The medical case mix reflects the complex nature of the patients within Addenbrooke’s: significant numbers of admissions are from hepatology, haematology, nephrology and transplant medicine. Addenbrooke’s performs around 100 liver transplants annually and is the UK’s sole multivisceral transplant centre. Patients are admitted to the JVF ICU from all major specialties excluding cardiothoracics and neurosurgery. The Rapid Response Team (RRT) successfully provides emergency support for deteriorating patients throughout the hospital and provides the medical component of the cardiac arrest team. There are regular multidisciplinary clinical governance and audit meetings. The unit is recognised for training at all levels in ICM by the GMC, and participates in the ICNARC case mix programme. There is a weekly ICU follow up clinic and monthly audit programme. Effective and multidisciplinary team-working is integral to the JVF ICU. We have an establishment of more than 100 WTE Nurses, a Specialist Physiotherapist and dedicated Pharmacist. Microbiology and radiology rounds are held each day on the unit. ICU and RRT Consultants are on-site 08:00-22:00 seven days a week and available on-call 24 hours a day. We are dedicated to supporting our junior medical staff. You will support both 2 and 3 critical care patients alongside colleagues from Critical Care, Anaesthaisia and Acute Medicine. Consultant support will be available 24 hours a day. Teaching and Training The JVF-ICU unit has an active educational programme with weekly trainee and Consultant-led sessions. There is also the opportunity for training in echocardiography for Fellows who have undertaken FICE or equivalent courses. All Fellows are allocated clinical supervisors and offered similar training opportunities to trainee Doctors. Opportunities exist for career advancement through quality improvement projects, teaching, and training in Intensive Care Echocardiography and Thoracic Ultrasound. Cambridge University Hospitals NHS Foundation Trust The Department of Acute Medicine Ambulatory Care & Follow-up Clinics Acute Medicine runs an Ambulatory Care Unit (EAU3) which is located above the Emergency Department and focuses on admission avoidance. A Consultant is present between 09:00- 17:00, and a middle grade Doctor between 12:00-20:00. Pathways have been developed for the management of conditions such as suspected pulmonary embolism, cellulitis, syncope, atrial tachycardia, sub-acute diabetes, low risk upper gastro-intestinal bleeds, COPD, asthma, anaemia and ischaemic chest pain. The Ambulatory Care Unit takes referrals from the Emergency Department and directly from GPs via a GP Liaison Service. The Unit is supported by dedicated nursing staff and direct access to a comprehensive range of investigative and diagnostic modalities. Ambulatory clinics are run jointly by a Consultant and middle grade Doctor, thereby offering excellent learning opportunities. We also provide a Friday morning General Medical follow-up Clinic. Acute Care Hub (EAU4 and EAU5) Situated above the Emergency Department, the two short-stay wards constitute our Acute Care Hub. These provide care for short-stay patients and during the initial acute phase of an admission. Patients are referred from the Emergency Department and medical senior review is being carried out on these wards. Both wards have a dedicated Consultant Lead on a rotational basis. On EAU 4 the expected length of stay is <24 hours, on EAU5 <72 hours. Excellent support is provided by Allied Health Professionals, as well as various clinical specialities. Daily multidisciplinary team meetings facilitate patient care and discharge planning. Outliers A dedicated Consultant led team looks after outlying patients under Acute Medicine. These are largely based on wards K3 / CCU, which are shared with Cardiology. Emergency Department CUH does not operate a conventional Medical Assessment Unit. The Medical Take team is based in the Emergency Department where we work alongside Emergency Physicians. From there patients are discharged, referred to Ambulatory Care or urgent outpatient pathways, or admitted to relevant ward and specialty care. A significant proportion of the Medical Take is transferred to the Acute Care Hub which is run by Acute Medicine and Geriatrics. A Medical Consultant is usually present in the Emergency Department between 09:00-22:00. During 17:00-2200 two Medical Consultants are on site. This helps provide senior input and guidance. This well supported environment provides excellent learning opportunities for Doctors in the transition period between CMT and higher medical training. It also allows more senior clinical decision makers to discuss complex clinical cases. Duties of this post You will spend time rotating through the different clinical areas in Acute Medicine – Ambulatory Care, Acute Care Hub, Outliers and the Medicine follow-up clinic. You will also provide support to the expanded Covid-19 bed base, working as part of the Trust response as a regional surge centre for Covid-19. As a department we are lucky to have Consultants with a wide variety of specialities who would be happy to help facilitate a particular interest. Beyond this CUH is a tertiary referral hospital and successful candidates would have the opportunity to pursue areas of interests outside the Acute Medicine Department.
