The Children’s and Young Person’s Clinical Business Unit is part of the division of Family Health.

Pilgrim Hospital, Boston

Pleasantly situated on the outskirts of Boston, Pilgrim Hospital is a modern general hospital, providing a full range of hospital services to a catchment population of approx. 200,000 rising to 250,000 during the summer, because of the holiday influx to Skegness and related resorts. The hospital has a total bed complement of 547.

The Paediatric Service

Currently, there is a Children and Young People Short Stay and Paediatric Assessment Unit (SSPAU) model of service delivery in place at Pilgrim Hospital. The Assessment unit is open 24/7, with maximum observation period of 12 hours for any child. Children who need inpatient care for >24 hours are transferred to Lincoln Hospital or any other hospital as per pathway. The SSPAU is staffed as per an inpatient unit.

There is a ‘consultant of the week’ system in place.

There are facilities to resuscitate and stabilise very sick children on the Unit before they are retrieved by the COMET team.

The Paediatric Outpatient department runs a number of general paediatric and specialist clinics (allergy, obesity, Cystic fibrosis, asthma), including some joint clinics with tertiary specialists(paediatric cardiology, nephrology ,neurology). The outpatient department is located on the main hospital corridor and has good play facilities.

There is a strong commitment to Postgraduate Medical Education with a robust teaching programme and simulation training sessions. This programme has very good feedback on the GMC survey.

Clinical Audits are done regularly and presented in departmental education meetings.

The Maternity & Neonatal Services

The maternity service is housed in a separate wing and has antenatal and postnatal beds, as well as the delivery suite. The Neonatal Unit (SCBU) is situated on the 4th floor on the main tower block adjacent to the SSPAU and has easy access to the labour ward. There are about 2000 deliveries a year. SCBU is a well-equipped modern Neonatal Unit with 2 intensive care cots (for short term ventilation and stabilisation prior to transfer to NICU) and 6 Special Care Cots. Also there is a 4 bedded Transitional Care Unit adjacent to SCBU. The Neonatal Service is functioning as a level 1 unit (SCBU) and works as part of a Trust wide Neonatal Service.

ULHT Neonatal service is part of the Trent Perinatal Network and works closely with the units within the Network. The Regional Perinatal Centre is based in Nottingham University Hospitals NHS Trust. There is a standalone neonatal transport system- CenTre, covering Trent Perinatal Network and Central New born Network which provides transport of neonates for ULHT.

All consultants cover both Neonatology and General Paediatrics when on-call. It would be preferable for the candidates to have recent neonatal experience in a tertiary.

The Obstetric Service has 8 consultant Obstetricians/gynaecologists. Junior doctors rotate between Boston and Leicester/Nottingham. There are good working relations with all Obstetricians and there are regular Perinatal Mortality and Morbidity meetings held every month.

Supporting Services

The Radiology department provides ultrasound, CT and MRI facilities. There is a bi- weekly MRI list for children needing general anaesthesia. There is a well-equipped Medical Physics department. Nuclear Medicine and Evoked Responses are available on site. Neonatal hearing screening is undertaken.

Paediatric Physiotherapy service is available on site. There is good support from a number of other departments. There are two Consultant Orthopaedic surgeons who have a special interest in Paediatrics. There is an Ophthalmologist who undertakes screening of preterm infants on the neonatal unit. There are ENT Consultants who perform common ENT surgical procedures in children.

General Hospital Facilities

An extensive range of general surgical, orthopaedic surgery, ENT, ophthalmology, maxillofacial, dental, obstetrics, gynaecological and oncological services are provided. In addition general medical and a wide range of medical subspecialty services are provided.

The hospital has close links with Nottingham and Leicester Medical schools providing undergraduate and postgraduate medical education. It has a modern postgraduate medical centre providing state of the art IT and educational facilities with an active postgraduate medical education programme.

Diagnostic facilities are comprehensive and the large Radiology Department has sophisticated x- ray facilities with modern CT and MRI scanners and investigative radiology services provided. There is also a fully developed Medical Physics Unit. The hospital is a lively, forward looking workplace with a strong identity and held in high regard within its community. It prides itself on providing a high standard of care and attention to the needs of patients and their families. Staff are well motivated and highly valued by management. Management encourage and support the professional development of staff. Medical and nursing staffs are active and represented in management throughout the organisation. Medical staff are encouraged to actively participate in management at all levels. Clinical Governance, Clinical Effectiveness, Clinical Audit and Risk Management are actively supported by management.

Lincoln County Hospital

The Paediatric Service

Rainforest Ward is a 24 bed Paediatric Ward for Medical as well as Surgical in-patients. In addition, Safari Day Unit works as an 8 bed Day Case unit. Both the paediatric wards have been modernised recently. The Paediatric Outpatient department has good play facilities and is located close to the Safari Day Unit. All the consultants pursue a subspecialty interest and where possible, we hope to increase the range of specialisms available within the department. In addition we welcome a number a Tertiary Specialists for joint clinics.

There is a strong commitment to both Undergraduate & Postgraduate Medical Education. Regular teaching sessions are led by all consultants. The consultant staff lead regular teaching of medical students, as well as junior medical staff, nurses midwifes and other staff.

Medical students from Nottingham are attached to the Department and do a 10 week attachment in Lincoln. There is Teaching Fellow in Paediatrics who supports the medical students. Clinical Audit sessions are held once a month.

The Maternity & Neonatal Services

The maternity service is housed in a separate wing and has antenatal and postnatal beds, as well as the delivery suite. The Neonatal Intensive Care Unit (Nocton Ward ) is situated on the sixth floor immediately above the Delivery Suite. There are about 3000 deliveries per year. The NNU was refurbished in 2017and is a well-equipped modern Neonatal Unit with 2 intensive care cots and 15 Special Care Cots. There is a separate 24 hour Tier 1 cover for the NICU comprising of ANNPs and junior clinical fellows. The department has sought to train and employ Advanced Neonatal Nurse Practitioners.

The unit works closely with the Neonatal Unit located in Pilgrim Hospital, which is part of ULHT. ULHT Neonatal service is part of the Trent Perinatal Network and works closely with the units within the Network. Regional Perinatal Centre is based at Nottingham University Hospitals NHS Trust. There is a standalone neonatal transport service- CenTre, covering Trent Perinatal Network and Central Newborn Network which provides transport of neonates for ULHT.

Grantham and District Hospital

Regular paediatric clinics are held in Kingfisher children’s unit in Grantham Hospital where clinicians from Boston and Lincoln do clinics. A Paediatric Cardiologist from Leicester and a Paediatric Nephrologist from Nottingham do outreach clinics in Grantham. Kingfisher unit also acts as a hub for community paediatric services.

A Community Midwifery service is provided for the locality. Antenatal, Postnatal, Gynaecology Outpatients and Day Care services are offered at Grantham.

ON CALL COMMITMENT

On call 1 in 8 including week days and weekends. On-call availability attracts a 5% supplement. The on call is prospective. During on call, you are expected to cover both neonatal and paediatrics emergencies and be responsible for the patients.

Professional Development

Specialty Doctors in Paediatrics are expected to register and maintain registration with the Royal College of Paediatrics and Child Health for CPD. The Trust will, within the existence of the service, support study leave to enable College CPD requirements to be met. The post holder is expected to have a formal appraisal annually.

Study leave applicable to Specialty Doctors for CPD is 30 days granted every 3 years.

Job Description

POST: Trust ST1/ST2/ST3 Doctor in Paediatrics SITE: Pilgrim Hospital, Boston MANAGERIALLY ACCOUNTABLE TO: PROFESSIONALLY ACCOUNTABLE TO: Clinical Lead TENURE: Fixed Term MANAGES: N/A

Job Purpose This is a post in general and neonatal paediatrics. The appointee will work alongside junior medical team to provide an eight person shift rota. The post is fixed for 12 months with a possibility to extend.

During the period wide basic paediatric experience is gained. There are weekly Paediatric Meeting and Teaching Sessions. There are regular Audit Meetings held within the Department and jointly with the Accident and Emergency Department and the Obstetricians.

The Hospital Paediatric Department is based at Pilgrim Hospital in Boston and consists of one Paediatric Assistant Unit (PAU), a purpose-built Paediatric Out-Patient Department and Neonatal Unit. It is a condition of the appointment that the post holder will be willing to work in any of the Trust’s locations or outreach district general hospital clinics.

There are 8 Consultant Paediatricians based at Pilgrim Hospital, with 5 WTE currently in post with the remainder of the posts covered by locum staff. The first on-call consists of 8 F2/ST1-3 or equivalent level doctors. Second on- call cover consists of 8 Specialty Doctor/ST4 and above level doctors.

Both tiers of staff operate full shift rota with prospective cover, covering both general paediatrics and the Neonatal Unit. The junior staff rotate through neonatal, general paediatrics and outpatients, rather than working for specific consultants.

DUTIES

1. GENERAL PAEDIATRICS: The in-patient service is based on one the Paediatric Assessment Unit. The ST1-3 may be called out to Accident & Emergency to give advice or deal with emergencies. The Out- Patient Department is based at the Pilgrim Hospital and, depending on grade, the post holder will be expected to attend Out-Patients regularly. These sessions include Boston-based and visiting Consultants.

2. NEONATAL PAEDIATRICS: The tier 1 doctors (this post) are supervised by the tier 2 doctors (ST4 and above/Specialty Doctors) at resuscitation on the labour suite, on the post natal wards and on the neonatal unit. The unit currently takes neonates up to 34 weeks. Infants with surgical problems and extreme prematurity are transferred to either Lincoln County Hospital or sub regional centres. Further experience of practical procedures will be gained.

3. TEACHING: The ST1-3’s are occasionally involved in teaching Nursery Nurses, Student Nurses and Midwives.

This Post will be part of 8 ST1/FY2's will undertake first on-call cover, on a full shift rota.

Please note this post is not recognised for training by the Deanery and will not count towards any national training programme. This post will count towards service only.

Recommended Off Duty and Cover Arrangements

In accordance with the Terms and Conditions of Service Hospital Medical and Dental Staff (England and Wales) and as recommended in Health Circular DS 378/75 the first on call will:

(a) Be expected in the normal run of his/her duties and within his/her contract and job description, to cover for the occasional brief absence of colleagues as far as possible.

(b) Be expected to provide cover for the annual and study leave of colleagues, providing always that the resulting increase in duties is reasonable in the circumstances.

(c) Be expected to perform duties in occasional emergencies and unforeseen circumstances without additional remuneration. Should this occur outside of normal contracted hours the Doctor will be given time off in lieu.

The appointee accepts that he/she will perform duties under these provisions, at the request of the appropriate consultant, in consultation when practicable with his/her colleagues, both Senior and Junior.
