Subsection (2)- "The copy of the register shall indicate with reference to every person whose name appears therein whether the person is the holder of an annual practising certificate for the then current year, and whether he is registered as a medical practitioner or conditionally registered. -

It delivers approximately 2,500 babies per annum and has one maternity operating theatre. As a level-one stand alone unit Waitakere Hospital only employs senior specialists. Purpose The purpose of this document is two part: • To ensure the woman and her family, involved in this accident, are fully informed as to the events surrounding the accident and the actions being taken by Waitakere Hospital and the Waitemata District Health Board. • To ensure that all surgical service providers in New Zealand have the opportunity to share in what was learnt during the investigation at Waitakere Hospital related to fire accidents in operating theatres. This investigation has sought to be comprehensive and adopt a systems approach that supports a culture of continuous improvement. This culture can actively support health professionals, and their organisations, to improve the safety of healthcare to the benefit all New Zealanders. The format of previous years has been followed but there is greater statistical analyses of the performance of prostheses especially for the hip and knee. As well as Kaplan Meier curves we have introduced revision rates per 100 component years which statisticians consider is the more accurate way of deriving our revision rates when analysing data with widely ranging follow-up times. -

The report was presented to the committee at its meeting on 28 April 2009. This is my fourth and final annual report for the Northern Y Regional Ethics Committee. This year has been another full year for the members of the committee, as it has for the members and chairpersons of the other six ethics committees. During 2008, the committee reviewed 119 full applications and 114 expedited applications. The expedited review process has minimally reduced the work for the full committee, but has increased the workload for chairpersons, particularly deputy chairpersons. The appointment of suitably qualified committee members continues to be a problem. Vacancies increase the workload and pressure on existing members and can cause gaps in the range of committee expertise. I encourage the Minister of Health to consider reviewing the appointment process and to fill vacancies.
