Inferior alveolar nerve blocs, one of the most common procedures in dentistry requires deep anatomical knowledge of the pterygomandibular space [6, 7]. This space is a narrow gap, which contains mostly loose areolar tissue. It communi- cates however through the mandibular foramen with the mandibular canal, which is traversed by inferior alveolar nerve, artery and comitant vein (IANAV). Many of the structures placed in this space are of certain importance for local anesthesia. Khoury at al. [8, 9] mention here the following: inferior alveolar nerve, artery and vein, lingual nerve, nerve to mylohyoid and the sphenomandibular ligament. The space contains mostly loose areolar tissue.

Mitea, and X A.A. Postma ABSTRACT BACKGROUND AND PURPOSE: Necrotizing external otitis is a serious complication of external otitis with different spreading patterns. A persistent foramen of Huschke is a dehiscence located antero-inferior in the osseous external ear canal and posterior-medial to the temporomandibular joint. This dehiscence can facilitate extension of infection in an anterior pattern next to classic spread along the fissures of Santorini. The aim of this study was to define the prevalence and size of a persistent foramen of Huschke in patients with necrotizing external otitis. MATERIALS AND METHODS: We retrospectively examined 78 CT temporal bone studies (39 patients with necrotizing external otitis, 39 control subjects). The side and presence of the foramen were noted, and its prevalence was calculated. The maximal width of the foramen of Huschke was measured in the axial plane and classified as subtle, mild, moderate, or extensive. RESULTS: A persistent foramen of Huschke was present in 21 patients (26 ears) and 7 control subjects (9 ears). Prevalence was 50% (20/40) and 11.5% (9/78) in affected ears of patients with necrotizing external otitis and control subjects, respectively. Almost all affected ears showed an anterior distribution pattern of necrotizing external otitis. The extensive dehiscence was most common in affected ears. CONCLUSIONS: An anterior necrotizing external otitis spreading pattern is associated with the presence and increased size of a persis- tent foramen of Huschke.

These anatomi- cal variations seem to attribute to complex embryology of the OphA. In human embryos and fetuses, primitive dorsal and ventral ophthalmic arteries (PDOphA and PVOphA) form the ocular branches, and the supraorbital division of the stapedial artery forms the orbital branches of the OphA, and then numerous anastomoses between the internal carotid artery (ICA) and the external carotid artery (ECA) systems emerge in connection with the OphA. These developmental processes can produce anatomical variations of the OphA, and we should notice these variations for neurosurgical and neurointerventional procedures. C o n c l u s i o n s: Pterygomandibular space is a clinically important region which is commonly missing in anatomical textbooks. More attention should be paid to it both from theoretical and practical point of view, especially in teaching the students of first year of dental studies. Key words: pterygomandibular space, Gow-Gates anesthesia, inferior alveolar nerve. INTRODUCTION Numerous cranial regions have been subjects of various anatomical and anthro- pological studies [1, 2]. Most of them indicate necessity of undertaking of such studies first of all because of clinical importance [3–5]. Inferior alveolar nerve blocs, one of the most common procedures in dentistry requires deep anatomical knowledge of the pterygomandibular space [6, 7]. This space is a narrow gap, which contains mostly loose areolar tissue. It communi- cates however through the mandibular foramen with the mandibular canal, which is traversed by inferior alveolar nerve, artery and comitant vein (IANAV). Many of the structures placed in this space are of certain importance for local anesthesia. Khoury at al. [8, 9] mention here the following: inferior alveolar nerve, artery and vein, lingual nerve, nerve to mylohyoid and the sphenomandibular ligament.
