UTAH JAZZ ONLINE SURVEY SWEEPSTAKES RULES & REGULATIONS (1) No Purchase Necessary UTAH JAZZ ONLINE SURVEY SWEEPSTAKES RULES & REGULATIONS (1) No purchase necessary. (2) Completion of the Survey is not required for participation in the Sweepstakes. (3) Eligibility: Sweepstakes open to residents of the state of Utah. Must be 18 years of age or older as of December 2, 2014. Employees and immediate families (spouse, parents, siblings, and children) of the Utah Jazz, the National Basketball Association and its Member Teams, NBA Properties, Inc., NBA Media Ventures, LLC and each of their respective parents, subsidiaries, affiliates, directors, owners, employees and agents (collectively, the “NBA Entities”) are not eligible to enter. (4) How to enter: (a) completely fill out and submit the Utah Jazz online Survey, including Sweepstakes entry form at https://nba.turnkeysurveyor.com/se.ashx?s=705E3F0F22B6DF7C. Upon completion, you will be automatically entered in the Sweepstakes; or (b) fill out and submit the Utah Jazz Sweepstakes entry form at http://bit.ly/11PLwb4. Entries are limited to one entry per email address. (5) Contest Term: December 2 – 15, 2014. Entries must be received by December 15, 2014. (6) Winner Selection: Winner will be chosen from a computer generated random sample of all entrants and will be notified via telephone or email before January 15, 2015. In the event the potential winner does not respond to prize notification within 30 days of attempted notification or declines the prize, another winner will be randomly chosen by computer from the remaining eligible entrants. Chances of winning depend on the number of eligible entries received. (7) Prize: Two (2) Row A game tickets and two (2) VIP dinner tickets to a game during the 2014-15 Utah Jazz season, one (1) official National Basketball Association basketball autographed by players on the 2014-15 Utah Jazz team, six (6) Salt Lake Bees tickets to a 2015 game (along with the opportunity to throw out the first pitch at the game) will be awarded as the prize for this sweepstakes. The prize is non-transferable to any other person, including relatives or friends. Winner is solely responsible for all applicable federal, state and local taxes and any expenses associated with the game, unless otherwise indicated. No substitution, cash equivalent or transfer of prize permitted except at the Jazz’s option for a prize of comparable or greater value. Approximate retail value (“ARV”) of prize: $2,750.00. ARV reflects unautographed merchandise. ARV of hand- signed merchandise subject to fluctuation based on the featured NBA players and current market value at the time prize is awarded. (8) Winner’s List: The winner’s name can be obtained by writing to the Utah Jazz Online Survey, 301 W. South Temple Salt Lake City, UT 84101. (9) Miscellaneous: By entering, entrant agrees to follow the Official Rules and hold harmless and indemnify the Utah Jazz, and the NBA Entities against any and all liability, damages or causes of action (however named or described), with respect to or arising out of either (a) entrant’s participation in the Sweepstakes; or (b) the receipt or use of the prizes awarded in the Sweepstakes. (10) In consideration for being awarded the prize, winner hereby agrees and consents(where legal), without further notification, authorization, compensation or remuneration of any kind, to the use of winner’s name and/or likeness in any and all advertising, promotions and other publicity conducted by the Utah Jazz and each of NBA Entities.     Dear Coach, As we prepare for the season to begin, I would like to take this opportunity to welcome you to another year of Jr. Jazz Youth Basketball and thank you for your involvement. Speaking from experience, I know that coaching is a labor of love—it can involve hard work and sacrifice, but it’s an extremely rewarding experience. Without your dedication and effort, this program would not be the overwhelming success that it is today. The two biggest things that I have been stressing to my players are playing with the pass on offense and connecting our efforts defensively. I strongly believe that sharing the ball, along with strong communication and positioning, are the most crucial elements to any team’s success. I think you’ll find that emphasizing these areas can help improve your team’s overall performance. Beyond the basic skills, I would hope your kids leave the season with an understanding of good sportsmanship and a sense of accomplishment. Naturally, some kids will have more talent and ability than others. Nevertheless, it’s important that every child feels that he/she is a contributing member of the team—regardless of whether they win or lose. And above all, let’s have fun. We must never forget that basketball, after all, is a game, and that games are supposed to be fun. Remember that coaching Jr. Jazz is about teaching, and that your success is not based on your win-loss record, but in watching kids grow and develop while enjoying themselves. Thirty scholarships, representing the team’s first 30 wins, will be awarded beginning with the 2021-22 school year -- The scholarships will be awarded to students across six Utah universities — Brigham Young University, Southern Utah University, University of Utah, Utah Valley University, Utah State University, and Weber State University SALT LAKE CITY (March 11, 2021) — The Utah Jazz today announced the launch of the Utah Jazz Scholars Program to provide college scholarships to underrepresented students of color. The program, which provides one scholarship for every Jazz win this season, will cover up to the full cost of attendance — including tuition, books, fees, and room and board — for each recipient’s undergraduate education at one of six universities in the state of Utah. The first 30 awards, representing the team’s first 30 wins this season (including preseason), will be granted to students enrolling as freshmen for the 2021-2022 academic year. All remaining scholarships that result from additional wins this season will be awarded beginning with the 2022-2023 school year.
