    OE OPEN ENROLLMENT APPLICATION Optional: Complete only if you are applying to a Magnet Program or school other than your home school. The Phoenix Union High School District (PUHSD) has many choices to offer high school students. Open enrollment enables Arizona students to attend a public school whether or not they live within the school or district boundaries. At Phoenix Union, we welcome students to explore how they can experience high school at one of our 15 high schools or in one of our many Magnet Programs. The following conditions apply to the Open Enrollment Program: 1. An Open Enrollment Application must be completed. 2. Transfer is subject to the capacity limit established for the school, program, and/or its grade level. 3. Student transportation will be the responsibility of the parent/legal guardian with the exception of students attending a Magnet Program or Metro Tech High School. Students that are attending a Magnet Program that is outside of their home attendance zone or Metro Tech High School can request that PUHSD provide them with available transportation. 4. Providing false information on this form may result in the request being denied or admission revoked. ADMISSION GUIDELINES Open Enrollment Applications must be completed by the individual requesting enrollment, and must be signed by the parent/legal guardian and student. Consideration for admittance will be determined by the administration of the receiving school. PHOENIX UNION HIGH SCHOOL DISTRICT NEWS RELEASE For Immediate Release May 15, 2015 Contact: Craig Pletenik (602) 764-1530 Michelle Halama-Alcantar (623) 845-4771 Phoenix Union College-going Seniors Helped by Foundation Scholarships PHOENIX--The Phoenix Union Foundation for Education awarded scholarships this year totaling $88,000, assisting 60 District seniors in furthering their education at Arizona colleges and universities next year. The amount was $18,000 more than last year. The Foundation for Education, a 501 (c) (3) that supports pathways to success in college, career and life, and provides support for District-wide, college-going student programs and college scholarships, began in 1985 as the Phoenix Union Partnership of Business and Education. In recent years, it has concentrated on generating need- based scholarships for PUHSD students. Scholarships ranged from $100 annually to $8,000 over a four-year academic career, distributed from a pool of scholarship resources, from individual and organization donors to employee contributions and fundraising events. Five seniors received four-year scholarships of $8,000 each--Joy Cocone, Alhambra; Omar Tay, Carl Hayden; Lal Len Mawi, Camelback; Jesslynne Alvarez, Central and Edwin Perez, Trevor Browne. AzCAN is pleased to have been selected by NCAN to participate in the Kresge FAFSA Completion Challenge in 2016. Through this project, Phoenix Union High School District (PUHSD) worked collaboratively with AzCAN and aligned community partners, like Be A Leader Foundation and Arizona State University (ASU) to exceed the 40% FAFSA completion goal set by the City of Phoenix and increase college-going rates for the graduating high school class of 2017. The grant funds supported the district’s college marketing campaign and FAFSA drives as well as summer collaboration efforts with high school counselors. Based on the district’s collaborative FAFSA completion success, it received additional reward money from NCAN to continue raising FAFSA completion rates for the high school class of 2018. Not only did the Kresge FASFA Completion Challenge help low-income and underrepresented students in Phoenix successfully access federal state and institutions financial aid, it also catalyzed a community-led movement to increase FAFSA completion rates for all high school seniors in Arizona. In Spring 2017, the Arizona State Board of Education, guided by stakeholders, educational partners and AzCAN members, redefined the meaning of college and career ready to include completion of the FAFSA in Arizona’s accountability system.

Scribner will travel to Washington to attend his first meeting and swearing-in ceremony Oct. 6-7. Scribner joins a select group of educational leaders nationwide who will work with White House staff, the U.S. Department of Education and other federal agencies to address the unique educational challenges faced by Hispanics. The commission, originally established by George H.W. Bush in 1990, includes representatives from early childhood education, K-12, colleges and universities as well as philanthropic foundations, nonprofit and community-based organizations. Scribner, 45, is now in his fourth year leading the 26,000-student, 16-school Phoenix Union district. He served as superintendent of the Isaac Elementary District in Phoenix for five years prior to his post with the Phoenix Union High School District. PHOENIX UNION HIGH SCHOOL DISTRICT NEWS RELEASE For Immediate Release May 13, 2016 Contact: Craig Pletenik (602) 764-1530 Michelle Halama-Alcantar, [email protected] Phoenix Union College-going Seniors Helped by Foundation Scholarships PHOENIX--The Phoenix Union Foundation for Education awarded scholarships this year totaling $88,330, assisting 48 District seniors in furthering their education at Arizona colleges and universities next year. This year’s amount puts the Foundation’s scholarship totals over $1 million dollars since its inception. The Foundation for Education, a 501 (c) (3) that supports pathways to success in college, career and life, and provides support for District-wide, college-going student programs and college scholarships, began in 1985 as the Phoenix Union Partnership of Business and Education. In recent years, it has concentrated on generating need- based scholarships for PUHSD students. Scholarships ranged from $100 annually to $8,000 over a four-year academic career, distributed from a pool of scholarship resources, from individual and organization donors to employee contributions and fundraising events. Seven seniors received four-year scholarships of $8,000 each—Vivian Mercado, Bioscience; Ellen Lam, Central; Abigail Orona, Cesar Chavez; Ricardo Ornales Montes, Carl Hayden and Andrea Gonzalez, Camelback. Suntec Concrete provided two more $8,000 scholarships to Yurian Bustamante of Franklin and Melina Florez of Metro Tech. Named scholarships to 17 students totaling $18,630 were supported by various community, business and PUHSD staff, family and friends to recognize loved ones, support students, or as part of a fundraising sponsorship.
