LA BO RATO RIES, INC. 2021 Montana Meeting While this course is designed for both podiatrists and non-podiatrists alike, those who are not podiatric doctors, or those podiatrists who do not want the contact hours, can opt for a package without lectures. There are no prerequisites in order to participate.

The Goldfarb Foundation is approved by the Council on Podiatric Medical Education as a provider of continuing education in podiatric medicine. The Goldfarb Foundation has approved this activity for a maximum of 12 continuing education contact hours.

COVID-19 Waiver Form Signature Required The 2020 Montana Meeting was held as a face-to-face event at the Doubletree in complete compliance with local mandates. Social distancing and the requirement of face masks in the hotel and face coverings while fishing were enforced. At the time of the 2021 meeting, the appropriate mandates will be followed. This could include traveling parties being seated together for the lectures and meals, face mask requirements, and daily temperature checks. In order to register for the 2021 Montana Meeting, attendees are required to sign the COVID-19 Waiver of Liability and Indemnification (https://tinyurl.com/MontanaWaiver). Upon signing the form, you can register to participate in the meeting by visiting the Online Store and selecting either the Single or Double Occupancy option. Note that registrants who select a package that does not include lodging will find their options under the Single Occupancy category. Registrations will not be processed until the waiver is signed. DEPOSIT: A 50% non-refundable deposit is due by Friday, April 16, 2021, to reserve your space at the meeting; full payment is due by Friday, July 16, 2021. In order to guarantee your reservation at this meeting, your deposit MUST BE received by APRIL 16. After APRIL 16, reservations will be taken based on availability at that time. No reservations will be taken after Friday, JULY 16, 2021. ADDITIONAL FISHING: Those who would like to add additional days of fishing to their package can contact the outfitter directly to make those arrangements and payment. ADDITIONAL HOTEL NIGHTS: Additional hotel nights must be arranged directly with the Doubletree Missoula-Edgewater at 406-728-3100. The hotel rate of $124 (single) plus taxes and other fees per night is available for the three days before and after the Montana Meeting. EXCLUSIONS: Air transportation to Missoula, flies, fishing licenses, gratuities for guides, incidental room charges, personal items, meals not included in package (see below), and additional recreational activities.

FISHING INCLUDED: The guided fishing is for 3 days—Thursday, Friday, and Saturday. Lunch is provided by the tour company on fishing days. Guided fishing are reserved for two people per boat; single reservations for participation made after June 1 may incur an additional charge if an extra boat for just one person is required.

LODGING INCLUDED: At the Doubletree for Wednesday, Thursday, Friday, and Saturday nights.

MEALS INCLUDED: Wed.–Dinner; Thurs.–Breakfast & Dinner; Fri.–Breakfast & Dinner; Sat.–Breakfast & Dinner.

Final payment is due by Friday, July 16, and absolutely no registrations for this event will be accepted after July 16. Registrations received between April 16 and July 16 are an additional $100 fee as shown on the fee chart.

Refund Policy The following refund policy applies to cancelled registrations for this meeting: Prior to April 16: 75% of payment refunded April 17–July 16: 25% of payment refunded After July 16: 0% of payment refunded

The Goldfarb Foundation reserves the right to cancel any meeting, or aspect of any meeting, at any time for any reason. The Foundation will provide as much cancellation notice to registered attendees as possible, and post the cancellation notice on our website for a reasonable amount of time. A refund will be made for registrations received in the manner in which it was received. Any expenses incurred by a registrant for a cancelled event (airfare, hotel, car rental) will not be refunded by the Foundation. Registrants may want to consider purchasing personal insurance to insure his/her expenses.

Seminar Objectives (subject to change as lectures confirmed)

At the conclusion of the Montana Meeting, attendees will be able to: 1. Compare and contrast wound care treatments and protocols. 2. Recognize physician burnout within themselves and prepare a treatment plan. 3. Treat various diseases of the foot and ankle as identified in podiatric periodicals that the learner may not be aware of.
