Powys Stroke Resource Folder

Powys Stroke Resource Folder

Powys Stroke Resource Folder

A stroke changes everything. You may need help and support to live your life to the full. This folder is to support you after your stroke.

This folder belongs to you.

It has spaces where you can tell your story. The story about your stroke and what has happened. You can put as much or as little as you like in it.

It has spaces where you can record how you would like to move forward and things you would like to achieve.

You can do this by writing, drawing or pictures. You can tell your story to someone else. They can help you fill it in.

Doctors, therapist and carers may add things to your folder to help everyone to work together. Working together as a team will improve the support you get.

It also has information that may be useful once you go home, such as telephone numbers and local services.

You can use this folder as you wish. You can:

 use it to record the progress you make and your achievements over months and years  take it to appointments with you so you don’t have to repeat what has happened to you  write questions you want answers to
