Hair is a derivative of the epidermis and consists of two distinct parts: the follicle and the hair shaft. The follicle is the essential unit for the generation of hair. The hair shaft consists of a cortex and cuticle cells, and a medulla for some types of hairs. Hair follicle has a continuous growth and rest sequence named hair cycle. The duration of growth and rest cycles is coordinated by many endocrine, vascular and neural stimuli and depends not only on localization of the hair but also on various factors, like age and nutritional habits. Distinctive anatomy and physiology of hair follicle are presented in this chapter. Extensive knowledge on anatomical and physiological aspects of hair can contribute to understand and heal different hair disorders. Keywords: hair, follicle, anatomy, physiology, shaft 1. We predicted that underweight individuals would eat less and overweight individuals would eat more during negative as well as positive emotional states and situations. Questionnaires to assess eating during emotional states and situations were distributed and collected in person in several major university and public libraries. Ninety questionnaires, representing for each gender the 15 most overweight, the 15 closest to normal weight, and the 15 most underweight, were analyzed. Gender had only minor effects on the eating ratings, and therefore the results are presented for the sexes combined. Underweight individuals reported eating less ( P=.000) than both the normal and overweight groups during negative emotional states and situations. More surprisingly, underweight individuals also reported eating more ( P=.01) than the other groups during positive emotional states and situations. Thus, part of the prediction was confirmed: the relative undereating by the underweight group, and the relative overeating by the overweight group during negative emotional states and situations. As compared to their usual eating behavior, undereating by underweight individuals during negative emotional states and situations was of a greater magnitude than their own overeating during positive states and situations ( P=.01). Undereating by underweight individuals when experiencing negative emotions may contribute to their low body weight. -

Wearing big or baggy clothes or dressing in layers to hide body and/or weight loss. 3. Obsession with calories and fat content of foods. 4. Obsession with continuous exercise. 5. Frequent trips to the bathroom immediately following meals (sometimes accompanied with water running in the bathroom for a long period of time to hide the sound of vomiting). 6. Visible food restriction and self-starvation. 7. Visible bingeing and/or purging. 8. Use or hiding use of diet pills, laxatives, ipecac syrup (can cause immediate death!) or enemas. 9. Isolation. Fear of eating around and with others. 10. Hiding food in strange places (closets, cabinets, suitcases, under the bed) to avoid eating (Anorexia) or to eat at a later time (Bulimia). 11. Flushing uneaten food down the toilet (can cause sewage problems). 12. Vague or secretive eating patterns. 13. They include Anorexia Nervosa, Bulimia Nervosa, Binge Eating Disorder, and Eating Disorder Not Otherwise Specified (EDNOS)1. These disorders are associated with a high level of morbidity and mortality; in fact Anorexia Nervosa has the highest rate of mortality of any psychiatric illness. They can be long- term, disabling disorders, and they carry substantial costs – economic, social and loss of life. As serious mental disorders with significant consequences for physical health and quality of life as well as mental health, eating disorders require the same quality, breadth and accessibility to research funding, prevention interventions, treatment options, and community awareness programs as other serious physical or mental illnesses. Eating Disorders have a significant and highly underestimated impact on Australian society: Physically, as well as mentally, the person with the eating disorder is profoundly affected. Here, we comment on the health complications and treatment options for individuals with obesity and comorbid eating disorder behaviors. It appears that in order to improve the healthcare provided to these individuals, there is a need for greater exchange of experiences and specialized knowledge between healthcare professionals working in the obesity field with those working in the field of eating disorders, and vice-versa. Obsessions with food, body weight, and shape may be signs of an eating disorder. These disorders can affect a person’s physical and mental health; in some cases, they can be life-threatening. But eating disorders can be treated. Learning more about them can help you spot the warning signs and seek treatment early. Remember: Eating disorders are not a lifestyle choice. They are biologically-influenced medical illnesses. Who is at risk for eating disorders? Eating disorders can affect people of all ages, racial/ethnic backgrounds, body weights, and genders. Although eating disorders often appear during the teen years or young adulthood, they may also develop during childhood or later in life (40 years and older). Remember: People with eating disorders may appear healthy, yet be extremely ill. The exact cause of eating disorders is not fully understood, but research suggests a combination of genetic, biological, behavioral, psychological, and social factors can raise a person’s risk. What are the common types of eating disorders? Common eating disorders include anorexia nervosa, bulimia nervosa, and binge-eating disorder. If you or someone you know experiences the symptoms listed below, it could be a sign of an eating disorder—call a health provider right away for help. -

After the exclusion of the most obvious causes, a concealed diuretic abuse associated with or without surreptitious vomiting and laxative abuse should be suspected, especially in young women concerned with their body image. A conclusive diagnosis may be difficult as such patients often vigorously deny diuretic intake[1]. Also, only a minority of patients with eating disorders (approximately 6%) abuse diuretics[2–4]. This case describes a woman with chronic hypokalaemia in whom a thorough medical history and proper laboratory tests were essential to an early and accurate diagnosis. -

Treatment of eating disorders 15.1 Introduction Please review the Trust document “Guidelines for the assessment and treatment of eating disorders” in the CAMHS Operational Policy. When screening for eating disorders one or two simple questions should be considered for use with specific target groups 1. Do you think you have an eating problem? 2. Do you worry excessively about your weight?’ Early detection may be helped by five screening questions using The SCOFF questionnaire. A score of two or more positive answers should raise clinical suspicion and lead to an in depth diagnostic evaluation. 1. Do you ever make yourself Sick because you feel uncomfortably full? 2. Do you worry you have lost Control over how much you eat? 3. Have you recently lost more than One stone in a three month period? 4. Do you believe yourself to be Fat when others say you are too thin? 5. Would you say that Food dominates your life? It is important to take into account that clients with eating disorders can develop Acute Kidney Injury through a variety of mechanisms associated with each condition. Clinicians should be vigilant in the monitoring of physical health especially serum creatinine and levels of hydration.3 15.2 Anorexia nervosa The following would represent a reasonable initial screen for Anorexia Nervosa in primary care if there are no other indications or diagnostic concerns: Full Blood Count, ESR, Urea and Electrolytes, Creatinine, Liver Function Tests, Random Blood Glucose, Urinalysis, ECG (should be considered in all cases and essential if symptoms/signs of compromised cardiac function, bradycardia, electrolyte abnormality and/or BMI less than 15 kg/m2 or equivalent on centile chart). -

Malnutrition is when someone is not eating enough or is underfeeding. In Western societies, malnutrition is often linked to overeating, which accumulates health risks. Yet you can also find incidents of undernourishment, for example in the elderly. Extreme diets excluding several food groups can lead to nutritional deficiencies. This is what is known as an imbalanced diet. If a food group is missing from or not sufficiently present in our diet, we also talk about malnutrition. DEFICIENCIES Iron is one of the most readily available elements in the world, but it is the one most often missing from people's diets. Two billion people suffer from a lack of iron. Meat and fish are two good sources of iron. Vegetables sometimes contain good quantities of iron but it is less easy to absorb. Absorption of iron can be improved by eating food rich in vitamin C at the same time. A lack of iron can be caused by intestinal parasites, loss of blood or higher requirements during growth. If diet is not sufficient alone, you can also make up for this lack of iron with dietary supplements. Malnutrition due to undernourishment is a major problem in some parts of the world, where most of the population suffers from various severe deficiencies. A similar gender disparity can be seen in eating disorders, where up to nine in ten sufferers are female (American Psychiatric Association, 2013). Studies have shown that most of the gender difference in depression occurs as a result of women experiencing a form of depression involving a number of body-centric symptoms, including headaches, weight changes, fatigue, and insomnia, which has been termed “somatic depression” (Silverstein et al., 2013). Some of the symptoms, such as a fear of becoming fat, restricting or binging, and an abnormal focus on shape, are also characteristic of eating disorders (American Psychiatric Association, 2013).
