Shah, in welcoming the participants from the APANPIRG member States, non-member States and the International Organizations, noted that APANPIRG serves as a body that is designated as the guiding and coordinating organ for all activities conducted within ICAO concerning air navigation system for the Asia/Pacific regions. 1.3.2 He acknowledged the participation of Tonga, represented by Mr Mapa Faletau, the Secretary for Civil Aviation who will be presenting his proposal for APANPIRG membership and thanked Tonga for seeing value in the Group. He also welcomed the participation of Mr. Jone Koroitama, Chief Executive, Airports Fiji Ltd, the founding chairman of APANPIRG who oversaw some very exciting developments in the region and who had served the body for a long period. With regard to International Organization, Mr. Shah welcomed the presence of Mr. Donald Spruston, former Director-General of Civil Aviation Canada and currently the Director-General of International Business Aviation Council (IBAC), who will make a presentation on IBAC’s application for regular attendance at APANPIRG. He also welcomed the presence of Mr. Vladimir Zubkov, Chief of Regional Affairs Office of ICAO Headquarters at the meeting. 1.3.3 Mr. Shah informed the meeting that the ASIA/PAC Basic ANP and FASID was approved by the President of the Council on 11 August 2001 and the immediate task ahead will be to fully utilise the documents for rapidly enhancing the implementation of all facilities and services in air navigation. 1.3.4 He highlighted some of the noteworthy developments in the region such as the successful execution of the IUSOAP and follow-up audits, cooperative development on operational safety and continuing air worthiness (COSCAP-SEA) which became operational on 09 July 2001 and the signing of an MOU between the President of the Council and the Honourable Minister of Communications and Information, Singapore in August 2001 on the development of a Country Training Programme providing 100 fellowships for developing countries. 1.3.5 Mr. Shah congratulated the Singapore Aviation Academy as the recipient of the prestigious Edward Warner Award for the year 2000 and looked forward to the dynamic role of the Academy in fulfilling much of the training needs in the Asia Pacific regions. 1.3.6 He also touched on some of the issues that lay ahead such as the 33rd General Assembly, expansion of IUSOAP to cover Annex 11 and 14, setting up of International Financial i-2 APANPIRG/12 History of the Meeting Facility for Aviation Safety (IFFAS), Global Air Transport Conference in March 2003, Global Air Navigation Conference in November 2003, importance of preparation for WRC-2003 and the 38th Conference of Directors-General of Civil Aviation in November 2001. 1.3.7 He underscored the important issue of establishing airspace system performance monitoring organization and funding arrangements for the implementation of reduced separation and CNS/ATM systems. 1.3.8 Mr Shah also took the opportunity to thank the Royal Thai Government, the host country of the Regional Office, for their generous support in the development of the new Conference Facility, which is expected to commence by end of 2001. 1.3.9 The Chairperson, Mr. H. S. Khola in welcoming participants informed that the work of the APANPIRG has been appreciated by ICAO ANC and Council. Its importance is also reflected from the fact that not only all the members of APANPIRG but eight other observer States, four International Organizations and the Chief of Regional Affairs Office, ICAO Headquarters are also participating in this meeting. 1.3.10 He also pointed out the need to have pro-active approach in the work so as to meet the future growth and safety requirements. Mr. Khola, however, expressed concern at the slow pace of implementation of SARPs and the Conclusions and Decisions of APANPIRG and also the resolution of the Shortcomings and Deficiencies identified by the ICAO Regional Office. He stressed the need to have focussed follow-up actions with target dates. While noting that the Regional Office is doing its best, the cooperation of States is needed to accelerate the pace of implementation. In order to achieve this goal, he exhorted that all possible forums such as the annual Conference of Directors-General of Civil Aviation, various workshops and seminars be utilised. 1.3.11 Mr. Khola thanked the ICAO Regional Office for the excellent arrangements made for the meeting. 1.4 Officers and Secretariat 1.4.1 Mr. H.S. Khola, DGCA, India as Chairperson of the Group presided over the meeting. Mr. Lalit B. Shah, ICAO Regional Director, Asia and Pacific Office, was the Secretary of the meeting assisted by Mr. K. W. Cheong, Regional Officer/AGA from the ICAO Asia and Pacific Office. Mr.V. Zubkov, Chief of Regional Affairs Office, ICAO Headquarters, was adviser to the meeting. 1.4.2 The meeting was also assisted by Mr. Shaukat A. Ali, Deputy Regional Director, Mr. H.V. Sudarshan, Regional Affairs Officer, ICAO Headquarters, Dr. E. Lysakov, Regional Officer/MET, Mr. J. E. Richardson, Mr. David Moores and Mr. Hiroshi Inoguchi, Regional Officers/ATM, Mr. K.P. Aviation Investigation Report A14Q0155 Runway excursion Air Canada Airbus A330-343, C-GFAF Montréal/Pierre Elliott Trudeau International Airport Montréal, Quebec 07 October 2014 Summary On 07 October 2014, the Air Canada Airbus 330-343 (registration C-GFAF, serial number 0277), operating as flight number ACA875, left the Frankfurt-Rhein/Main International Airport in Germany, bound for Montréal/Pierre Elliott Trudeau International Airport (CYUL), Quebec. During the daylight approach to Runway 24R at CYUL, with a thunderstorm just north of the airport, the crew was advised that the lighting on Runway 24R was out of service. This was the only runway in operation. During final approach, meteorological conditions changed from visual meteorological conditions to instrument meteorological conditions.

    National Transportation Safety Board Aviation Accident Final Report Location: Kwigillingok, AK Accident Number: ANC12LA002 Date & Time: 11/07/2011, 1830 AST Registration: N6314H Aircraft: CESSNA 207A Aircraft Damage: Substantial Defining Event: Loss of control in flight Injuries: 3 Minor, 3 None Flight Conducted Under: Part 135: Air Taxi & Commuter - Scheduled Analysis The pilot departed on a scheduled commuter flight at night from an unlit, rough and uneven snow-covered runway with five passengers and baggage. During the takeoff roll, the airplane bounced twice and became airborne, but it failed to climb. As the airplane neared the departure end of the runway, it began to veer to the left, and the pilot applied full right aileron, but the airplane continued to the left as it passed over the runway threshold. The airplane subsequently settled into an area of snow and tundra-covered terrain about 100 yards south of the runway threshold and nosed over. Official sunset on the day of the accident was 48 minutes before the accident, and the end of civil twilight was one minute before the accident. The Federal Aviation Administration's (FAA) Airport/Facility Directory, Alaska Supplement listing for the airport, includes the following notation: "Airport Remarks - Unattended. Night operations prohibited, except rotary wing aircraft. Runway condition not monitored, recommend visual inspection prior to using. Safety areas eroded and soft. Windsock unreliable." A postaccident examination of the airplane and engine revealed no mechanical anomalies that would have precluded normal operation. Given the lack of mechanical deficiencies with the airplane's engine or flight controls, it is likely the pilot failed to maintain control during the takeoff roll and initial climb after takeoff.

    Runway Safety Checklist For: • Air Traffic Controllers (or Air Navigation Service Provider) • Airport Operations Staff (or Airport) • Pilots (or Airline/Flying Organisation) Aim Runway Safety considers three main areas: • Runway incursions • Runway excursions • Runway confusion There are many organisations who are directly and indirectly involved in the maintenance of runway safety across the globe. In addition to specific actions on individuals and organisations, runway safety relies on all parties to work together: • at specific geographical locations (eg. airports); and • within/across organisations o at the tactical level (eg. Pilot, Air Traffic Controller, Airport Operations); and o at the strategic level (eg. Airlines, ANSP, Airport). The aim of the Runway Safety Checklist is to ask the user a range of questions which will: • Allow them to assess their level of runway safety, • Ask how they can improve runway safety, and • Increase their knowledge on factors which are important to runway safety. The checklist is written for consideration at the tactical level, but can be interpreted to allow runway safety to be considered from a strategic or organisational perspective. The checklist is divided into sections for Air Traffic Control, Airport and Aircraft Operator, to enable the user to answer specific questions relating to how their particular functions contribute to runway safety. However, users can also review the other sections to gain a better understanding of other functions and/or to informally consider whether any opportunities for improvement exist for the other areas. The checklist was intentionally designed to cover a range of aviation operations (eg. International airline operations at major airports, through to General Aviation operations at non-towered airports).

General aviation (GA) flights account for 92% of all the aviation accidents. In spite of all the official and unofficial sources of weather visualization tools available to pilots, there is an urgent need for visual- izing several weather related data tailored for general aviation pilots. Our system, Aviation Weather Data Visualization Environment (AWE), presents graphical displays of meteoro- logical observations, terminal area forecasts, and winds aloft forecasts onto a cartographic grid specific to the pilot's area of interest. Decisions regarding the graphical display and de- sign are made based on careful consideration of user needs. Integral visual display of these elements of weather reports is designed for the use of GA pilots as a weather briefing and route selection tool. AWE provides linking of the weather information to the flight's path and schedule. The pilot can interact with the system to obtain aviation-specific weather for the entire area or for his specific route to explore what-if scenarios and make "go/no-go" decisions. The system, as evaluated by some pilots at NASA Ames Research Center, was found to be useful.
