Everest and Oxygen—Ruminations by a Climber Anesthesiologist By David Larson, M.D. Gaining Altitude and Losing Partial Pressure with Dave and Samantha Larson Dr. Dave Larson is an obstetric anesthesiologist who practices at Long Beach Memorial Medical Center, and his daughter Samantha is a freshman at Stanford University. Together they have successfully ascended the Seven Summits, the tallest peaks on each of the seven continents, a feat of mountaineering postulated in the 1980s by Richard Bass, owner of the Snowbird Ski Resort in Utah. Bass accomplished it first in 1985. Samantha Larson, who scaled Everest in May 2007 (the youngest non-Sherpa to do so) and the Carstensz Pyramid in August 2007, is at age 18 the youngest ever to have achieved this feat. Because of varying definitions of continental borders based upon geography, geology, and geopolitics, there are nine potential summits, but the Seven Summits is based upon the American and Western European model. Reinhold Messner, an Italian mountaineer known for ascending without supplemental oxygen, postulated a list of Seven Summits that replaced a mountain on the Australian mainland (Mount Kosciuszko—2,228 m) with a higher peak in Oceania on New Guinea (the Carstensz Pyramid—4,884 m). The other variation in defining summits is whether you define Mount Blanc (4,808 m) as the highest European peak, or use Mount Elbrus (5,642 m) in the Caucasus. Other summits include Mount Kilimanjaro in Kenya, Africa (5,895 m), Vinson Massif in Antarctica (4,892 m), Mount Everest in Asia (8,848 m), Mount McKinley in Alaska, North America (6,194 m), and Mount Aconcagua in Argentina, South America (6,962 m). -

climb Mount Elbrus actionchallenge on Mount Elbrus Our ascent of Mount Elbrus begins in the foothills of the lower Caucasus. Elbrus, an inactive volcano, is one of the famous Seven Summits and, at 5,642 m, is the highest mountain in Europe. It’s a fantastic, rewarding challenge - all you need is an adventurous team spirit, good fitness and two weeks out of the office. We begin our challenge from Kislovodsk, in the heart of the lower Caucasus. From here we carry out acclimatisation climbs, ascending progressively higher each day, practicing our crampon, rope and ice axe techniques. From Base Camp we climb through the rocky moraine of the Mikelchiran Glacier to our next camp at 3,600 m far above the grasslands, with stunning views across the glaciers and the lower Caucasus range. Together we make the climb to Lenz Rocks, crossing several crevasses, and prepare for our bid to reach the summit of Mount Elbrus. The ascent to the top is tough but exhilarating, beginning with a glacier climb to the saddle between the two summits at 5,416 m. We work as a team to climb up the next steep section to the ridge and, after a final short push, we reach the summit at 6,642 m. We are rewarded with a breathtaking view over the Caucasus from the highest point in Europe. Radiological Protection and Radioactive Waste Management
Occupational radiation doses of United Kingdom high altitude mountain guides as a result of cosmic ray exposures. Robert W Kerra* aRP Alba Ltd, Geanies House, Fearn, Tain, Ross-shire, Scotland, IV20 1TW, UK. Abstract UK based mountain guides lead multiple expeditions throughout the course of a year. They will receive radiation doses from cosmic rays during air travel and their time spent at elevated altitudes in the mountains of the world. These radiation doses are received as part of their employment. This paper illustrates that UK based high altitude mountain guides can potentially receive greater than 1 milliSv per year of cosmic radiation dose in excess of what they would have received at ground level. These individuals are “occupationally exposed” to cosmic radiation as a result of their profession. The European Community Basic Safety Standards Directive 96/29/EURATOM does not apply to exposure to cosmic radiation prevailing at ground level. The highest “ground level” that a UK mountain guide may be working at is 8848m. The maximum flying altitude of some internal flights in the UK is 7925m. 96/29/EURATOM does apply to cosmic radiations being received by aircrews therefore there is an anomaly in radiation protection where the cosmic radiation exposures of aircrew operating for short durations at altitudes lower than mountain guides operating at high altitudes for prolonged times have to be taken into account. UK based high altitude mountain guides are undergoing planned occupational exposures to cosmic radiation whilst still on the ground. Consideration should be given by the legislative authorities to include the control and assessment of cosmic radiation exposures of professionals likely to receive greater than 1 milliSv per year of cosmic radiation in excess of what would have been received in their home country at ground level. -

Book Reviews 1988 COMPILED BY GEOFFREY TEMPLEMAN Touching the Void Joe Simpson ]onathan Cape, 1988, Pp174, £10·95 In 1985 Joe Simpson and Simon Yates set out to climb the W face of Siula Grande in the Peruvian Andes. They succeeded in their objective after three days and started on the descent of the N ridge in atrocious snow conditions. After a night on the ridge they continued their slow progress, breaking through cornices and having short falls, until one such fall resulted in Simpson breaking his leg. Yates started lowering Simpson down the W face, and they had almost reached the bottom when Simpson shot off over an overhanging cliff and was left hanging. After a considerable length of time in which nothing could be done, Yates cut the rope. Giving Simpson up for dead, he made his way back to Base Camp and, during the next three days, sorted out Simpson's belongings, burnt his clothes and prepared to leave camp. It was then that Simpson arrived in a state of exhausted delirium, having escaped from the deep crevasse into which he had fallen and crawled and hopped over the long glacier and moraine. The book recounts the story in a graphic and frank way. The psycho logical trauma following the decision to cut the rope; the thoughts engendered by being trapped in a deep crevasse with a broken leg; the embarrassment of the eventual meeting: these things are written about in depth and with a clarity that is only too well understood. You know Simpson will survive, but the tale is like a thriller - I almost said, a cliff-hanger - progressing from one dramatic situation to the next. -

10 Interesting Facts About Mount Kilimanjaro 1) Mount Kilimanjaro is the tallest mountain in Africa, making it one of the seven summits. It very popular with both experienced hikers and first time adventurers because it is considered to be the easiest of the seven summits. Scaling the mountain requires no technical skills or equipment, such as rope, harness, crampons or ice axe. It is a hiking peak, not mountaineering. 2) Kilimanjaro is not only Africa’s tallest peak, but also the world’s tallest free standing mountain. The summit, named Uhuru Point, is 5,895 meters (19,341 feet) above sea level. While most high mountains are part of ranges, such as Mount Everest’s Himalayan Mountain Range, free standing mountains like Kilimanjaro are usually a result of volcanic activity. 3) Kilimanjaro lies just 205 miles from the equator, in the country of Tanzania. The equator is an imaginary line that divides the Northern Hemisphere and Southern Hemisphere. When early explorers reported seeing glaciers on the top of Kilimanjaro, people did not believe them as they thought it was impossible for ice to form so close to the hot, equatorial sun. Scientists now believe that the glaciers shrink and then regrow during the planet’s ice ages. 4) The origin of the name Kilimanjaro is not certain. The most popular answer is that the name comes from the Swahili word “Kilima” (mountain) and the Chagga word “Njaro” (whiteness). Another possibility is that Kilimanjaro is the European pronunciation of a KiChagga phrase meaning “we failed to climb it.” 5) Now approximately 30,000 people climb Kilimanjaro every year. -

ECENTL Y it has become kno\vn that a number of very high Andean mountain tops had not only been ascended but also permanently occupied by the Indians, possibly as much as three centuries before de Saussure's ascent of Mont Blanc. They climbed peaks of up to 22,ooo ft., they constructed shelters on or near their tops, and they used the high places as watch-towers or as sacrificial shrines. Some authorities believe that this activity took place as early as the late four teenth century, though we cannot prove that some of it did not take place long after, possibly as late as the nineteenth century. These Indian accomplishments have been left unmentioned in practi cally all mountaineering history books. In this article, which may be the first to attempt a comprehensive survey,1 my purpose is to review briefly the location and the nature of each discovery. The area in which these Indian mountain ascents took place is what in physical geography is known as the Atacama desert (although this name is nowadays used in political and cultural geography for a much more restricted area). It is a treeless, sandy and volcanic waste-land seldom visited by mountaineers. It stretches from the neighbourhood of Arequipa, in Peru, as far south as Elqui in Chile; to the east it reaches the Andean slopes that face the jungles of Argentina and Bolivia, and to the west, the Pacific Ocean. -

The Aconcagua Expedition
The Aconcagua Expedition Steve took some time recently to talk with us about what we could expect of the Aconcagua Expedition. Six people form the Expedition Team, flying in from around the country. They’ll gather in Mendoza Argentina, altitude about 2,500 feet at the base of the Andes Mountains. It is summer in South America and the temps may reach 90 degrees on Friday. A nice break from the snows of Alaska! From there on Day 3, they will take a taxi (or several taxis to hold the Team and all their gear) to Puente del Inca, altitude 8,900 feet. In the Andes, it will be cooler, maybe reaching the 60’s during the day. At this point, the Team will be at an elevation above all major cities in the US (Denver is only 5,900 feet, and your commercial plane is pressurized to simulate about 7,000 feet; individual mountains in a number of states do have higher elevations). And this is where the climb really begins. They will be taking the “Normal” route. Day 4 is an uphill hike, with mules to carry their gear, up to Confluencia (11,000 feet). Days 4-6 will be spent in the climb and adjusting to the higher elevation. The camp is almost luxurious, as nearby villagers know climbers are avid customers of pizza and Internet. From here on, Team members will carry their own gear as they climb. Each person has approximately 120 pounds. They can carry about 60 lbs at a time. To handle this situation, they practice a routine that adheres to the mountaineer wisdom of "Climb high, sleep low".
