Right to Health Through Education: Mental Health and Human Rights* Riikka Elina Rantala, Natalie Drew, Soumitra Pathare, and Michelle Funk he World Health Organization (who) is a specialized United Nations agency responsible for providing leadership on global health matters. At the time of its creation in 1948, health was de- Tfined as being “a state of complete physical, mental, and social well-being and not merely the absence of disease or infirmity.” However, in reality the main focus of global health work has predominantly been on physical health with much less focus given on mental and social health. Yet there is com- pelling evidence that mental health problems are one of the leading causes of sickness, disability, and even premature mortality in certain age groups.1 Indeed hundreds of millions of people worldwide are affected by mental, be- havioral, neurological and substance use disorders. For example, estimates made by who in 2002 showed that one hundred fifty-four million people globally suffer from depression and twenty-five million people from schizo- phrenia; 91 million people are affected by alcohol use disorders and fifteen million by drug use disorders.2 Another who report reveals that fifty million people suffer from epilepsy and twenty-four million from Alzheimer disease and other dementias.3 About 877,000 people die by commiting suicide every year.4 In addition it is well known that the human rights of the poorest and most marginalized people in society are most often violated or neglected, and people with mental disabilities are certainly no exception. Dear Friend of Freedom from Hunger, As a Public Health Researcher
Dear Friend of Freedom from Hunger, As a public health researcher and program planner, I have spent much of my career focused on efforts to reduce maternal and child malnutrition. I have been fortunate to work for many excellent organizations from international agencies to small nonprofits. That is how I could recognize what is so potentially effective about the Freedom from Hunger approach. But let me back up. After almost three decades of international work I was discouraged by the magnitude of the problem of child malnutrition. Many approaches to ending hunger were narrowly focused and overly politicized. When I was first introduced to Freedom from Hunger, it was refreshing and exciting to see that they focus on increasing women’s financial access as well as their health and nutrition knowledge. From my work in the field, I know that women play a key role in eliminating malnutrition. Extremely poor, rural women are an untapped resource, but they are as hardworking as any others, with both determination and dreams for their children. On a trip to Burkina Faso with Freedom from Hunger, I met a woman whose small food stand sat under a tarp she had set up. She was busy selling rice and sauces – what I call West African fast food – to passersby. She told me that before she joined her women’s association she could only buy small, more expensive bags of rice. Worse still, sometimes the rice ran out before the day was over. This meant lost business and less money for her family. Thanks to Freedom from Hunger, she could now purchase 50-kilo bags of rice, making each serving of rice less expensive. -

Freedom from Hunger-Integrating Health and Microfinance, UNC Gillings School of Global Public Health, University of North Carolina Role: Research Intern to be based in Chapel Hill, North Carolina and Davis, CA Project Lead: Freedom from Hunger Research and Evaluation Specialist Project Stipend: $2,500 from UNC Gillings School of Global Public Health. Student is responsible for his/her own transportation to/from Davis, California and living expenses in Davis. Project Period: Approximately 10-12 weeks between May and August 2013, exact dates negotiable. At least two weeks at Freedom from Hunger International Center, Davis, California. Background: Freedom from Hunger is a forerunner in integrating health protection services and products onto the platform of microfinance in order to reach the rural poor with a sustainable, affordable, reliable and effective mechanism to provide services that will help protect them against financial and health shocks. In 2005–2010, under the first Bill & Melinda Gates Foundation microfinance funding, Freedom from Hunger partnered with 5 MFIs in 5 countries to develop and sustainably offer a package of health protection services that included combinations of health education, health savings, health loans, linkages to health providers and products, and health micro-insurance to improve the health and financial stability of their clients while also contributing to the sustainability of the institutions themselves. Four focus groups with 17 individuals with food allergies were conducted to learn about their dining experiences. All sessions were audio-recorded, transcribed verbatim, and organized for extracting key concepts. Eight participants experienced allergic reactions after dining out at the restaurants, and many had unpleasant experiences when dining out. Participants perceived cross- contact, hidden ingredients, and miscommunication as potential causes of food allergic reactions. Participants identified lack of training, awareness, and knowledge about food allergy, and other operational restrictions such as lack of resources, as barriers to providing allergen-free food in restaurants. Buffet, ethnic, and specialty restaurants were seen as high-risk dining places due to potential risks of cross-contacts and hidden allergens in sauces. The participants took various precautions such as asking for clarifications of ingredients and seeking restaurants that are familiar to them. Consumers with food allergies experienced many difficulties in restaurants due to restaurant employees’ lack of knowledge and training regarding food allergy. -

Prior to the onset of COVID-19, Feeding America estimated that nearly 843,000 Virginians were food insecure, including 234,000 children. Current Feeding America projections indicate that an additional 447,000 Virginians will experience food insecurity at some point during the next year because of COVID-19. The Commonwealth has made dramatic progress against hunger in recent years but we know that we can and must do more to fix this solvable problem. -

Questions and Answers 1) What is the human right to food? The right to food is first of all a human right. It is the right to feed oneself in dignity. It is the right to have con- tinuous access to the resources that will enable you to produce, earn or purchase enough food to not only prevent hunger, but also to ensure health and well-being. The right to food only rarely means that a person has the right to free handouts. 2) Is the right to food a right to food aid? The right to food is primarily the right to feed oneself and one’s family in dignity, but of course there can be situa- tions when people cannot do that. There are emergency situations – wars, natural disasters – and there are persons – disabled, sick or orphaned – who cannot feed themselves and need assistance. People in those circumstances must receive assistance, whether food aid, cash or any other form. 3) Is the right to food a legally binding right? For the 160 countries that have ratified the International Covenant on Economic, Social, and Cultural Rights, the right to food is a legally binding right, on equal footing as the human rights prohibiting torture and protect- ing free speech and the press. In addition, many countries have included the right to food in constitutions and legislation. Countries that have not ratified the Covenant should at least recognize their moral responsibility to realize this right. 4) Who is responsible for implementing the right to food? States are the primary duty bearers for implementing human rights, including the right to food. -
