Dear Applicant,

Thank you for expressing an interest in working for The Royal Wolverhampton NHS Trust (RWT). We hope that after reading the enclosed information, and perhaps making contact informally, you will feel enthusiastic about applying to join our organisation.

RWT in partnership with The University of Wolverhampton, Academic Institute of Medicine (AIM) is excited to have developed the Clinical Fellowship Programme. Applications are invited for Fellowship posts to work across acute/general medicine and its specialties to commence in August 2016.

Appointments are for 12 months in the first instance with the possibility of extension to 2 years. The posts are being offered at 1st year Core Trainee level or higher and the appointment would be at a grade based on the qualifications and experience of the applicant.

Successful applicants will work in a dynamic friendly environment in general and acute medicine combined with a medical specialty of their choice as expressed within the application form.

Opportunities exist to work in Emergency Medicine within the Urgent and Emergency Care Centre (UECC).

Although the posts are of non-training grade, there will be direct Consultant supervision and an educational programme including annual appraisal, 4-monthly supervisor meetings, access to an e-portfolio, study leave and a study leave budget.

There will be a specific programme of protected teaching and a wide range of other clinical training activities including Core Curriculum Teaching, PACES teaching, a weekly grand round and departmental specific meetings. Post holders will be encouraged to undertake audit, quality improvement projects, to pursue Membership examinations in their chosen specialty and opportunities in medical education or research.

Post holders would be strongly encouraged to register for a Masters qualification in General Medicine or one of its specialties with our partner Postgraduate Medical School, the University of Wolverhampton Academic Institute of Medicine (AIM). Subject to satisfactory progress, this will be fully funded and protected time will be allocated. As part of this it is expected the post holder will undertake a clinical project leading to presentations at meetings and publications.

Undergraduate Medical Education

We are a teaching Trust of the University of Birmingham and we aim to provide high quality undergraduate medical education to inspire our future doctors to deliver excellent medical care and patient safety.

The Academic Institute of Medicine

The University of Wolverhampton and The Royal Wolverhampton NHS Trust have formed a strategic partnership to encompass the breadth of medical education and research encapsulated within a postgraduate medical school environment. Doctors can simply affiliate with AIM to access the substantial online learning resources of the University, join speciality groupings and networks, or attend CME events and conferences. If they register for a higher degree, they will become Fellows of AIM. Postgraduate doctors at any level can access Masters level qualifications in Medical Education, Clinical Leadership, Research and Evidence Based Practice or in the speciality of their choice. Such provision will be tailored and negotiated to fit with the doctor’s development and training needs and be targeted to their requirement for career progression.

Research & Development

The Trust has a busy, proactive Research & Development Directorate with an overall objective to improve patient care, treatment and quality of life by the performance and dissemination of clinical research and innovation.

Formed in 1998, the R&D Directorate has grown rapidly in parallel with the development of the Trust’s research and innovation culture. The R&D Clinical Director and management team are supported by a team of experienced research nurses and administrators who operate efficient and effective systems foresearch r management and governance. We aim to increase the opportunities for local patients and the public to participate in and benefit from, research.

The Directorate supports all research conducted within the Trust from questionnaire studies to randomised controlled trials and the Trust acts as sponsor for a variety of own account investigator led studies, guiding and streamlining the management of research across many clinical areas. The Directorate has established links with a number of Universities and other research partners and has a wealth of experience conducting commercial research.

11 Clinical Fellowship Programme

Medical Training Framework Induction

There will be a full Trust induction for all clinical fellows commencing the clinical fellowship programme.

Continued Professional Development.

The opportunity for Continued Professional Development (CPD) is supported and encouraged by the Trust, within funding, time and available facilities.

Revalidation and Appraisal

Medical revalidation is the process by which the General Medical Council (GMC) confirms the continuation of a doctor’s licence to practise in the UK, provides greater assurance to patients, the public, employers and other healthcare professionals that licensed doctors are up-to-date and fit to practise. It is a key component of a range of measures designed to improve the quality of care for patients.

The Trust fully supports the GMC’s Revalidation agenda and we have a dedicated Revalidation Support Team who: • Support the co-ordination of Revalidation and medical appraisal activity, • Provide personalised support for all doctors, which includes the facilitation of 360° Multi- Source Feedback, • Provide advice and guidance on completing appraisal documentation and sourcing evidence.

Education and Training

Successful candidates will have an educational framework equivalent to training grade doctors. There will be a named Educational and Clinical supervisor with 4-monthly formal supervisor meetings. Annual appraisal would include 360° appraisal and Personal Development Plan. The post holder would have access to internal and external study leave with a defined study leave budget and access to e-portfolio from their equivalent College. Throughout their tenure post holders will be supervised and guided by Consultants who will provide clinical training during day to day working and at Supervised Learning Events. Further clinical training will be provided during team educational meetings and multidisciplinary meetings.

Teaching Experience

Candidates will have an opportunity to actively participate in medical student teaching programmes. This role can be expanded for those candidates who have an interest in teaching and wish to obtain greater experience and qualifications in Medical Education.

Audit and Clinical Governance

The Trust promotes evidence based practice, professional development and safe and effective clinical care through Clinical Governance. There is a fully resourced Audit Department with a well-organised team and there is regular, on-going clinical audit. Clinical audit has a strong base within the Trust and this has been incorporated into a robust clinical governance structure. This is being developed with the Trust Board having an increasing focus on the monitoring of clinical outcomes.

Research

For candidates with previous research background or those with interest in research, appropriate speciality attachment, supervisor, training and support would be provided.

Higher qualifications

Candidates will also be encouraged and supported to pursue Royal College Membership examinations in their chosen specialty. During the two years, subject to satisfactory progression, post holders would be expected to register for a Master level qualification in General Medicine or one of its sub-specialties with our partner Postgraduate Medical School, the University of Wolverhampton AIM. This will be fully funded and protected time will be allocated. As part of this it is expected the post holder will undertake a clinical or service- based project leading to presentations at specialty meetings and publications. To attain a Post Graduate Certificate would require tenure of at least 1 year and to attain a full Masters, no less than 2 years.

Career Progression

It is the intent of the service that, at the end of their tenure, the post holder will be fully prepared and highly marketable for their next career step by virtue of their clinical experience, educational portfolio and higher qualification.

13 Clinical Fellowship Programme

The posts, who can apply, programme and the intended outcomes

All posts would are fully paid substantive posts within the NHS with a very clear medical education framework including the opportunity to extend to Masters level accreditation.

Grade and Specialty

Applications are for 1 to 2 year posts, depending on experience, grade and preference, in general and acute medicine in linkage with a variety of medical subspecialties. Appointments would be at a level equivalent to junior or senior hospital residents depending on the level of previous training. Posts are available in the majority of the specialties of Internal Medicine.

These posts would suit doctors from the UK or abroad under the MTI scheme that may be at various stages of their career and would include:

• Doctors who have completed their basic medical degree (MBBS) and the Foundation Programme, or 2 – 3 years of overseas clinical experience post-qualification / post registration (more junior resident)

• Doctors who have completed a recognised post-graduate qualification in General Medicine and are seeking further specialization or qualification or both (more senior resident)

• Doctors who have attained specialist status in any of the medical specialties and seek further specific experience and research opportunities (established practitioner)

These tiers would map to UK NHS grades as outlined in Appendix A.

The programme is standard for both UK and Overseas doctors, there being no differentiation or discrimination.

We are looking for the highest calibre and quality doctors. Such doctors are seeking to gain further experience, skills escalation, career progression and higher qualifications. Currently we are focused on Medicine and its subspecialties but opportunities in Surgical specialities, Paediatrics, Obstetrics and Gynaecology and in Primary Care are available. The grade of doctor that this would suit is described in Appendix A.

Overseas doors will come to the UK under the UK MTI initiative. If applicants are accepted to the programme following the application and interview process, and if there are suitable posts available for them, they will then be offered a fully paid post with the partner NHS organisations, currently the Royal Wolverhampton Hospital NHS Trust.

The terms and conditions of employment will be exactly equivalent to a UK NHS doctor of the same grade. The NHS will pay their salary and their remuneration will be exactly the same as an equivalent UK doctor.

All doctors will be given a full medical education programme. This will be tailored and matched to each doctors training and career progression needs. The programme will be under full programme management of Clinical Tutor and other senior clinicians of the NHS Trust and under the direct authority of the Medical Director. Subject to satisfactory progress within this programme, RWT will fully fund a Masters level qualification being a Masters in Clinical Medicine with the Academic Institute of Medicine. The value of this enhanced training opportunity which might amount to a maximum of £10,000. Clinical Fellows also have the opportunity to sit their Royal College Examinations, which they must self-fund.

Fellows will stay in the programme for up to 2 years. At the end of the programme, all Fellows, whether UK or Overseas, will have a full portfolio of evidence of clinical experience, appraisal and assessment and, if they chose to take them, a Masters level higher qualification and Royal College exams.

As for UK applicants, oversees doctors will be responsible for their own relocation and living expenses.

15 Clinical Fellowship Programme

Overview of Terms and Conditions Terms and Conditions of Service

The successful appointee will be employed by the Royal Wolverhampton NHS Trust subject to the National Terms and Conditions for NHS Medical and Dental Staff (England) 2002 as amended from time to time. This job description is not exhaustive and may be updated from time to time, with consultation, as the needs of the service dictate.

Remuneration and Contract of Employment

The specific final detail of the post will be assessed, agreed and matched during the processes of application and interview to the applicant’s medical training experience and needs. This will be agreed prior to appointment and form the contract of employment. This will include grade, pay scale, working hours and banding, and the mix and balance of clinical experience. Clinical Fellows, whether from the UK or overseas can expect to have the same UK national standard terms and conditions as the equivalent UK training grade doctors.

Tenure

The appointment is a fixed term, full or part time position. The notice period is one month on either side. Post may be of 1 – 2 years duration. Annual Leave The annual leave entitlement is 27 working days per annum depending in seniority. In addition, there is an entitlement to 8 days Public Holiday per annum. Personal leave years will run from date of appointment. At least six weeks’ notice is required before taking annual leave.

Study Leave

At least six weeks’ notice is required before taking study leave.

Superannuation

This post is superannuable under the NHS Pension Scheme for the main contract only. Banding supplements are not superannuable.

Registration

Candidates must have full registration and a licence to practise with the GMC for the duration of the appointment.

Travelling Expenses

Travelling expenses are paid in accordance with the TCS. Interview expenses will be paid; however if a post is offered and subsequently refused, expenses will not be reimbursed.

Interviewed candidates travelling from outside the UK (this includes Eire) will be entitled to

16 Recruitment Information Pack travelling and subsistence expenses but only in respect of the journey from the point of entry in the UK to the interview location.

Accommodation & Removal Expenses

Single accommodation is available for which a charge will be made. Some family accommodation may be available and again a charge is made for this accommodation.

Relocation and / or removal expenses are paid in accordance with the Trust’s Policy on Relocation Expenses.

Interview accommodation is available on request.

Car Parking

Car parking is available and it is the condition of employment that all employees who use these facilities purchase a car permit and pay the required fee.

Health and Safety

All employees of the Trust have a responsibility to abide by the safety practices authorised by the Trust. They have an equal responsibility with the management for maintaining safe working practices.

Infection Prevention and Control

All healthcare workers have an overriding duty of care to patients and are expected to comply fully with best practice standards. As a senior medical staff member you must act as a role model in reducing all risks of patient harm. You have a responsibility to comply with Trust

policies for personal and patient safety and for the prevention of Healthcare Associated Infection (HCAI). This includes a requirement for rigorous and consistent compliance with Trust policies for hand hygiene, use of personal protective equipment, safe disposal of sharps, dress code and compliance with mandatory training requirements. Knowledge, skills and behaviour in the workplace should reflect this; at annual appraisal you will be asked about application of practice measures known to be effective in reducing HCAI.

Safeguarding

All employees have a responsibility to support the safety and well-being of children, young people and adults at risk of harm and to practice in accordance with legislation. Knowledge, skills and competency are to be maintained according to role and responsibilities in order to fulfil Safeguarding Children and Adults at Risk responsibilities. All employees are expected to comply with existing local Safeguarding policies and procedures, and Trust and Wolverhampton Safeguarding Children Board and Safeguarding Adults at Risk requirements.

19 Clinical Fellowship Programme

The City of Wolverhampton

Wolverhampton is a modern industrial city situated on the western fringe of the Black Country. It is compact, with no boundary more than four miles from the centre. It is approximately a 30 minute drive to Birmingham (15 miles) and 1 hour 20 minutes to Manchester (60 miles).

Wolverhampton is an urban, multi-ethnic city with a population of around 250,000. This is made up of 77.79% white and 22.21% black and other ethnic minorities. The wider hospital catchment area has a population of in excess of 350,000.

Shopping in Wolverhampton offers a varied selection of facilities and amenities. There are modern shopping precincts (the Mander Centre and the Wulfrun Centre) with further shopping areas in Darlington, Victoria and Streets along with Queen Square. Shopping is also available in nearby conurbations, including Birmingham.

There are the usual public amenities found in cities. The Civic and Wulfrun Halls are used for concerts and promotions. In addition, there is an Art Gallery and a good public library. The Grand Theatre in the city centre has been extensively refurbished. The city centre has an active night life, offering a variety of night clubs, bars, restaurants and a cinema. Sporting facilities include Wolverhampton Wanderers Football Club, several health and fitness clubs as well as clubs for cricket, hockey, rugby, tennis, squash, athletics, and sailing. In addition there is an all- weather racetrack for horses and at least six golf clubs are within easy reach.

Accommodation and Education

The main residential areas within the city are situated on the western boundary and include Tettenhall, Wergs, Wightwick, Compton, Finchfield, Merry Hill and Penn. In addition there is a more modern housing development at Perton which offers choice at all price levels. This development has its own supermarket and supporting shops, schools and churches which makes this area a self-contained community.

Outside the city there are numerous villages, both small and large, which offer attractive opportunities for housing. They include Beckbury, Ryton, Burnhill Green, Pattingham, Worfield, Claverley, Seisdon, Trysull and Albrighton to the west, Brewood and Codsall to the north and Womborne to the south. There is further choice of residential areas between Wolverhampton and Birmingham if family needs so require.

Education is well catered for with many excellent maintained schools, Wolverhampton College and the University of Wolverhampton. Independent schools include the Wolverhampton Grammar School, the Royal Wolverhampton School, Tettenhall College, St Dominics (Brewood), Birchfield Boys Preparatory and Newbridge Mixed Preparatory Schools.

The Environs

To the south east is the Black Country (with the major urban areas of Dudley, Walsall and Wolverhampton) which used to be the industrial heartland of England. There is a working Black Country Museum at Dudley, which offers an opportunity to see the cultural heritage of

Facilities there include theatres, concert halls and the major sporting facilities of Aston Villa, West Bromwich Albion and Birmingham City Football Clubs, Warwickshire County Cricket Club at Edgbaston and the Alexander Athletics Stadium. There is also the Barclaycard Arena (NIA) and National Exhibition Centre which host many national and international events.

The nearby countryside of Staffordshire and Shropshire is delightful. To the north is Cannock Chase and to the west the Shropshire borders with attractions of , the Clee Hills, the , Wenlock Edge, and the Severn Valley. South of Wolverhampton, the City of Worcester is easily accessible, and to the west north- can be reached in just over an hour.

The area has a large network of canals, the majority renovated, offering adjacent housing and popular for narrow boat holidays.

Transport Links

Travel links to Wolverhampton are excellent. The area is well served by the motorway network, with the M5, M6 and M54 within easy reach. There are regular direct rail services to London Euston (journey time about 2 hours) and to Manchester (journey time about 1 hour). Cross country rail services to many parts of England and Scotland are operated by Virgin Trains which has its hub in Birmingham.

Birmingham International Airport is 30 minutes away by road and Manchester International Airport can be reached within 1 hour 20 minutes.

Clinical Fellowship Job Description & Person Specification Clinical Role

The post holder will be working with a team of Consultants, Specialist Trainees, Clinical Fellows, Core Trainees and Foundation Year Trainees. Each post has a generic clinical component in Acute/General Medicine and the duties will be on the ED/MAU as described in the information pack. In addition the post holder will have an agreed experience in the specialty of their choice including outpatient clinics, ward cover, specialist in-patient reviews and procedures, audit and research. The precise role within each team and the proportion of clinical time spent between the two components would depend on previous experience and qualifications of the candidate and the specialty chosen. For candidates with more than 3 years of post-qualification experience, MRCP or equivalent qualifications or full timeesearch r experience, Senior Fellowship position can be offered.

The equivalent grades of appointment are laid out in Table 1.
