat problems due to over stimulation of the lining of the throat and “up-regulation” of this reflex. Some of the common factors which cause throat problems, along with treatment suggestion are described below. Reflux This is due to acidic stomach contents passing upwards to the throat. Not surprisingly, this causes an irritation in the throat as a result of a low level chemical burn. Dietary alterations, postural care and medications are the best treatments for this condition. Your doctor will give you more advice about this if necessary. -

Inhaled and Oral Corticosteroids Corticosteroids (steroids) are medicines that are used to treat many chronic diseases. Corticosteroids are very good at reducing inflammation (swelling) and mucus production in the airways of the lungs. They also help other quick-relief medicines work better. The steroids (corticosteroids) used to treat asthma and other chronic lung diseases are not the same as anabolic steroids, used illegally by some athletes for bodybuilding. Corticosteroids do not affect the liver or cause sterility. Does the Body Make Steroids? Corticosteroids are similar to cortisol, a hormone produced by the adrenal glands in the body. Cortisol is one of the body's own natural steroids. Cortisol is essential for life and well being. During stress, our bodies produce extra cortisol to keep us from becoming very sick. Normally the adrenal glands release cortisol into the blood stream every morning. The brain monitors this amount and regulates the adrenal function. It cannot tell the difference between its own natural cortisone and that of steroid medicines. Therefore, when a person takes high doses of steroids over a long time, the brain may decrease or stop cortisol production. This is called adrenal supression. Health care providers generally decrease a steroid dosage slowly to allow the adrenal gland to recover and produce cortisol at a normal level again. If you have been on steroids long- term do not stop taking them suddenly. Follow your doctor’s prescription. What are Some Steroid Medicines? Steroid medicines are available as nasal sprays, metered- dose-inhalers (inhaled steroids), oral forms (pills or syrups), injections into the muscle (shots) and intravenous (IV) solutions. -
