Sport Entry Forms must be completed for each eligible athlete or team qualified and entered in a Youth Olympic Competition. For the SINGAPORE 2010 Youth Olympic Games, the International Olympic Committee (IOC) and the SINGAPORE 2010 Organising Committee for the Youth Olympic Games (SYOGOC) made a co-operative effort to streamline the Entries process. 1.2 Final Entries The Youth Olympic Sport Entries & Qualification System has been developed into a two-part process, so as to manage the official registration of athletes for the SINGAPORE 2010 Youth Olympic Games. The first phase is Qualification Verification. This began in September 2009 and will run through the end of the qualification period. The second phase is Final Entries, where NOCs must submit the names of all qualified athletes who will be competing in the SINGAPORE 2010 Youth Olympic Games. SYOGOC must receive Sport Entry Forms from participating NOCs by midnight (Singapore time), on 15 July 2010, with a copy forwarded to the relevant International Federation. Team Managers or any appointed person from the NOC are asked to give the Event Delegate written notification of any amendments to the content of the Sport Entry forms, prior to the sports Technical Meeting or Draw, after which no further amendments will be accepted. 1.3 Entries Checklist Sport Entry Forms (Hardcopies + Softcopies in CD ROM + Microsoft Excel file) SEQ Manual (Hardcopy + Softcopy in CD ROM) 1 1.4 Entry Forms In addition to this manual, NOCs will receive the following Entry Forms: Sport Entry Forms For all Athletes A Sport Entry Form (individual or team) is required for each athlete/team eligible to participate in the SINGAPORE 2010 Youth Olympic Games. This form officially registers the athlete/team for Youth Olympic competition and is to be completed by the NOC in conjunction with the respective National Federation (NF). Sport Entry Form should only be submitted for those athletes/team who have qualified and have been selected to compete by the NOC. The Sport Entry Form for all athletes must be submitted to SYOGOC by 15 July 2010. Note: Refer to both ‘Specific Instructions by Sport’ and ‘Sport Entry Forms’ sections of this manual for copies of all Sport Entry Forms and their accompanying instructions. Sample of Partially Completed Form 2 3 1.5 Deadlines Applications for Accreditation (For all potential athletes) 1 May 2010 Sport Entry Forms (For qualified and eligible athletes only) 15 July 2010 The completed Sport Entry Forms for athletes/team must reach SYOGOC no later than midnight (Singapore time) on 15 July 2010. Each qualified athlete/team must be properly registered through the Entries process. In addition, the Chef de Mission of the respective NOC must have completed the Delegation Registration Process (DRP) before arriving in Singapore. A complete and accurate Sport Entry Form must be submitted to SYOGOC for each athlete/team entered in the SINGAPORE 2010 Youth Olympic Games. The co-operation of the NOCs in the adherence to entry deadlines is essential for facilitating and concluding the DRP before the Chefs de Mission arrival into Singapore. 1.6 Return Address All completed Sport Entry Forms must be sent to the address below. OR d) Regular Mail (must reach SYOGOC no later than midnight - Singapore 4 time on 15 July 2010). 5 1.7 General Instructions A Sport Entry Form must be completed for each athlete entered in the SINGAPORE 2010 Youth Olympic Games, qualified to compete according to the qualification system of the respective IF, and eligible to participate within team delegation quotas.
