SOUTHWEST KEY PROGRAMS OVERVIEW Southwest Key Programs (SWK) is a national nonprofit organization that exists to create opportunities and improve the quality of life for thousands of youth and families each day by providing safe shelter, alternatives to incarceration, career development, and quality education. This is done through four areas of programming: Immigrant Children’s Shelters, Youth and Family Services, and Workforce Solutions. 1. SCOPE OF WORK: OVERVIEW SWK is looking to move our N-95 masking fitting process onsite to all our residential shelter locations, this includes Arizona, California, and Texas. Moving this process onsite will help SWK facilitate a more efficient and streamlined process, which includes quicker turn-around times for OSHA questionnaire, fit testing, results, and reporting. The purpose of N-95 mask fitting is to provide the proper personal protective equipment (PPE) to any employee identified by leadership based on position duties where, the employee works in isolation, quarantine, or any other scenario where they may be potentially exposed to airborne and droplet particles, or a positive COVID-19 youth they must be fitted for an N-95 mask. SWK will also revisit employee PPE needs on a bi-annual basis, and testing renewal is annually. Requirements: The successful bidder should be able to provide the following services: • Complete OSHA Respirator Questionnaire (sections part A, Sections 1 & 2 (mandatory questions 1-9) – English/Spanish including medical opinion review for pass/fail prior to fit test – and written recommendation to employee – for reference OSHA Sec. 1910.134 • Coordinate the collection of Patient Authorization Form or any other Employer Protected Health HIPPA Release Form (s). • Perform Qualitative mask fit testing – utilizing N-95 masks provided by Southwest Key Programs (see mask models/sizes below, as well OSHA accepted test agents to verify smell and measure leakage • Perform Pulmonary Functionality Study (testing) • Provide timeline (estimated timing based on locations and fittings needed) Masks: SWK currently provides employees with three different mask sizes/models for proper mask fittings. The organization is also exploring two additional mask sizes/models to accommodate the employee. Bidder shall provide mask fittings for any mask approved by SWK. The current masks provided are: • McKesson item # 1181773 – Small Medical N-95 - Flat Fold Pouch Elastic Strap Small White Nonsterile ASTM Level 3 • McKesson item # 1164068 – Medium/Large Medical N-95 - White Non-Sterile ASTM Level 2 NIOSH Page 2 of 5 • McKesson item # 1181774 – One-Size Fits All N-95 - Flat Fold Pouch Elastic Strap Regular White Non-Sterile ASTM Level 2. SHIPPING TERMS: F.O.B. destination, freight prepaid and allowed (included in unit price). 3. PRODUCT ADDITIONS TO THE PO/CONTRACT: If the awarded Bidder can guarantee fixed pricing for a term beyond this PO award, SWK may request to enter into an annual contract with the awarded bidder for the same goods and services beyond the initial Purchase Order. 6. REQUIREMENTS/LIMITATIONS: Quantities may be increased or decreased based on need during the term of the PO. The quantities shown are estimates only and do not constitute a guarantee of purchase. 7. SWK POINT OF CONTACT – PROCUREMENT STAFF: All communications relating to this IFB must be directed to the SWK Procurement Department as named on this IFB. All communications between respondents and other SWK staff members concerning this IFB are strictly prohibited. Failure to comply with these requirements may result in bid disqualification. Page 4 of 5 8. SPECIFICATIONS: SWK reserves the right to make minor adjustments to the specifications. See Price Sheet for detailed items. It is highly encouraged, but not required, for Bidders to bid on all lines. SWK reserves the right to award to multiple vendors if it is in its best interest to do so. 9. PRICING: This is a fixed price per unit and price will remain the same during the term of the PO. Bidder must propose their price based on the specifications provided for each line item. Bidder shall indicate any additional fees required for their services. In addition to the services provided, Bidder shall detail, travel expense fees, reporting fees and cancellation fees. Bidder will indicate prices and fees on the Bid Pricing Sheet (Attachment B). 10. REQUIRED SUBMITTALS: Include the following information in the email subject line: IFB No., Bidder Name, Due Date, and Time. Do not copy the Buyer when submitting the bid. General Terms and Conditions The following terms and conditions shall govern this agreement, regardless of any contradictory provisions. 1. Services provided by Contractor; Compensation; Billing. Contractor hereby agrees to provide the services as stated in the Specific Terms and Conditions. In consideration for said services, Southwest Key hereby agrees to pay Contractor the rate outlined in the Specific Terms and Conditions of this agreement. Contractor shall complete and sign an invoice, mutually acceptable to both parties, which shall be delivered, within ten (10) days of completion of the work, to the address listed in the Specific Terms and Conditions. 2. Term and Termination. This agreement is effective as stated in the Specific Terms and Conditions. Additionally, this Agreement may be terminated immediately upon written notice by Southwest Key, with or without cause.
