MINIMUM QUALIFICATIONS OF PROPOSER In order to be considered, a proposer must, as of the proposal return date stated in this RFP and throughout the duration of the program, meet the following applicable minimum qualifications. Proposer must provide documentation of existing qualifications in Section VII of the proposal. (See Section 8.1 of this RFP for complete instructions for submitting your proposal) Third Party Health Plan Administrator (TPA) · Authorized by the State of Florida Department of Financial Services Department to provide the services requested in the RFP. · Operating as a TPA for a minimum of five (5) years in Florida at the time of the proposal due date. Exclusive Provider Organizations/Network · Licensed and authorized by the State of Florida Department of Financial Services to provide the services requested in the RFP. · Comply with any requirements imposed by the Florida Department of Financial Services with respect to quality assurance accreditation. All HMOs must have achieved accreditation by the submission date of the proposal. · Demonstrate financial ability to provide the services requested in the RFP. Insurance Companies – PPO or POS Plans/Network, Parent Holding Companies · Licensed by the State of Florida Department of Financial Services to provide the services requested in the RFP; and · Hold an A.M. Best rating of “A" or better and a financial size category of IV or higher; OR · Hold an A.M. Best financial performance rating of “6" or better for those insurers with a letter rating of NA-2 or NA-3 and a financial size category of IV or higher; OR · Hold a Moody's Investor Service insurance financial strength rating of Aa3 or better; OR · Hold a Weiss A rating While not a minimum requirement, the City will award evaluation points to companies that provide documentation as part of the RFP and have received favorable SAS 70 or SSAE 16 audit reports.     Biographical Dictionary of Kansas Artists (active before 1945) Compiled by Susan V. Craig, Art & Architecture Librarian Univ. of Kansas August 2006 1 This book began with a 1981 reference question about John Noble, a name I did not recognize despite having studied art history and worked as an art librarian for more than 10 years. Learning that John Noble was a Kansas artist, I went looking for the best available book on Kansas art only to learn the resources were few. As a new faculty member at the Univ. of Kansas, I needed to establish a research project so I decided to prepare a dictionary of Kansas artists thus fulfilling both the research requirement and educating myself about the history of the visual arts in my native state; I just didn't intend the project to take 25 years or realize that I would have more than 1750 entries in the dictionary. I began by defining the scope of the work: • "Kansas artist" was loosely defined as artists who were both born in the state as well as artists who were born elsewhere but were artistically active in Kansas. Under this latter definition, I included artists who produced significant artworks such as the murals installed in Kansas post offices.     ABSTRACT Title of Document: BUTCH MORRIS AND THE ART OF CONDUCTION® Thomas Stanley, Ph.D., 2009 Directed By: Lawrence Witzleben, PhD Director Ethnomusicology Division Lawrence D. “Butch” Morris is a 62-year-old composer and bandleader who was part of a cadre of North American jazz innovators whose work began coming to public attention in the mid-1970s. Since 1985 he has developed, refined, and implemented a method for creating unique ensemble music using a patented vocabulary of conducting gestures. This novel strategy and the music it produces present an array of theoretical questions. Some of these have been simplified as questions of classificatory nomenclature: Is Conduction improvisation, interpretation, composition, or none of the above? How does Conduction as a system compare to other methods of structuring musical performance in real time? Other critical and social questions are addressed whose answers hinge upon the values and functions that sustain Conduction in the real world of monetized and competitive musicianship. Through interviews with Morris and members of his ensembles as well as observations conducted at numerous Conduction rehearsals and performances, my study documents Morris’ art form as a new instrumentality that offers new ways of making and thinking about music. In the course of this study, a variety of materials and sources are used to describe how Conduction® was developed, what its historical precedents are, and how it operates in real performance situations. The explanatory implications of framing Conduction practice as a novel musical instrument are also examined. This new instrument has garnered a community of users with differential investments in and expectations for Morris’ vehicle and how these investments and expectations have defined Conduction’s place in the domain of musical performance and education.
