The Old Age Liaison Psychiatry Team provides clinical and educational support to both and and is seen nationally as an exemplar in service delivery. The team benefits from close working relationships with the 7 General Practices aligned Older Adult Community Mental Health Teams in Aberdeen and and senior colleagues in the Department of Geriatric .

The appointees are likely to be involved in undergraduate and post graduate teaching and will be registered with the continuing professional development programme of the Royal College of Psychiatrists. They will also contribute to audit, appraisal, governance and participate in annual job planning.

There are excellent opportunities for research.

Applicants must have full GMC registration, a licence to practise and be eligible for inclusion in the GMC Specialist Register. Consultant Psychiatrist Old Age Psychiatry (sub-specialty: Liaison Psychiatry)

Old Age Psychiatry in Grampian is delivered by 9 multidisciplinary teams:  Aberdeen City North  Aberdeen City Central  Aberdeen City South  Aberdeenshire Central  Aberdeenshire South   Banff and Buchan  East  Moray West

Old Age Psychiatry Services in Aberdeen and Aberdeenshire are delivered by three Aberdeen City and four Aberdeenshire multidisciplinary Community Mental Health Teams based at Royal Cornhill Hospital, Aberdeen. These teams are led by five Consultants in the City covering a population of 36, 000 people over 65 and six Consultants in the Shire who have a population of 43,000 people over 65. Most inpatient facilities are centred at Royal Cornhill Hospital with the Shire teams spending some time at outlying hospital and clinics. Two multidisciplinary Community Mental Health Teams are based in Dr Gray’s Hospital, Elgin and serve Moray West and Moray East.

Applications are invited from holders of the Certificate of Completion of Training (CCT) or the Certificate of Eligibility for Specialist Registration (Combined Programme)(CESR(CP) in Old Age Psychiatry (or those within six months of obtaining CCT/CESR(CP)) and doctors who are on the GMC’s specialist register or are eligible for such inclusion.

Principle Duties  The Consultant will be responsible for providing a psychiatric consultation and liaison service for patients aged over 65 years admitted to certain Geriatric Medical wards in Aberdeen Royal Infirmary and also rehab wards at Woodend Hospital. The Consultant will also be responsible for a small catchment population of patients over 65yrs (2229) in Aberdeenshire with access to functional beds at Royal Cornhill Hospital and dementia beds at Hospital.  The consultant will be expected to build close links with primary teams, community care services, geriatric services, general adult psychiatry and with private and voluntary agencies in the locality.  The consultant will provide excellent clinical leadership to the multidisciplinary liaison team and also to the sector team which is shared with a fellow Consultant.

 The consultant will be expected to contribute to undergraduate and postgraduate teaching and participate in supervision of junior medical staff.  There is the opportunity for research in association with Aberdeen University and for candidates to become involved in academic commitments. The postholder will be expected to participate in teaching on the MRCPsych course.  Patients under the age of 65 with organic brain disorder may become the responsibility of the Old Age Psychiatry clinical teams by mutual agreement with other clinical services. Transfer of older patients to the service is arranged on a case by case basis in negotiation with General Adult services.  The appointee will be expected to participate in the out-of-hours Consultant on-call rota. This will involve being on-call overnight or on weekend days no more than one in twenty four, reduced pro rata in accordance with sessional commitment, if applicable. On weekend days the on-call Consultant attends a hand over meeting at 09.00am and shares with the on-call Senior Trainee assessment of patients admitted in the previous twenty-four hours.  Reciprocal cover for annual leave, study leave and professional leave is by arrangement with old age psychiatry consultant colleagues.

The Population And Services  The Old Age Psychiatry service enjoys close working relationships with the Dept of Medicine for the Elderly based at Aberdeen Royal Infirmary and Woodend Hospital. There is ready access to the full range of diagnostic facilities, including CT, MRI and SPECT brain imaging. Arrangements are in hand for joint training experiences for Specialist Registrars in the Department of Medicine for the Elderly.  Old Age Psychiatry services are delivered by Aberdeen City and Aberdeenshire community mental health teams based at Royal Cornhill Hospital, Aberdeen. These teams are led by four WTE consultants in the city covering a population of 36,000 people over 65 and six WTE consultants in the shire who have a population of 43,000 people over 65. Most inpatient facilities are centred at Royal Cornhill Hospital with a small number of the shire teams spending some time at outlying hospital settings.  Consultation and Liaison psychiatry services for patients aged 16-65 years at Aberdeen Royal Infirmary are provided by Dr Rainer Goldbeck and his team with out-patient facilities.  The appointee will take part in the psychiatric consultant on-call rota (currently 1:24) providing supervision for specialist registrars who in turn supervise senior house officers and foundation grade doctors.  The primary role for the successful applicant would be to develop and maintain a consultation and liaison service for patients over 65 admitted to medical wards at Aberdeen Royal Infirmary and Woodend Hospital. Out- patient clinics may be held at any of the two hospital sites at the postholder's discretion. There are no designated in-patient facilities for this service as there is expected to be an element of shared care in the general . Patients requiring psychiatric admission will be transferred to the appropriate catchment team. Therefore the successful applicant will be working closely with old age psychiatry colleagues throughout the old age service.

 The successful candidate will be encouraged to participate fully in future developments.

Research, Audit and Teaching  There are currently a number of Senior Trainees 4-6 in Old Age Psychiatry based in Royal Cornhill Hospital who are each assigned to one of the multidisciplinary teams on a rotational basis.  The appointee will have the opportunity to conduct research in an area relevant to old age psychiatry and to promote research in psychiatry amongst psychiatric trainees. Research facilities are available within the University Department of Medical Sciences and the Health Services Research Unit.  Clinical research is encouraged by NHS Grampian and is supported by the Clinical Research Centre and an on-site library. The appointee will have access to the NHS e-Library. The appointee would have the opportunity to conduct research in an area relevant to Old Age Psychiatry and to promote research in psychiatry amongst psychiatric trainees. The hospital is a short distance from Aberdeen University Medical School on the site which has a full medical library and other research facilities.  Time for audit and other clinical governance activities may be allocated within the consultant job plan and resources made available from the staffing establishment and supplies.  Undergraduate and postgraduate teaching is an integral part of the service. Specialist Psychiatric Training at Royal Cornhill Hospital is fully approved by PMETB. The postholder will be expected to apply for approval as a higher specialist trainer in Old Age Psychiatry.  There are designated seminar rooms with full audio-visual and video teleconferencing facilities in Royal Cornhill Hospital.

Study, Training and Appraisal There is an active programme of continuing professional development. The appointee will maintain his/her standards of knowledge and clinical practice and register with the Continuing Professional Development Programme of the Royal College of Psychiatrists for which funding will be provided. He/she will prepare a Personal Development Plan in consultation with colleagues and as part of the consultant appraisal process.

Support Staff And Facilities Appropriate medical secretarial support will be available at Royal Cornhill Hospital. Clinical office space will be available. All Consultants have access to a computer with e-mail facility and Internet access. Access to day hospital places, in-patient beds and community mental health teams for the elderly will be through current sector arrangements. Placement of training doctors is agreed by the Psychiatric Postgraduate Education Committee and is based on training needs.  The post holder will also be supported by a team of Liaison nurses: 1 WTE Band 7 and 1.5 WTE Band 6.  Sector support is shared with another Consultant in Old Age Psychiatry and comprises an enthusiastic and cohesive Community Mental Health team with 1 WTE Band 7 CPN, 2.5 WTE Band 5 CPNs, 3 support workers, 0.5 WTE Band 6 OT an 1 WTE Band 5 OT. The successful applicant will also share input from the Outreach Team created to follow up patients with a diagnosis of

Dementia or Cognitive Impairment consisting 1 WTE Band 6 CPN and 1.5 WTE Band 5 CPNs with 2 support workers.

Planning Planning for Old Age psychiatry services is led by the Clinical Director of Old Age Psychiatry and the Service Manager working alongside the Consultant teams.

Service, Clinical and Professional Structures  The Consultant will provide leadership and direction to the multidisciplinary team and supervision to medical staff in training.  Consultant Old Age Psychiatrists are directly responsible to the Clinical Director of Old Age Psychiatry with the Medical Director for Mental Health and LD Services holding overall responsibility for MHS medical staff and reporting to the Medical Director of NHS Grampian.  Monthly meetings of the consultants and Service Manager give opportunities for planning for OAP services. Plans and development proposals are brought to NHSG Board level by the Clinical Director of Old Age Psychiatry. Consultant to have ready access to the Service Manager and other member of the Directorate Management Team.  The post holder will become a member of the Psychiatric Medical Advisory Committee (PMAC). The Chairman of PMAC is a full member of the Mental Health Services Clinical Management Board.

Job Plan and Working Arrangements It is expected that the 7.5 sessions of clinical work will include fixed commitments, such as liaison with elderly medicine wards, an out-patient clinic and participation in the Psychiatric consultant rota. Clinical office space is available at Woodend General Hospital & ARI. Access to day hospital places, in-patient beds and community mental health teams for the elderly will be through current sector arrangements. The job plan will be reviewed every year with the Trust’s Medical Director.

Employing Organisation and Working Relationships Job planning reviews are undertaken on an annual basis between the Consultant and the Clinical Director of Old Age Psychiatry. Appraisal will be carried out annually by a consultant colleague who will be allocated by agreement with the appointee.

General Information about NHS Grampian Mental Health Services The Mental Health Services Clinical Management Board of NHS Grampian is responsible for the provision of psychiatric services to Aberdeen City, Aberdeenshire, Moray and to Orkney. In-patient psychiatric care is provided for the Shetland population and an innovative out reach service to Shetland involving telepsychiatry supported by one consultant psychiatrist is in place. NHS Grampian embraces the acute services, mental health, primary care and other services across Grampian. The population covered by the Mental Health Services Clinical Management Board is 528,400, of whom approximately 40% live in Aberdeen, 42% in Aberdeenshire and 18% in Moray. A full redevelopment of ROYAL CORNHILL HOSPITAL, which is located close to the centre of Aberdeen was completed in 1994. The new hospital includes acute and specialty wards, functional and organic Day Hospitals, central treatment complex (Physiotherapy, Occupational Therapy and ECT suite) and forensic and intensive psychiatric care unit. A 20-bedded acute psychiatric ward is located in Dr. Gray’s Hospital, Elgin.

Academic Psychiatry at the The former Department of Mental Health, in common with all clinical departments in Aberdeen, now sits within a medical school division. The University team is based in the Clinical Research Centre on the Cornhill site, and comprises clinical researchers, lecturers and scientists working on a range of projects alongside NHS staff with dedicated research and teaching sessions. Current themes include the study of mood disorders; neuroimaging; the genetics of schizophrenia and autism; the philosophy of psychiatry; neuro-economics, eating disorders, the evaluation of teaching methods and the assessment and promotion of healthy life-styles in mental health care.

The appointee will be eligible to apply for Honorary Clinical Senior Lecturer status, and participation in research and teaching is strongly encouraged and actively supported: facilities are available develop staff at every level – from novice to expert in both teaching and research domains. Description of Hospitals and Related Facilities Royal Cornhill Hospital (Aberdeen) has a bed complement of 416 and provides a comprehensive range of general and specialist psychiatric services. A new Nursery for the children of members of staff is located on this site.

Community Rehabilitation Unit (Polmuir Road, Aberdeen) has five two-bedroom flats for patients undergoing rehabilitation.

Great Western Lodge (Aberdeen) is a “halfway” residential facility located near the centre of Aberdeen. It provides for eight forensic in-patients who are preparing for discharge.

Royal Aberdeen Children's Hospital (located on the site of Aberdeen Royal Infirmary at Foresterhill) houses the department for Child and Family Psychiatry with day- patient and out-patient facilities.

Dr. Gray’s Hospital (Elgin, approximately 65 miles from Aberdeen) has a 25 bedded ward serving Moray.

Elmwood Learning Disability services are provided from this facility developed close to the Royal Cornhill site.

Terms and Condtions of service The appointment will be made by the Board on the recommendation of an Advisory Appointments Committee, constituted in terms of the National Health Service (Appointment of Consultants) (Scotland) Regulations, 1993 - NHS Circular 1993 No 994 (S.140) which will include University representatives. Any person suitably qualified and experienced who is unable for personal reasons to work full-time, will be eligible to be considered for the post.

Salary The whole-time salary, exclusive of any distinction award, will be a starting salary of £80653 - £107170 progression of salary is related to experience.

Appendix 8 of the contract sets out the code of conduct for private practice which applies to all interested parties. In general consultants will be free to undertake private practice as long as this is undertaken outwith the agreed job plan and employers are informed, in writing, of private commitments. While employers have discretion to allow some private practice to be undertaken alongside a consultant’s NHS duties, such provisions of private service should not prejudice the interests of NHS patients or disrupt NHS services.

Job plan Job plans must be agreed in association with the appropriate General Manager and Clinical Managers; for signature on behalf of the Chief Operating Officer. Changes will be discussed and agreed by these officers and yourself in line with Clinical Grouping service needs and changes in service requirements as well as at annual review.

Training The person appointed will be expected to take part in undergraduate and postgraduate teaching programmes. You will therefore be awarded appropriate Aberdeen University Honorary Status.

Research and Development Consultants are encouraged to undertake research and development in their own field and to link with the University research areas.

Psychiatry adivsor The person appointed will act as an adviser to the Board in General Adult Psychiatry.

Domiciliary consultations The person appointed will be expected to undertake domiciliary consultations as may be required by the Board.

Advisory visits The person appointed will be expected to undertake advisory ("pastoral") visits to hospitals in the Area – usually Cottage and local hospitals run by the GPs.

Working outside on NHS Grmapian You may exceptionally be required to undertake duties at other hospitals in the Grampian Area or other Health Board areas and at hospitals in Orkney, Shetland and elsewhere for which service agreements would be arranged.

Accommodated patients The person appointed may undertake the diagnosis and treatment of patients occupying accommodation made available under Sections 57(1), 57(2) and 58 of the National Health Service (Scotland) Act 1978 at the above hospitals insofar as the patients have not made private arrangements for such treatment.

Study Leave The Board, in partnership with the BMA Local Negotiating Committee has a study leave policy for all Career Grade Medical and Dental staff Policy available on request from the Human Resources Department.

Superannuation The appointment will be superannuable if the person appointed so chooses. He or she will be subject to the regulations of the National Health Service Superannuation Scheme and the remuneration will be subject to deduction of contributions accordingly, unless he or she opts out of the Scheme.

Residence The private residence of the person appointed should not normally be more than 10 miles by road from their principal place of work unless otherwise agreed locally. They must be contactable by phone.

Negligence NHS Grampian is legally liable for the negligent acts or omissions of employees in the course of their NHS employment. Medical staff are however advised to ensure that they have defence cover for activities not covered by the Board’s indemnity.

GMC Registration The officer appointed will be required to be registered on the General Medical Council’s Specialist Register.

Hepatitis B As a result of guidance issued by the Scottish Office on "Protecting Health Care Workers and Patients from Hepatitis B" NHS Grampian is required to:-  Ensure health care workers who may be at risk of acquiring hepatitis B from a patient are protected by immunisation.  Protect patients against the risk of acquiring hepatitis B from an infected health care worker. Due to the nature of this post, any offer of appointment will be conditional upon the successful applicant either:-  Undergoing a process of screening/immunisation/monitoring in accordance with the Board's Policy and Procedure, or

 Producing acceptable documentary evidence that he/she is not an infective risk to others.

In the event that he/she is an infective risk to others or if he/she fails to comply with the above requirements, the conditional offer of appointment will be withdrawn.

As a condition of his/her subsequent employment in this post he/she is also required to undergo further immunisation and monitoring at the intervals specified by the Board's Occupational Health Service in order to boost/maintain his/her level of immunity. Should he/she become hepatitis B e antigen positive and therefore an infective risk to others at any stage in the future the appointment will be subject to review in accordance with the Board's agreed Procedure for dealing with such situations where the postholder is involved in "Exposure Prone Procedures". This review may result in the postholder having to alter his/her clinical exposure to remove risk to patients and others. In circumstances where this is not a practical option, it will be necessary to provide industrial compensation for this prescribed industrial disease prior to the postholder leaving the Board's employment.

Fitness for employment The appointment is made subject to satisfactory fitness for employment. The candidate will therefore be required to complete a pre-employment health screening questionnaire and may/will subsequently be required to attend for health screening.

Notice period Termination of the appointment is subject to three months' notice on either side.

Disclousre Scotland The Board is required to instigate a check to be made with the Disclosure Scotland Office for any convictions recorded before an offer of appointment can be made (rehabilitation of Offenders Act 1974 amended 1985 and 1986) and (Disclosure of Criminal Convictions of NHS Staff with Substantial Access to Children 1989).
