We are pleased that you chose MetroPlus GoldCare I for your health plan. Please read the information below. It will help you choose a Primary Care Provider for you and your family if you have not done so already. It will also give you other important information. Your Primary Care Provider When you join MetroPlus, you choose a MetroPlus participating physician or nurse practitioner to be your Primary Care Provider (PCP). Your PCP will take care of your health care needs and refer you to other health care providers if you need them. We have many PCPs in our network. Your PCP can be a MetroPlus Medical Doctor (MD), a Doctor of Osteopathic Medicine (DO) or a Nurse Practitioner (NP). Female members can choose a MetroPlus Obstetrician/Gynecologist (OB/GYN) as their PCP. Pregnant women can choose a MetroPlus Certified Nurse Midwife (CNM) or Licensed Midwife (LM) as their PCP. Members who are 65 or older can choose a MetroPlus Geriatric Provider as their PCP. How to Choose Your Primary Care Provider If you have a PCP that is a MetroPlus provider, you can continue to see that provider. If you don’t have a PCP, you can select one in this Provider Directory. If your children have MetroPlus, you can choose a PCP for each child. You can find a PCP who speaks your language, is associated with a particular hospital or facility, has an office in or near a particular zip code that is convenient to you and that is wheelchair accessible. The Provider Directory also indicates PCPs that are not accepting new patients. You can also use the MetroPlus web site, www.metroplus.org, to find a PCP. On the website, go to the “Find a Doctor/Provider” tab and then select “Find a Primary Care Provider.” You can search for a PCP who meets your needs, speaks your language, is associated with a particular hospital/facility, has an office in or near a particular zip code that is convenient to you and that is wheelchair accessible. If you choose a new PCP, call and make sure the provider is taking new patients. This will ensure that you can have the PCP you want. If your PCP is not listed in this Directory or you would like help in selecting a PCP, call MetroPlus Member Services at 1-877-475-3795 (TTY: 711), Monday – Saturday, 8 am – 8 pm. After hours, please call 1-800-442-2560. 5 How to Use This Directory This directory only includes providers in Queens County. If you would like a provider/pharmacy directory from another participating county, please visit our website at www.metroplus.org or call 1-800-303-9626 (TTY 1-800-881-2812).
