How to find us:

5 minutes’ walk from Havant train station: 10 minutes’ walk from Havant Bus station. We are past the police station and council plaza, next right after leisure centre. Parking on site however, you must give registration to receptionist and request you are put on the white board. If you do not you will be fined.

Nature of Service provided: Welcome to the Older Persons Mental Health Team in Havant Health Centre. We provide assessment, treatment and support to older people with mental illness in the community and across number of inpatient sites in . Our Community Mental Health Teams work across Hampshire to help patients live as independently as possible. We help older people experiencing mental health problems. We can assess their condition and provide them with care and support that’s tailored to their individual needs. We can also referrals to other teams, such as our specialist memory services. Our staff work in integrated care teams which see multi- disciplinary staff working together to provide a comprehensive care package to our patients. Our aim is to provide high quality integrated care and to work collaboratively with our service users to support their recovery, making a positive difference to their health, well-being and independence. We have a strong recovery ethos and are dedicated to promoting service user involvement at all levels. We look to accelerate work to improve needs-led services for patients: • in crisis • requiring liaison services in acute hospitals • accessing psychological therapies. We also look to work across evidence-based care pathways so we can provide personalised care and treatment based around the needs of the person. These teams include professionals from a number of organisations including health professionals (Physical and Older Peoples Mental Health) from Southern Health, Adult Services colleagues from and local doctors from GP practices. Additional support may join the team when needed including voluntary services, the ambulance service and younger adult mental health professionals. Integrated Care Teams (ICTs) aim to develop person centred care planning and support for eligible adults, particularly elderly people and those with long term conditions, complex needs or who are approaching the end of their life, ensuring that services: Maximise independence and functioning Minimise intervention, unnecessary hospital admission and premature admission to long term care

Primarily organic and functional patient assessment & diagnosis will follow up.

Smart dress, no jeans, trainers. No uniform

Very specific is the lone working policy. We have our own SOP for this as well as the trust policy. Must be adhered too at all times.
