Social determinants of health are social and economic factors that can influence a person’s health and wellbeing, such as housing, transport, connections with family and friends, education, and mental health.

On the following pages you will find an assessment tool which you can use to help you assess your current level of need. You can then refer to the applicable page number/s for services that will assist you to take steps to better health.
