This directory is for Colorado Service Area: Adams, Arapahoe, Boulder, Denver, Douglas, Jefferson, Larimer, and Weld county. To access Clear Spring Health’s online provider directory, you can visit www.clearspringhealthcare.com. We can also give you information in Braille, in large print, or other alternate formats at no cost if you need it. To get detailed information about your health care coverage, please see your Evidence of Coverage (EOC). Clear Spring Health is a PPO plan that allows the use of in and out of network providers to obtain medical care and services. Clear Spring Health does not require you to select a physician to be your Primary Care Provider (PCP). Clear Spring Health does encourage you to identify a physician to be your PCP as generally, you must get your health care services from your PCP Your PCP should be your first point of contact for undiagnosed health concerns or health problems and routine health care needs. In addition to providing you care, your PCP will help arrange or coordinate the covered services you get as a plan member. This includes your x-rays, laboratory tests, therapies, care from doctors who are specialists, hospital admissions, and follow-up care. Referrals are not required with Clear Spring Health PPO plans, but your PCP should be your primary resource to use when seeking specialist and hospital care and coordination of your medical services. The network providers listed in this directory have agreed to provide you with your health care services. You may go to any of our network providers listed in this directory. Out-of-network providers are under no obligation to treat Clear Spring Health enrollees, except in emergencies. For a decision about whether we will cover an out-of-network service, we encourage you or your provider to ask us for a pre-service organization determination before you receive the service. Please call our Member Service Department at 877-384-1241, we are open 8:00 am to 8:00 pm Monday – Friday from April 1 – September 30 and 8:00 am to 8:00 pm Monday – Sunday from October 1 – March 31. In cases where out-of-network providers submit a bill directly to you, you should not pay the bill but should submit it to Clear Spring Health for processing and determination of your liability, if any. Box 4048 Scranton, PA 18505 You may get covered emergency medical care whenever you need it from the nearest hospital or provider. You don’t need to get approval first from Clear Spring Health or your doctor. If you 3 have an emergency, Clear Spring Health will talk with the doctors who are giving you emergency care to help manage and follow up on your care. The doctors who are giving you emergency care will decide when your condition is stable and the medical emergency is over. After the emergency is over, you are entitled to follow-up care to be sure your condition continues to be stable. Your follow up care will be covered by our plan. Clear Spring Health pays for covered services when you visit a doctor or medical facility that is not included in the Clear Spring Health network. Between 1930 and 1940 the population increased by nearly 50% from 77400 in 1930 to 111800 in 1940. New cities formed as the population grew including the incorporation of Belmont, Menlo Park and Woodside in 1927. Increasing transportation needs were answered by the opening of Mills Field (San Francisco Airport) in 1927 and the building of the San Mateo Hayward Bridge in 1929. The Great Depression was one of the reasons for an increase in population. Whereas previously immigrants might be likely to settle in San Francisco neighborhoods, now the lack of jobs and high rents led them to relocate to the peninsula. With much of the county still unincorporated it was still possible to find a strip of land and put up an inexpensive dwelling.

SINGLETON, EDITOR BRUNO BIAIS and MICHAEL ROBERTS, CO-EDITORS I AM HAPPY TO report that 2012 was another good year for the Journal of Finance. We received 1,276 submissions, of which 1,147 were new manuscripts. The number of new submissions was virtually unchanged from 2011. There were 129 resubmissions, slightly below the 138 in 2011. Note that the Journal of Finance no longer counts “style” revisions in the official revision count, which explains the sharp drop in resubmissions after 2008. In 2012, the Journal published 60 articles, written by authors whose primary affiliations include 73 different institutions. Table I details the number and timing of submissions received throughout the year. The primary affiliations are summarized in Table II, which reports the number of authors per institution (where an article with n authors is counted as 1/n articles for each author’s institution). The institutions with the most Journal authors last year were the University of Chicago, Oxford University, and the University of Texas at Austin. The Journal’s visibility and impact remain extremely high. As shown in Ta- ble III, articles published in the Journal were cited 18,293 times in all journals during 2011, a total that ranks first among business and finance journals and third among all economics journals (behind the American Economic Review and Econometrica). Our one-year impact factor (cites during 2011 to articles published in 2009 and 2010, divided by the total number of articles published in those two years) is 4.22 (up from 3.76 in 2009), which ranks second among business and finance journals and fourth among all economics journals (up from seventh in 2009).

Sindone Introduzione e note di Gian Maria Zaccone Inizia la lettura Indice Condividi Colophon indice successivo → PRESENTAZIONE Renzo Savarino Due primati rendono grande la fama di Torino nel mondo: la Sindone e don Bosco, il personaggio italiano più noto, la cui fama oscura il già collaudato mito di Giuseppe Garibaldi. L’ostensione sindonica prevista per il 2015 in occasione del duecentesimo anniversario della nascita di don Bosco congiunge i due dati sopra ricordati, riapre gli entusiasmi e le discussioni nei confronti di questa misteriosa immagine. Nelle precedenti ostensioni l’afflusso dei pellegrini fu mosso dalle più diverse motivazioni che andavano dalla devozione della maggioranza convinta della certezza dell’autenticità dell’icona; alla curiosità suscitata dal senso di mistero che emana, agli interrogativi sulle origini tuttora inesplicate dell’immagine. Tra commozione sincera, entusiasmo manifesto, pregiudizi favorevoli o contrari, dubbi, negazioni e curiosità più o meno motivate, rimane il fatto del lenzuolo sindonico con l’opportunità e la necessità morale di capire che cosa realmente sia ed indichi. Una singolare ed esemplare figura di un prete torinese, don Adolfo Barberis, artista, architetto, apostolo, avviato verso la glorificazione degli altari, dichiarato venerabile da papa Francesco il 3 aprile 2014 a coronamento di un lungo processo di beatificazione, compose per l’ostensione del 1933 un opuscoletto, Come si guarda la SS Sindone, strutturato in tre parti: I Come guarderò la Sindone; II Storia della Sindone; III Autenticità della Sindone. Le sue figlie spirituali, le suore del Famulato Cristiano, hanno pensato di mettere a disposizione dei visitatori della Sindone questo libretto che, oltre ad essere un documento dell’intelligenza e del cuore dell’autore, è un prezioso aiuto per porsi in relazione con l’immagine in modo razionale e corretto dal punto di vista religioso.
