Medicine Department A. Revised Clinical Privilege Application Card for Department of Revisions: 47.00 – Nephrology Core Privileges to admit, evaluate, diagnose, provide consultation, perform history and physical exam and nonsurgical treatment to patients of all ages – except where specifically excluded from practice and except for those special procedures listed be- low – presenting with illnesses and disorders of the kidneys.

Privileges include, but are not limited to, catheter insertion, and placement of temporary vascular access, plasmapheresis and hemodialysis and placement of catheter.

B. Revised Clinical Privilege Application Card for Department of Medicine – /Medical 44.51 Medical Oncology Core Privileges to admit, evaluated, diagnose, provide consultation, perform history and physical exam and treat patients of all ages with cancer.

Privileges include bone marrow biopsy and interpretation, lumbar puncture, plasmapheresis, administration of chemothera- peutic agents and biological response modifiers through all therapeutic routes, and management and maintenance of in- dwelling venous access catheters.

Surgery Department A. Revised Clinical Privilege Application Card for Department of Revisions: 98.50 – Urology Core Privileges to admit, evaluate, diagnose, consult, perform history and physical exam, and provide non-surgical and surgical care to patients of all ages – except where specifically excluded from practice and except for those special procedures listed below – to correct or treat illnesses, disorders, diseases, or injuries of the genitourinary systems.

Privileges include the following minor procedures: Circumcision; meatotomy; cystoscopy; hydrocelectomy; orchidectomy; orchiopexy-scrotal approach, urethrostomy; vasectomy; contigen / collagen implant; Spermatocelectomy, indigo laser abla- tion of the prostate, injection/infusion of indigo carmine, transurethral needle ablation, Prostatron, Gynecare TVT Secure; Gynecare Prolift; epididymectomy; placement of male sling; placement of mid urethral sling (ie.
