In medicolegal situationsitis typicaltodescribe theconsequences ofa stabbingincidentinrelative termsthatare qualitativeanddescriptivewithoutbeing numerically quantitative. Here, the mechanical variables involved in the possible range of knife–tissue penetration events are considered so as to determine the necessary parameters thatwould needto becontrolled in a measurement device. These include knifegeometry,in-planemechanical stressstateof skin, angle and speed of knife penetration, and underlying fascia such as muscle or cartilage. Four commonly available household knives with different geometries were used: the blade tips in all cases were single-edged, double-sided and without serrations. Appropriate synthetic materials were usedto simulate the response of skin, fat and cartilage, namely polyurethane, compliant foam and ballistic soap, respectively. The force and energy appliedby the blade of the knife and the out of plane displacement of the skin were all used successfully to identify the occurrence of skin penetration. -

NURSING Cricothyrotomy: Assisting with PRACTICE & SKILL What is Cricothyrotomy? › Cricothyrotomy (CcT; also called thyrocricotomy, inferior laryngotomy, and emergency airway puncture) is an emergency surgical procedure that is performed to secure a patient’s airway when other methods (e.g., nasotracheal or orotracheal intubation) have failed or are contraindicated. Typically, CcT is performed only when intubation, delivery of oxygen, and use of ventilation are not possible • What: CcT is a type of tracheotomy procedure used in emergency situations (e.g., when a patient is unable to breathe through the nose or mouth). The two basic types of CcT are needle CcT (nCcT) and surgical CcT (sCcT). Both types of CcTs result in low patient morbidity when performed by a trained clinician. Compared with the sCcT method, the nCcT method requires less time to set up and is associated with less bleeding and airway trauma • How: Ideally, a CcT is performed within 30 seconds to 2 minutes by making an incision or puncture through the skin and the cricothyroid membrane (i.e., the thin part of the larynx [commonly called the voice box])that is between the cricoid cartilage and the thyroid cartilage) into the trachea –An nCcT is a temporary emergency procedure that involves the use of a catheter-over-needle technique to create a small opening. Because it involves a relatively small opening, it is not suitable for use in extended ventilation and should be followed by the performance of a surgical tracheotomy when the patient is stabilized. nCcT is the only type of CcT that is recommended for children who are under 10 years of age - A formal tracheotomy is a more complex procedure in which a surgical incision is made in the lower part of the neck, through the thyroid gland, and into the trachea. -

When girls complete the requirements they will be awarded a Sharp Girl Wallet Card. The Leaders of AHG Troop NC0512 recognizes that AHGers look forward to using a pocket knife. It is our troop's goal that any interested AHGer learn how to safely use a pocket knife with her parent's permission. We have established the following mandatory policy for safe pocket knife use. AHGers in Troop NC0512 may carry and use pocket knives ONLY when they obey the following rules: Only AHGers that have earned a Sharp Girl Card may carry and use a pocket knife. Knives may only be carried on outdoor camping trips, hikes or other approved activities ONLY when permitted by the Troop Coordinator or other Leader. The Sharp Girl Card MUST is carried by the AHGer at all times when she is carrying or using her pocket knife. AHGers may use their pocket knife only when the Unit Leader or a designated Leader is directly supervising the AHGer's activities. The supervising adult MUST NOT be involved in any other activities while supervising AHGers’ use of knives. AHGERS MUST NEVER CARRY THEIR POCKET KNIVES TO SCHOOL OR AHG ACTIVITIES AT SCHOOL. Under no conditions are switchblade knives or sheath knives permitted. How do you sharpen an Opinel knife ? Use an Opinel fine‐grain natural stone on a dry, degreased blade. As if you wanted to cut the stone, move the blade from the base towards the tip, at an angle of 18/20 degrees. A wide angle will thicken the cutting edge: it will be more resistant but less sharp. A small angle will thin the cutting edge: it will be less resistant but sharper. A burr forms on the opposite side of the blade. Remove this by passing this side over the stone, without pressing. To finish off, pass both sides over the stone, twice. 3. Can I put my Opinel knife in the dishwasher ? Folding knives are not designed to be washed in the dishwasher. You can wash them by hand, being careful never to leave them soaking in the water and to dry and grease them properly afterwards. Carey1 e-mail: [email protected] Cricothyrotomy Device Morgan Gwin Cricothyrotomy is one of the procedures used to ventilate patients with upper airway Andrew Kan blockage. This paper examines the most regularly used and preferred cricothyrotomy devices on the market, suggests critical design specifications for improving cricothyro- Roger Toogood tomy devices, introduces a new cricothyrotomy device, and performs an engineering evaluation of the device’s critical components. SAEMS PREHOSPITAL PROTOCOLS Cricothyrotomy I. Introduction A cricothyrotomy is an invasive surgical procedure aimed at obtaining a patent airway in a specific patient population. It should only be performed in the situations outlined below. In these situations, speed is of the essence. However, do not allow the urgency of the situation to take precedence over reasonable judgment or action. The indications and technique must be clearly documented whenever it is utilized. II. Indications A. Acute upper airway obstruction which cannot be relieved by other BLS and ALS maneuvers, including any available supra-glottic advanced airway technique (laryngeal mask airway -- LMA, Combitube, King Airway, etc.) B. Patient in respiratory arrest with neck injury or head injury who cannot be ventilated adequately with bag/valve/mask and in whom orotracheal and nasotracheal intubation cannot be accomplished. After intubation attempts have failed, or is clearly not possible, attempt to ventilate the patient with BVM technique. If this also fails to result in adequate ventilation, then proceed with surgical cricothyrotomy. C. Patient who is in respiratory arrest with facial injuries which preclude endotracheal and nasotracheal intubation, and who cannot be adequately ventilated with BVM technique. D. Patient with neck injury in which tracheal intubation either cannot be accomplished or has failed to ventilate the patient due to damage to the airway, and who cannot be adequately ventilated with BVM technique. E. Other patients who are apneic and in whom all other BLS and ALS airway techniques have failed and, the time to the receiving hospital is prolonged. III. Contraindications A. Traumatic obliteration of trachea. -
