The CPHL MLTC network is comprised of hundreds of qualified professionals and long term care services throughout downstate and upstate counties of New York State, which are: Bronx, Kings, Erie, Nassau, New York, Niagara, Richmond, Rockland, Queens, Suffolk and Westchester All of our providers have met strict licensure requirements before they became a provider in the Medicaid Managed Long-Term Care Plan of Centers Plan for Healthy Living LLC (CPHL). This directory contains a listing of the network community providers for Centers Plan For Healthy Living’s (CPHL’s) Medicaid Managed Long-Term Care Plan members. Our provider network is comprised of hundreds of highly-qualified healthcare professionals and long-term care services agencies in the following counties of New York State: Bronx, Kings, Erie, Nassau, New York, Niagara, Richmond, Rockland, Queens, Suffolk and Westchester. How Do I Find a Provider in the Directory? The providers in our directory have been organized by county and specialty. A few services that may be delivered in your home, such as home-delivered meals or personal care services will be listed according to appropriate specialty. Additional roadways include Coney Island Avenue, Metropolitan Avenue, Utica Avenue, Flatbush Avenue & Flatbush Avenue Extension, Roosevelt Avenue, Victory Boulevard, 7th Avenue, Amsterdam Avenue, Bowery, Houston Street, Park Avenue, Avenue of the Americas, 3rd Avenue. New York City Department of Transportation (DOT) Commissioner Polly Trottenberg today announced fourteen additional Arterial Slow Zones to be implemented in the coming months. Beginning Monday, August 4, the first of this second round of Slow Zones will be implemented at Jerome Avenue in the Bronx, between East 161 Street to Bainbridge Avenue, a five-mile stretch. The speed limit will be lowered by 5 mph and new signs indicating this change will be put up. Jerome Avenue, will be joined by thirteen additional new Arterial Slow Zones in the program’s second phase: Coney Island Avenue, Utica Avenue, and Flatbush Avenue & Flatbush Avenue Extension in Brooklyn; Roosevelt Avenue and Metropolitan Avenue in Queens; Victory Boulevard in Staten Island; 7th Avenue, Amsterdam Avenue, Bowery, Houston Street, Park Avenue, and Avenue of the Americas in Manhattan and Third Avenue in the Bronx. In total, dangerous speeding will be reduced on more than 65 miles of major corridors that have seen 83 fatalities. “Slow Zones are a critical and widely endorsed element of Vision Zero,” said DOT Commissioner Trottenberg.

    The 2005 Transit Strike : Transportation Impacts and Analysis Analysis and Findings During the course of the Transit Strike, the Department of Transportation (NYCDOT) implemented a comprehensive data collection and monitoring program. NYCDOT synthesized this data to understand the regional travel patterns by various modes of travel during the course of the strike. These findings are summarized in the following section. This program focused on providing the following components: • Traffic Patterns in the Manhattan Central Business District (CBD) • Vehicle entries and exits into and out of the CBD • Vehicle volumes on key arterials and limited access highways • Vehicle occupancy and classification • Vehicle Travel Times and Speeds • Traffic Patterns in the Boroughs • Vehicle volumes on limited access roadways • Vehicle speeds • Pedestrian and Bicycle Travel Trends • Functioning Transit Service • Waterborne Transportation • Private Bus Lines • Commuter Rail/PATH Service • Taxicab Usage 34 © 2006 Adam Fields, http://www.adamfields.com All Rights Reserved The 2005 Transit Strike : Transportation Impacts and Analysis Traffic Patterns in the Manhattan Central Business District (CBD) One of the primary goals of the 2005 Strike Contingency Plan was to limit vehicle entries into Manhattan and maximize vehicle occupancy. The Manhattan Central Business District (CBD) represents one of the primary trip generators in the city with the high rate of employment, land use density and number of trips into and out of the area. The Department frequently tracks traffic patterns in the CBD, so there was a good foundation for existing or before numbers. Vehicle Entries into the Manhattan CBD The implementation and enforcement of the HOV-4 restriction during the 5-11 AM period had a dramatic effect on controlling vehicle entries into the Manhattan CBD.
