POST OFFICE CUSTOMER

The Domestic Manual, section 508.4 requirements relative to PO Box customers and their responsibility to maintain their PS Form 1093, Application for Box Service. The section includes the following:

1. The applicant must complete all required items on PS Form 1093 2. When any information required on PS Form 1093 changes, the box customer must update the application on file at the Post Office.

To help eliminate fraud and possible identity theft, Postal regulations require that PS Form 1093, Application for Service, contain current information for each box holder. Additionally, Postal Regulations require that each Post Office Box Customer receiving mail in a Post Office Box be listed on the PS Form 1093, Application for Post Office Box Service and each person must have two forms of verifiable identification prior to mail commencing. Our regulations require that PS Form 1093, Application for Post Office Box Service, contain updated current information for each box holder. This notice is being provided to you because required information on your current PS Form 1093, Application for Post Office Box Service, is either outdated or missing. Please present this to any window clerk at the post office within 10 days of receipt. When you do so, you will be asked to:

1. Show two types of acceptable identification, one bearing your photograph. The identification presented must be current, contain sufficient information to confirm your identity, and must show your street . Acceptable identification includes: a. Valid driver’s license b. State non-driver’s license c. Armed forces or ID d. University or recognized corporation ID e. f. Alien registration card g. Certificate of naturalization h. Current lease or mortgage i. Deed of trust j. Voter or vehicle registration card k. Home or vehicle policy Note: Credit cards, social security cards and birth certificates are not acceptable forms of identification.

2. Update information such as the street address of where you currently reside

3. Update names of individuals receiving mail at your PO Box number. Note: Each person over the age of 18 that receives mail in the post office box must accompany you to the post office and provide two types of ID.

The information required on your PS Form 1093, Application for Post Office Box Service, is for your protection and the protection of others. Privacy Act Statement: Your information will be used to provide Post Office Box service. Collection is authorized by 39 U.S.C. 401, 403, and 404. Providing the information is voluntary, but if not provided, we will be unable to provide this service to you. We do not disclose your information to third parties without your consent, except to facilitate the transaction, to act on your behalf or request, or as legally required. This includes the following limited circumstances: to a congressional office on your behalf; to financial entities regarding financial transaction issues; to a U.S. Postal Service auditor; to entities, including law enforcement, as required by law or in legal proceedings; to contractors and other entities aiding us to fulfill the service (service providers); to process servers; to domestic government agencies if needed as part of their duties; and to a foreign government agency for violations and alleged violations of law. Information concerning an individual boxholder who has filed a protective court order with the will not be disclosed except pursuant to court order.
