Missouri Division of Behavioral Health National Guard Initiative The Division of Behavioral Health is collaborating with the Missouri National Guard to offer substance use consultation and evaluations, assessments, and treatment services at no cost to currently serving Missouri National Guard Service Members. This initiative applies to both Air and Army National Guard. Steps to access services: 1. Find a DMH contracted provider. Either visit our provider directory https://dmh.mo.gov/alcohol- drug/help or call the Division for Behavioral Health (800) 575-7480 for assistance. 2. Schedule an appointment with the provider. 3. When you show up for your appointment, present your military identification card and explain that you are “currently serving in the National Guard and you are here for the no cost substance use services.” If you have any health insurance, you will still provide it but there will be no copay. The evaluation, consultation, and all subsequent services delivered are confidential. Information about your participation will only be released to the National Guard if you sign a written release. Note: If you are being command referred for evaluation by the National Guard, please present: • PC Referral Sheet, DA 4465 (Patient Intake Screening) • DA 4466 (Patient Progress Report) • DA 5018-R (Release of Treatment Information) • Explain to the treatment facility that you will need these forms for the National Guard. If you have questions, need additional information, or are unable to reach a treatment provider, contact the Department of Mental Health - Veterans Services Director during business hours at (573) 751-2368 or by email at [email protected]. Services are supported through funding from the Missouri Department of Mental Health, Division of Behavioral Health; Substance Abuse and Mental Health Services Administration, Center for Substance Abuse Treatment Block Grant. .
