Abbey Grange Medical Practice Who Have Applied to Close the Branch Leeds West Abbey ClinicalGrange Commissioning Group Medical Practice Proposed closure of branch GP surgery within Holt Park Health Centre 1. What this document is about The purpose of this document is to inform patients and staff of the proposal to close Holt Park branch surgery. The surgery is a branch of Abbey Grange Medical Practice who have applied to close the branch. The closure will have an impact on patients who access services provided at the branch surgery. We would like to stress that this closure would only affect the GP surgery. Holt Park Health Centre will remain open and continue to provide all the other services offered at the site. This document outlines the proposed changes and gives patients the opportunity to comment on the proposed closure. We hope you will read this document and feedback your thoughts by Wednesday 30 November. 2. Background Following the merger of Abbey Medical Centre and Moor Grange Surgery in April 2015, a small number of patients still choose to attend the Holt Park branch however these numbers have reduced considerably over the last year. We have been working closely with our patients, our staff and ward councillors and the local MP to look at options for our site in Holt Park. Based on the fall in patients accessing the site and feedback from our discussions we are proposing to close our branch practice at Holt Park. We have outlined some of the reasons for this below. • More people are choosing to attend our main sites at Abbey Medical Centre and Moor Grange Surgery • In recent months we have had to reduce the number of clinics we offer at Holt Park due to a decrease in attendance. At present we estimate that up to 150 of our 8,800 patients attend an appointment at the Holt Park branch each week. We find many Holt Park patients choose to use Abbey Medical Centre and Moor Grange Surgery in order to see the doctor or nurse of their choice or to take advantage of the extended opening hours available at these two sites. • People are increasingly choosing to collect their prescriptions from or have them delivered by the pharmacist and we have seen a reduction in the number of patients calling to reception. • We have extended our opening times at Abbey Medical Centre to make it easier for people to make an appointment. We have recruited additional staff to cover these extended hours but patients still tell us that we often don’t have enough staff to adequately cover reception and the phones. Moving our staff from Holt Park to the other sites will help us provide a more efficient service to our patients. • To make the best use of our staff time we propose closing the Holt Park site so that we can provide a wider range of services from the two others sites which also offer extended opening hours. We would like to make it clear that the proposed closure of the Holt Park surgery will not result in any job cuts. • Because of the size and layout of the space we have use of in Holt Park Health Centre we are unable to provide the range of services we offer at the main surgery sites. Closing date for your comments: Wednesday 30 November 2016 Abbey Grange Medical Practice offers the latest primary care services. Patients choosing to attend Abbey Medical Centre or Moor Grange Surgery will benefit from: • Recent and ongoing refurbishment of both premises; • Extended opening hours; • An extensive range of healthcare services; • Improved access to the doctor or nurse of your choice; and • Improved continuity of care from both the GP practice and the community and neighbourhood network teams for patients with long-term or complex health conditions. We hope that consolidating our services on two sites will help ensure we are able to deliver the most efficient, caring service to our registered patients. ALL the appointments currently offered at Holt Park would be transferred to the other two sites. This means we will still be able to offer the same number of appointments as we currently do. 3. What will this mean for me? If you currently use the branch surgery at Holt Park Health Centre you would be affected by the closure of the surgery. • You may have to travel a little further to your appointment, but we find that many patients who use Holt Park have been happy to attend the other sites as their first choice. • Your future appointments would be at either Moor Grange Surgery or Abbey Medical Centre. This would involve a 5 minute car journey to Moor Grange or a 10 min car journey to Abbey Medical Centre. The map overleaf gives you more information about travelling to each site. Benefits We understand that some of the issues identified above will be of concern however by using one of our other sites you will benefit from the below. • Extended opening times: Abbey Medical Centre is open 7am to 7pm Monday to Friday and Moor Grange is open 8am to 6pm Monday to Friday. In addition, patients registered with Abbey Grange Medical Practice can be seen for both routine and urgent matters at Ireland Wood Surgery on Saturdays and Sundays between 8am to 4pm and also on bank holidays. • Extensive range of healthcare services: You will be able to access a range of different services from either site, including: - Minor operations - Midwifery services - Travel vaccinations and advice - Cryotherapy (‘treatment using low temperature’ usually to remove skin lesions) - Family planning services including coil/implant insertion and removal - Sexual health screening Make use of online services, including ordering repeat prescriptions You can ask a local pharmacy to order, collect and deliver your prescription. You can also register for our online service which allows you to order repeat medications using our website www.abbeygrangemedicalpractice.co.uk. You can also make and cancel appointments online. Many patients are now finding that they can manage their repeat prescriptions without the need to attend the GP practice. Ro ad 4. What do I need to do? You are not required to do anything, your records will remain registered with Abbey Grange Medical Practice. You will still be able to see the same GP and nursing staff at the otherM sites. 8 5 You will not need to change doctors or move to a different practice unless thisA is something that you wish to do. We can provide you with details of other GP practices in the area. Both of our neighbouring practices in the Holt Park area have confirmed that their lists are open and that they are able to accept any new patients. Of course we hope that you will remain registered with Abbey Grange Medical Practice. However, we wanted to offer you the option of being able to choose another practice if your preference is to access a GP surgery in Holt Park.     Headingley Development Trust Promoting Headingley A Headline History of Headingley Headingley Village The beginnings of Headingley village are lost in the mists of time. But its situation in a prime location suggests that it might be very ancient. The village is located on the higher, drier ridge between what would have been the less hospitable valleys of the River Aire to the west and Meanwood Beck to the east. And it is in a saddle on the ridge, providing ready access between the valleys. [An off-set cross-roads is still central to Headingley.] The Anglo-Saxon Era During the sixth century, the Angles arrived, settling in the north, giving England its name – and also naming the village. Among their number were Headda and his family, who founded, or perhaps occupied, the settlement on the hill, which became known as ‘Headda’s people’s place’ – or ‘Head-ing-ley’. Under Headda’s descendants, the village evidently flourished. [One field remains, on Headingley Hill, now used for grazing.] The Viking Era The Vikings colonised the north in the ninth century, and Jorvik Scir (York Shire) became one of the fourteen shires of the Danelaw. The Shire was divided into Ridings (thirdings), each in turn comprising wapentakes (where votes were taken by a show of weapons). The West Riding included Skyrack Wapentake, extending from the Wharfe to the Aire, and named after the Scir Ac (or Shire Oak) meeting place on the edge of Headingley village green. So, a millennium ago, Headingley was the number one settlement in Skyrack Wapentake. [The oak stood for another thousand years, finally collapsing in 1941.] The Norman Era The north did not take kindly to William of Normandy’s conquest in 1066, and rebellion broke out.

The Boundary Committee for England is a committee of The Electoral Commission, an independent body set up by Parliament under the Political Parties, Elections and Referendums Act 2000. The functions of the Local Government Commission for England were transferred to The Electoral Commission and its Boundary Committee on 1 April 2002 by the Local Government Commission for England (Transfer of Functions) Order 2001 (SI 2001 No. 3692). The Order also transferred to The Electoral Commission the functions of the Secretary of State in relation to taking decisions on recommendations for changes to local authority electoral arrangements and implementing them.
