CID 2012 The Clinical Significance of VAN MIC in S.aureus Infections: A Systematic Review and Meta-analysis. Conclusion • Patients with MRSA BSI and higher vancomycin MIC values by Etest have a higher likelihood of mortality and treatment failure. • The cause is not well defined; most likely reflects an interaction among pathogen-specific variables, host responses, and suboptimal vancomycin exposure. • Non vancomycin anti-MRSA therapies should be considered for patients with MRSA BSI with high vancomycin MIC, especially for values >2.0 μg/mL by Etest. • There are currently no data to support better survival rates with alternative antibiotics for MRSA BSI. van Hal SJ et al. EID 2011 #Catheter kept at least 7 days since isolation of MSSA in blood cultures. Antibiotic choice may not explain poorer outcomes in patients with S aureus bacteremia and high VAN MIC concentrations. • We assessed 532 patients with SAB from 8 hospitals. • All patients with MRSA bacteremia were treated with VAN, and patients with MSSA bacteremia received either flucloxacillin or VAN. Increasing VAN MIC was associated with increased mortality in VAN-treated patients. • However, even in patients with MSSA bacteremia treated with flucloxacillin, mortality was also higher if the vancomycin Etest MIC of their isolate was >1.5 μg/mL, compared with those with lower MIC isolates (26.8% vs 12.2%; P < .001). • After adjustment in a multivariate model, age, hospital-onset SAB and vancomycin MIC were independently associated with mortality, but methicillin resistance and antibiotic choice were not. Efficacy assessed following 4 days of treatment by colony counts of infected bone tissues (joint fluid, femoral bone marrow and bone). • Vancomycin was ineffective against the MRSA strain and poorly active against GISA infections in this model. • Ceftaroline and linezolid demonstrated significant activity in bone marrow and bone, and were significantly better than vancomycin treatment. • Ceftaroline was the only drug to exhibit significant activity against MRSA in infected joint fluid. In this case, the microbial degrada- ion of AB-PC, caused by penicillinase (PC-ase), was strongly inhibited in the presence of MCI-PC. The enzymatic hydrolysis of AB-PC by extracellular nd cell-bound PC-ase, obtained from an AB-PC resistant strain of Staphylo- coccus aureus, was also inhibited in the presence of MCI-PC. Against that, t a under comparable conditions, penicillin G (PC-G) was almost completely destroyed by cell-bound PC-ase from this organism. An in vivo synergism was observed when mixtures of AB-PC and MCI-PC were applied for the treatment of mice challenged with a clinically isolated strain of E. coli resistant to both of these penicillins. Similar results were obtained in the treatment of mice experimentally infected with a strain of Staphylococcus aureus, which is highly resistant to AB-PC, but sensitive to MCI-PC. A decrease in the rate of hydrolysis of methicillin (dimethoxyphenyl penicillin, DMP-PC) by staphylococcal penicillinase (PC-ase) was first noted by Rolinson and his co-workers^.

The fosfomycin resistance rate was 6.7% and was significantly higher in LTCF-onset isolates than community-onset and hospital-onset isolates. Twenty-one sequence types (STs) were identified among 19 fosfomycin-resistant E. coli isolates, showing diverse genotypes. fosA3 was found in only two isolates, and diverse genetic variations were identified in three genes associated with fosfomycin resistance, namely, GlpT, UhpT, and MurA.

THE PIPELINE REPORT Pipeline for Gonorrhea, Chlamydia, and Syphilis By Jeremiah Johnson Introduction The current toolbox for addressing gonorrhea, chlamydia, and syphilis is inadequate. At a time where all three epidemics are dramatically expanding in locations all around the globe, including record-breaking rates of new infections in the United States, stakeholders must make do with old tools, inadequate systems for addressing sexual health, and a sparse research pipeline of new treatment, prevention, and diagnostic options. Lack of investment in sexual health research has left the field with inadequate prevention options, and limited access to infrastructure for testing and treatment have allowed sexually transmitted infections (STIs) to flourish. The consequences of this underinvestment are large: according to the World Health Organization (WHO), in 2012 there were an estimated 357 million new infections (roughly 1 million per day) of the four curable STIs: gonorrhea, chlamydia, syphilis, and trichomoniasis.1 In the United States, the three reportable STIs that are the focus of this report—gonorrhea, chlamydia, and syphilis—are growing at record paces. In 2017, a total of 30,644 cases of primary and secondary (P&S) syphilis—the most infectious stages of the disease—were reported in the United States. Since reaching a historic low in 2000 and 2001, the rate of P&S syphilis has increased almost every year, increasing 10.5% during 2016–2017.
